 PLANS ARE TO BE SIGNED WITH A FULL SIGNATURE FROM THE
 ARCHITECT OF RECORD PER 481.221 WITH A MINIMUM OF FIRST
 AND LAST NAME AS SHOWN ON THE ARCHITECTS' LICENSE, FAC

 PLEASE PROVIDE A SIGNED ACKNOWLEDGEMENT FROM THE

 INSTRUCTIONS ON THE WEBSITE OF ASBESTOS PROGRAM
 COORDINATOR, FLORIDA DEPARTMENT OF HEALTH PALM BEACH
 COUNTY WILL BE FOLLOWED, AND THAT NOTIFICATION WILL BE
 GIVEN TIMELY. ADDITIONAL INFORMATION REGARDING ASBESTOS
 REQUIREMENTS CAN BE FOUND ON THEIR WEBSITE:

 EITHER THE CONDOMINIUM BOARD OR THE MANAGEMENT COMPANY
 FOR BUILDINGS WITH 100 UNITS OR MORE GIVING PERMISSION
 FOR ALTERATIONS TO THE COMMON AREA PROPERTY.
