 1) THIS LOCATION IS WITHIN A HISTORIC DISTRICT. PLEASE
 PROVIDE A COPY OF THE STATE HISTORIC PRESERVATION
 OFFICE APPROVAL OF THIS LOCATION.
  
 2) THE PROPOSED WIRELESS FACILITY POLE IS SHOWN LOCATED
 WITHIN A BULB OUT. BOTH THE CITY TRAFFIC ENGINEER AND
 PLANNING WILL NOT APPROVE THIS LOCATION. THE WIRELESS
 FACILITY POLE NEEDS TO BE MOVED TO THE NORTHEAST OUT OF
 THE BULB OUT (A MARK UP WITH LOCATION HAS BEEN PROVIDED
 TO CHRISTINA WARR). NOTE THIS LOCATION ALSO KEEPS THE
 POLE FROM BEING LOCATED IN FRONT OF THE HOUSE ON THE
 SOUTH SIDE OF 36TH STREET.
  
 3) THE HANDHOLE AND BORE PIT NOTES ON SHEET P-1 STATE
 THAT THE SIDEWALK WILL BE RESTORED; HOWEVER, PARKWAY,
 SIDEWALK, AND STREET SURFACE WILL NEED RESTORATION.
 PLEASE UPDATE THE NOTATIONS ACCORDINGLY.
  
 4) PROVIDE A CROSS SECTION OF THE UNDERGROUND ELECTRIC
 WHICH VERIFIES THAT THERE ARE NO CONFLICTS WITH
 EXISTING UTILITIES.
  
 5) THE PLANS SHOW THE COM LINES WITHIN 1-FOOT OF THE
 HANDHOLE AND INSTALLATION DIG. IN ORDER TO VERIFY THAT
 THE INSTALLATION WILL NOT INTERFERE WITH THE EXISTING
 FACILITIES, PROVIDE A PUBLIC UTILITY LETTER OF NO
 OBJECTION FROM THE COM PROVIDER(S), MOST LIKELY AT&T
 AND COMCAST.
  
 6) THE POLE STYLE AND HEIGHT MEET CITY REQUIREMENTS.
 THE POLE COLOR AT THIS LOCATION SHALL BE RAL/RAYCAP
 7042 TRAFFIC GREY A.
