 SHALL COMPLY WITH THE REQUIREMENTS OF THE FLORIDA

 1A) THE PROPOSED FLOOR PLAN, FLOOR PLAN KEY NOTES, NOTE

 UNDERLAYMENT WITH PORCELAIN TILE. THE CONTRACTOR
 UPLOADED A SALES BROCHURE FOR PROFLEX MSC, MEGA SOUND
 CONTROL MEMBRANE WITH A FLAME SPREAD REPORT. THE CITY
 HAS NOT RECEIVED ANY APPROVED TESTING REPORTS FOR THIS
 PRODUCT ON A 6-INCH SLAB. DISCREPANCY IN PRODUCTS.
 PLEASE NOTE THE CITY IS IN RECEIPT OF TESTING REPORTS
 FOR SUPER SAM-125 UNDERLAYMENTS ON A 6-INCH CONCRETE
 SLAB THE FLOOR TOPPING BEING TRAVERTINE TILE, THE IIC
 OF 51 AND STC OF 54. AND FLAME SPREAD REPORT. NO
 REPORTS FOR SUPER SIM 90.
 1B) IF THE PRODUCT VARIES FROM WHAT THE DESIGNER OF
 RECORD REQUEST A SHOP DRAWING STAMP OF APPROVAL MUST
 ACCOMPANY THE FLOORING UNDERLAYMENT REPORTS. FL
 61G1-23.015 (2) THE ARCHITECT IS RESPONSIBLE FOR
 SUPERVISING AND REVIEWING ALL PROJECT DATA, REPORTS,
 SHOP DRAWINGS ETC.
 2) FLOOR INSULATION (FBC-B 2020) FOR TILE, STONE,
 MARBLE, VINYL AND WOOD FLOORING ALL NEED TO HAVE HAD
 THE SOUND UNDERLAYMENT TESTED FLOOR ASSEMBLIES TO THE
 FOLLOWING STANDARDS AND MEET THE QUALIFICATIONS LISTED
 UNDER EACH OF THE STANDARDS FOR A 6 INCH CONCRETE FLOOR
 ASSEMBLY/ NO DROPPED CEILING:
 PLEASE PROVIDE ALL THREE TEST REPORTS FOR TYPE I AND OR
 TYPE II BUILDINGS SHOWING COMPLIANCE WITH:
  
 2A) FBC-B 2020 1207.3 STRUCTURE-BORNE SOUND.
 FLOOR/CEILING ASSEMBLIES BETWEEN DWELLING UNITS OR
 BETWEEN A DWELLING UNIT AND A PUBLIC OR SERVICE AREA
 WITHIN THE STRUCTURE SHALL HAVE AN IMPACT INSULATION
 CLASS (IIC) RATING OF NOT LESS THAN 50 (45 IF FIELD
 TESTED) WHEN TESTED IN ACCORDANCE WITH ASTM E-492.
  
 2B) FBC-B 2020 1207.2 AIR-BORNE SOUND. WALLS,
 PARTITIONS AND FLOOR/CEILING ASSEMBLIES SEPARATING
 DWELLING UNITS FROM EACH OTHER OR FROM PUBLIC OR
 SERVICE AREAS SHALL HAVE A SOUND TRANSMISSION CLASS
 (STC) OF NOT LESS THAN 50 (45 IF FIELD TESTED) FOR
 AIR-BORNE NOISE WHEN TESTED IN ACCORDANCE WITH ASTM E
 90.
  
 2C) FLAME SPREAD- FBC-B 2020 603.1. EXCEPTION 2.
 INSULATION INSTALLED BETWEEN A FINISHED FLOOR AND SOLID
 DECKING WITHOUT INTERVENING AIRSPACE SHALL BE ALLOWED
 TO HAVE A FLAME SPREAD INDEX OF NOT MORE THAN 200.
  
 FLAME SPREAD INDEX. A COMPARATIVE MEASURE, EXPRESSED AS
 A DIMENSIONLESS NUMBER, DERIVED FROM VISUAL
 MEASUREMENTS OF THE SPREAD OF FLAME VERSUS TIME FOR A
 MATERIAL TESTED IN ACCORDANCE WITH ASTM E 84 OR UL 723.
  
 3) A TRANSMITTAL LETTER LISTING THE ORIGINAL REVIEW
 COMMENT NUMBER, WITH A DESCRIPTION OF THE REVISION
 MADE, IDENTIFYING THE SHEET OR SPECIFICATION PAGE WHERE
 THE CHANGES CAN BE FOUND WILL HELP TO EXPEDITE YOUR
