 A) PROVIDE COPY OF THE FLAME SPREAD INDEX AND SMOKE
 DEVELOPED INDEX TEST DONE IN ACCORDANCE WITH ASTM E 84
 OR UL 723 AS REQUIRED BY SEC. 720.1 OF FBC 7TH EDITION
 (2020).
  
 RESPONSE:
 INFORMATION SUBMITTED IS FOR THE VINYL FLOORING PLANK.
 NEED THE TEST FOR THE SENTINEL PROTECT PLUS
 UNDERLAYMENT.
  
 ORIGINAL COMMENT:
 B) PROVIDE COPY OF THE SOUND TRANSMISSION CLASS (STC)
 TEST DONE IN ACCORDANCE WITH ASTM E 90 AS REQUIRED BY
 SEC. 1207.2 OF FBC 7TH EDITION (2020).
  
 RESPONSE:
 SAME INFORMATION FROM LAST TIME WAS SUBMITTED. NEED
 ACTUAL TEST REPORT FOR THE "SENTINEL "SENTINEL PROTECT
 PLUS UNDERLAYMENT .
 NOTE: PRODUCT SPECIFICATION SHEET FOR SENTINEL PROTECT
 PLUS UNDERLAYMENT SUBMITTED IS FOR 6" CONCRETE SLAB
 WITH DROP CEILING. THIS BUILDING DOESN'T HAVE A DROP
 CEILING. THEREFORE, NEED COPY OF TEST REPORT WITHOUT
 THE DROP CEILING.
  
 ORIGINAL COMMENT:
 C) PROVIDE COPY OF THE IMPACT INSULATION CLASS (IIC)
 TEST DONE IN ACCORDANCE WITH ASTM E 492 AS REQUIRED BY
 SEC. 1207.3 OF FBC 7TH EDITION (2020).
  
 RESPONSE:
 SAME INFORMATION FROM LAST TIME WAS SUBMITTED. NEED
 ACTUAL TEST REPORT FOR THE "SENTINEL "SENTINEL PROTECT
 PLUS UNDERLAYMENT .

 NOTE: TEST SAMPLES TO MATCH BUILDING FLOOR/CEILING
 CONSTRUCTION AND PROPOSED FLOOR FINISH. ALSO, THERE ARE
 TESTS FOR 6" OR 8" CONCRETE SLABS WITH OR WITHOUT
 ACOUSTICAL CEILING. TYPICALLY THERE ARE NOT SUSPENDED
 ACOUSTICAL CEILINGS IN THESE BUILDINGS.
