 1) THIS LOCATION IS WITHIN A HISTORIC DISTRICT. PLEASE
 PROVIDE A COPY OF THE STATE HISTORIC PRESERVATION
 OFFICE APPROVAL OF THIS LOCATION.
  
 2) THE SURVEY ONLY PROVIDES THE HEIGHT OF A CAMERA POLE
 AT 10 FEET IN HEIGHT IN THE VICINITY OF THE PROPOSED
 WIRELESS FACILITY POLE. THE ENGINEERING PLAN CALLS OUT
 A LIGHT POLE THAT IS 14 FEET IN HEIGHT. PER CITY CODE,
 NEITHER WILL PERMIT THE PROPOSED 24-FOOT-TALL WIRELESS
 FACILITY POLE. THERE APPEAR TO BE NEARBY POLES THAT ARE
 TALLER THAN THE LABELED POLES. PROVIDE THIS HEIGHT
 DIMENSION SO THAT STAFF CAN VERIFY THE PROPOSED
 WIRELESS FACILITY POLE DOES NOT EXCEED THE HEIGHT
 ALLOWANCE.
  
 3) THE ENGINEERING PLAN THAT IS OVERLAID ON THE AERIAL
 NEEDS TO SHOW THE HOUSE ON THE SOUTH SIDE OF 28TH
 STREET SO THAT STAFF CAN VERIFY THAT THE POLE IS NOT
 DIRECTLY IN FRONT OF THE HOUSE. STAFF HAS TRIED TO LINE
 UP THE POLE THE PROPOSED 16.7-FOOT DIMENSION FROM THE
 INTERSECTION THAT IS PROVIDE ON THE SURVEY, AND IT
 APPEARS THAT THE WIRELESS FACILITY POLE IS DIRECTLY IN
 FRONT OF THE HOUSE. THE WIRELESS FACILITY POLE MAY NEED
 TO BE SHIFTED A FEW FEET TO THE EAST TO COMPLY WITH
 THIS CODE REQUIREMENT.
  
 4) THE HANDHOLE AND BORE PIT NOTES ON SHEET P-1 STATE
 THAT THE SIDEWALK WILL BE RESTORED; HOWEVER, THE IS
 BOTH PARKWAY AND SIDEWALKS THAT WILL NEED RESTORATION.
 PLEASE UPDATE THE NOTATIONS ACCORDINGLY.
  
 5) PROVIDE A CROSS SECTION OF THE UNDERGROUND ELECTRIC
 WHICH VERIFIES THAT THERE ARE NO CONFLICTS WITH
 EXISTING UTILITIES.
  
 6) THE POLE STYLE MEETS CITY REQUIREMENTS. THE POLE
 COLOR AT THIS LOCATION SHALL BE RAL/RAYCAP 7042 TRAFFIC
