 FAILED COMMENTS FROM THE 1ST REVIEW ARE COPIED BELOW
 WITH ADDITIONAL NOTES
  
 1) FAILED. P101: THE PLAN APPEARS TO BE A COPY AND DOES
 NOT HAVE A VERIFIABLE ARCHITECT'S SIGNATURE AND SEAL.
 PLEASE PROVIDE AN ORIGINAL DIGITALLY SIGNED AND SEALED
 PLAN IN ACCORDANCE WITH FL STATUTE 481.221 AND SEC.
 61G1-16.005 FAC. THE CONTRACTOR MAY ALSO SUBMIT HIS OWN
 PLUMBING PLAN- SEC. 107.2.1 WPB. THE NEW PLAN DOES NOT
 HAVE A VERIFIABLE DIGITAL SIGNATURE AND SEAL APPARENTLY
 DUE TO AN ERROR EITHER IN THE SIGNING OR UPLOADING
 PROCESS. A ERROR MESSAGE IN A WINDOW APPEARS WHEN
 TRYING TO CLICK ON THE SEAL FOR VERIFICATION WHICH
 TERMINATES THE PROCESS.
  
 2) FAILED. NO RESPONSE WAS GIVEN TO CLARIFY IF THE
 SHOWER IS A BATHTUB/SHOWER CONVERSION. THE PLAN APPEARS
 TO SHOW A CONVERSION WHICH WILL FAIL AGAIN FOR FAIR
 HOUSING COMPLIANCE AS STATED BELOW. THE CLEAR FLOOR
 SPACE SHALL BE ALIGNED PARALLEL TO THE SHOWER AND FLUSH
 WITH THE CONTROL SIDE. PLEASE NOTE THE $1500.00 JOB
 VALUE APPEARS LOW FOR A BATHTUB/SHOWER CONVERSION- SEC.
 109.3 WPB. CLARIFY IS REPLACEMENT OF THE WALL TILE
 BACKER BOARD AND TILE IS PART OF THE PROJECT. A BULIDNG
 DUB-PERMIT IS REQUIRED FOR INSTALLATION OF THE BACKER
 BOARD- SEC. 105.1 WPB.
  
 FAIR HOUSING ACT COMPLIANCE: THE ONE CITY PLAZA
 CONDOMINIUMS AT 801 S. OLIVE AVE. ARE SUBJECT TO
 COMPLIANCE WITH FAIR HOUSING ACCESSIBILITY GUIDELINES
 WHICH CAME INTO EFFECT FOR MULTIFAMILY HOUSING BUILT
 FOR FIRST OCCUPANCY AFTER MARCH 13, 1991. PLEASE
 CLARIFY IF THE PROJECT IS A BATHTUB TO SHOWER
 CONVERSION OR AN EXISTING SHOWER UPGRADE. A BATHTUB TO
 SHOWER CONVERSION SHALL COMPLY WITH ITEMS #2 & 3 BELOW.
 PLEASE NOTE: AS AN ALTERNATIVE TO MEETING THE
 GUIDELINES FOR THE BATH ALTERATIONS, THE OWNER AND
 DESIGNER OF RECORD FOR THE PROPOSED PROJECT MAY PROVIDE
 A SIGNED AND NOTARIZED AFFIDAVIT WHICH WOULD INDICATE
 ACKNOWLEDGEMENT THAT THE PROPOSED DESIGN DOES NOT MEET
 THE REQUIREMENTS OF THE FAIR HOUSING ACCESSIBILITY
 GUIDELINES, BUT WILL ALSO REQUIRE THE OWNER TO AGREE TO
 REVERT THE UNIT BACK TO COMPLIANCE AT TIME OF SALE IF
 REQUESTED BY THE BUYER. IF THIS OPTION IS CHOSEN,
 PLEASE SEND AN EMAIL TO [email protected] AND I WILL SEND A
 COPY OF THE FAIR HOUSING AFFIDAVIT.
  
 2) REFER TO PAGES 7.56- 7.59 OF THE FHDA MANUAL AND
 PROVIDE A DRAWING FOR THE BATHROOM THAT SHOWS THE
 DIMENSIONS OF THE SHOWER AND THE REQUIRED 30X48-INCH
 CLEAR FLOOR SPACE DIAGRAM POSITIONED PARALLEL TO THE
 SHOWER AND FLUSH WITH THE CONTROL WALL. PLEASE NOTE IF
 THE SHOWER IS THE ONLY BATHING FIXTURE IN THE CONDO IT
 SHALL BE MINIMUM 36X36-INCHES IN SIZE. IT IS ALSO
 REQUIRED THAT NO OTHER FIXTURES SHALL OBSTRUCT THE
 CLEAR FLOOR SPACE REQUIRED FOR THE SHOWER WITH THE
 EXCEPTION OF A LAVATORY WHICH MAY OVERLAP BY 6-INCHES.
 THE LAVATORY SHALL BE A WALL HUNG WITH KNEE AND TOE
 CLEARANCES OR HAVE A REMOVABLE CABINET.
  
 3) PROVIDE AN ELEVATION DETAIL FOR THE SHOWER THAT
 SHOWS THE LOCATIONS OF REQUIRED GRAB BAR REINFORCEMENTS
 IN ACCORDANCE WITH PAGE 6.13.
  
 WHEN RESUBMITTING, IT IS HELPFUL TO PROVIDE A RESPONSE
 LETTER ADDRESSING EACH ITEM ALONG WITH THE CITY
 RE-SUBMITTAL FORM.
