| 1) PROVIDE A SURVEY OF THE AREA SUBJECT TO THIS PERMIT. |
| THE SURVEY, OR A SITE PLAN, MUST SHOW THE SETBACK |
| DIMENSIONS FROM THE EXISTING SANCTUARY BUILDING AND THE |
| WEST PROPERTY LINE. |

| 2) THE FLOOR PLAN OF THE BUILDING DOES NOT MATCH THE |
| BUILDING APPROVED THROUGH THE SITE PLAN PROCESS. A NEW |
| DOOR HAS BEEN ADDED ON THE EAST SIDE OF THE BUILDING |
| THAT DIRECTLY CONFLICTS WITH THE APPROVED LANDSCAPE. |
| ALL THE PLANS MUST SHOW ACCURATE, CONSISTENT, CODE |
| COMPLIANT CONDITIONS. THE NEW BUILDING FLOOR PLAN IS |
| NOT SHOWN ON THE PLUMBING SITE PLAN, THE MECHANICAL |
| SITE PLAN, AND THE ELECTRICAL SITE PLAN. |

| 3) WHILE THE PARKING, LANDSCAPE, DUMPSTER ENCLOSURE AND |
| CHILLER PLANT ARE NOT PART OF THE REVIEW OF THIS |
| BUILDING PERMIT, THE PLANS SHOW CHANGES TO THESE |
| FEATURES FROM WHAT WAS APPROVED AS PART OF THE SITE |
| PLAN APPROVAL. A MINOR SITE PLAN AMENDMENT MUST BE |
| SUBMITTED TO THE PLANNING DIVISION AND APPROVED, PRIOR |
| TO ZONING APPROVAL OF THIS PERMIT. THE SITE PLAN |
| AMENDMENT REVIEW WILL INCLUDE REVIEWS BY THE CITY'S |
| SANITATION AND LANDSCAPE REVIEWERS TO VERIFY THAT ALL |
| CODE PROVISIONS STILL COMPLY. |

| 4) THE SITE PLAN APPROVAL, INCLUDING THE PALM BEACH |
| COUNTY TRAFFIC APPROVAL, ARE FOR A 17,733 SQUARE FOOT |
| BUILDING. THE PERMIT SHOWS A 17,740 SQUARE FOOT |
| BUILDING. THE MINOR SITE PLAN AMENDMENT MUST REFLECT |
| THIS INCREASE IN SQUARE FOOTAGE, TO INCLUDE AN UPDATED |
| PALM BEACH COUNTY TRAFFIC APPROVAL. |

| 5) THE SECOND LEVEL FLOOR PLAN SHOWS THE MAJORITY OF |
| THE SQUARE FOOTAGE AS OFFICE SPACE, WHICH IS NOT WHAT |
| WAS APPROVED AS PART OF THE SITE PLAN REVIEW, WHICH |
| CLASSIFIED THE ENTIRE BUILDING AS ASSEMBLY SPACE FOR |
| THE PROVISION OF PARKING. THE MINOR SITE PLAN AMENDMENT |
| MUST ACCURATELY REFLECT THE PARKING REQUIREMENTS FOR |
| THE OFFICE SQUARE FOOTAGE AND VERIFY THAT THE SITE |
| MEETS THE MINIMUM PARKING REQUIREMENTS. |

| 6) THE DIMENSIONS ON THE FLOOR PLAN DO NOT MATCH THE |
| DIMENSIONS ON THE SITE PLAN. THIS MUST BE UPDATED AS |
| PART OF THE MINOR SITE PLAN AMENDMENT. |

| 7) SEPARATE PERMITS WITH PLANS ARE REQUIRED FOR THE |
| FOLLOWING: |

| B. DUMPSTER ENCLOSURE. |
| C. CHILLER PLANT. NOTE: THE CHILLER ENCLOSURE MUST |
| SCREEN THE FULL HEIGHT OF THE CHILLER EQUIPMENT. |
| D. NEW GENERATOR. |
| E. BICYCLE RACKS. |

| 8) PRIOR TO THE REMOVAL OR RELOCATION OF ANY TREES ON |
| THE PROPERTY, A TREE ALTERATION PERMIT SHALL BE |
| SUBMITTED, REVIEWED AND APPROVED BY THE DEVELOPMENT |
| SERVICES DEPARTMENT, PLANNING DIVISION. |
