 PLEASE PROVIDE WRITTEN RESPONSES.
  
 1) THE SHEETS LABELED AS THE SURVEY AND SHEETS 023 OF
 029 THROUGH 029A OF 029 SHOW A DIFFERENT CONDUIT
 LOCATION THAN IS SHOWN ON THE COVER SHEET 001 OF 014
 AND THE OVERALL VIEW 02A OF 014. THIS LOOKS LIKE THERE
 ARE TWO DIFFERENT PROJECTS ALONG SAPODILLA AVENUE, ONE
 SOUTH OF FERN STREET AND ONE NORTH OF FERN STREET. ALL
 PLANS MUST SHOW THE SAME CONDITIONS AND SCOPE OF WORK.
  
 2) THE UPLOAD TITLES DO NOT MATCH THE SHEET NAMES. EX.
 TITLE 012 PLAN & PROFILE IS FOR SHEET 023 OF 029 AND
 TITLE 012A PLAN & PROFILE IS SHEET 23C OF 029. ALL
 PLANS MUST BE CORRECTLY LABELED. THIS FAULTY LABELING
 LED TO INCONSISTENT PLANS BEING INCLUDED IN THE
 RESUBMITTAL AND/OR INCORRECT PLANS NOT BEING REMOVED.
 STAFF IS UNABLE TO TELL EXACTLY WHAT THE SCOPE OF WORK
 ON THIS PERMIT IS.
  
 3) PROVIDE A SURVEY, PREPARED BY A FLORIDA LICENSED
 SURVEYOR, THAT SHOWS THE RIGHT-OF-WAY LINE, THE
 SIDEWALK, AND ANY TREES WITHIN THE RIGHT-OF-WAY ALONG
 THE ENTIRE FIBER CONDUIT PATH SO THAT STAFF CAN VERIFY
 THAT THE BORE AND HANDHOLES ARE WITHIN THE
 RIGHT-OF-WAY. THE SHEETS THAT ARE PROVIDED AND LABELED
 AS THE SURVEY, DO NOT INCLUDE THE NAME OR SEAL OF THE
 SURVEYOR, NOR DO THEY INCLUDE THE DATE OF THE SURVEY
 AND APPEAR TO BE COPIES OF THE PROPERTY APPRAISER
 AERIALS AND NOT THE SURVEYED RIGHT-OF-WAY LINE
 LOCATIONS.
  
 4) PROVIDE THE DIMENSION FROM THE EDGE OF THE
 EXCAVATION FOR BORE PIT 03+50 TO THE TRUNK OF THE
 NEAREST TREE WITHIN THE RIGHT-OF-WAY. THIS DIMENSION
 SHALL BE AT LEAST 10 FEET.
  
 5) THE RIGHT-OF-WAY LINE ON "SURVEY" SHEET 15 OF 19
 DOES NOT MATCH THE LOCATION SHOWN ON SHEET 025 OF 029.
 BASED ON THE SURVEY, BORE PIT 04+92 AND PORTIONS OF THE
 BORE ARE LOCATED ON PRIVATE PROPERTY, WHICH IS NOT
 PERMITTED.
  
 6) THE RIGHT-OF-WAY LINE ON "SURVEY" SHEET 16 OF 19
 DOES NOT MATCH THE LOCATION SHOWN ON SHEET 026 OF 029.
 BASED ON THE SURVEY, HANDHOLE 08+12 AND THE BORE ARE
 LOCATED ON PRIVATE PROPERTY, WHICH IS NOT PERMITTED.
  
 7) THE RIGHT-OF-WAY LINE ON "SURVEY" SHEETS 16 AND 17
 OF 19 DOES NOT MATCH THE LOCATION SHOWN ON SHEET 027 OF
 029. BASED ON THE SURVEY, BORE PIT 12+12, HANDHOLE
 14+60 AND THE BORE ARE LOCATED ON PRIVATE PROPERTY,
 WHICH IS NOT PERMITTED. ADDITIONALLY, THE CONTOURS OF
 THE RIGHT-OF-WAY DIFFER ON THE SURVEY AND PLANS
 (90-DEGREE VS CURVED).
  
 8) THE RIGHT-OF-WAY LINE ON "SURVEY" SHEET 17 AND 18 OF
 19 DOES NOT MATCH THE LOCATION SHOWN ON SHEET 028 OF
 029 (ALSO DOES NOT MATCH SURVEYS THAT THE CITY HAS ON
 FILE). BASED ON THE SURVEY, BORE PIT 18+62 AND THE BORE
 ARE LOCATED ON PRIVATE PROPERTY, WHICH IS NOT
 PERMITTED. THE BORE AT THE SOUTHEAST CORNER OF ROSEMARY
 AVENUE AND FERN STREET CLEARLY ENCROACHES ONTO PRIVATE
 PROPERTY ON THE ENGINEERING PLAN. ADDITIONALLY, THE
 ENGINEERING PLAN DOES NOT MATCH THE CURRENT CONDITIONS
 OF FERN STREET EAST OF ROSEMARY AVENUE BASED ON THE
 RECENT RECONSTRUCTION OF THE ROADWAY BY THE 360
 ROSEMARY PROJECT. THE PLANS MUST SHOW CURRENT ACCURATE
 CONDITIONS SO THAT STAFF CAN VERIFY THE PROPOSE
 INSTALLATIONS DO NOT INTERFERE WITH THE STREETSCAPE
 ELEMENTS. PLEASE NOTE THAT THE LANDSCAPE AREA UTILIZES
 A STRUCTURAL SOIL SYSTEM EXTENDING OUT FROM THE TREE
 PITS THAT MAY NOT BE DISTURBED BY THE BORE OR OTHER
 EXCAVATIONS SUCH AS TRENCHING OR SETTING A HANDHOLE.
 THE PROPOSED TRENCH AND HANDHOLE 22+13 APPEAR TO BE IN
 DIRECT CONFLICT WITH A TREE AND THE STRUCTURAL SOIL ARE
 IT IS PLANTED IN.
  
 9) THE FILES TITLED AS 014B PLAN & PROFILE AND 017
 RUNNING LINES, UPLOADED ON JULY 21, 2022, ARE FROM THE
 NORTH OF FERN STREET SECTION OF THIS PROJECT, WHICH DO
 NOT SEEM TO BE WHAT THIS PERMIT IS COVERING AND SHOULD
 BE REMOVED FROM THIS SUBMITTAL.
  
 10) THE CITY HAS APPROVED A NEW PROJECT LOCATED AT 515
 FERN STREET. STAFF CAN PROVIDE A COPY OF THE APPROVED
 LANDSCAPE PLAN TO THE PROPOSED HANDHOLE 25+23 CAN BE
 LOCATED SO AS NOT TO INTERFERE WITH THE PROPOSED
 LANDSCAPE AREAS. NOTE: THE PROPOSED LOCATION MAY NOT
 PRESENT PROBLEMS, BUT STAFF IS UNABLE TO TELL BY
 COMPARING THE PLANS.
  
 11) THE BORE, BORE PITS AND HANDHOLES ARE ONLY
 PERMITTED ON PRIVATE PROPERTY WITHIN A GENERAL UTILITY
 EASEMENT. THE SURVEY DOES NOT SHOW A GENERAL UTILITY
 EASEMENT AT ANY OF THE LOCATION LISTED AS HAVING
 ENCROACHMENTS ONTO PRIVATE PROPERTY.
  
 12) ROSEMARY AVENUE AND CLEMATIS STREET ARE CITY OF
 WEST PALM BEACH CONTROLLED RIGHTS-OF-WAY, SO THE DETAIL
 SHEETS NEED TO BE THE CITY OF WEST PALM BEACH STANDARD
 DETAILS, NOT THE PALM BEACH COUNTY DETAILS.
  
 13) THE TUBULAR MARKERS ARE NOT PERMITTED. MARKERS MAY
 ONLY BE INSTALLED WITHIN THE HANDHOLES OR OTHERWISE
 BURIED.
  
 14) THE BORE DEPTH FOR THE MAJORITY OF THE PROJECT IS
 SUFFICIENT TO AVOID TREE ROOTS. THE AREA THAT NEEDS
 ADDITIONAL CLARITY IS ON THE NORTH SIDE OF FERN STREET.
  
 15) THE MOT PLANS SHALL NOT BE REUPLOADED WITH THIS
 PERMIT. THEY ARE REVIEWED AS PART OF THE FUTURE
 RIGHT-OF-WAY PERMIT.
