 A)WHISPER MAT UNDERLAYMENT INFORMATION SUBMITTED IS FOR
 ENGINEERED WOOD OR LAMINATE FLOORING. ACCORDING TO
 FLOOR PLAN SUBMITTED THE BATHROOM FLOORING IS TILE NO
 WOOD. NEED TO PROVIDE SOUND IMPACT AND IMPACT TESTING
 FOR TILE FLOOR.
  
 B) MISSING COPY OF FLAME SPREAD INDEX AND SMOKE
 DEVELOPED INDEX TEST AS REQUESTED ON ITEM A.

 FLOORING SOUND INSULATION MATERIAL:
 A) PROVIDE COPY OF THE FLAME SPREAD INDEX AND SMOKE
 DEVELOPED INDEX TEST DONE IN ACCORDANCE WITH ASTM E 84
 OR UL 723 AS REQUIRED BY SEC. 720.1 OF FBC 7TH EDITION
 (2020).
  
 B) PROVIDE COPY OF THE SOUND TRANSMISSION CLASS (STC)
 TEST DONE IN ACCORDANCE WITH ASTM E 90 AS REQUIRED BY
 SEC. 1207.2 OF FBC 7TH EDITION (2020).
  
 C) PROVIDE COPY OF THE IMPACT INSULATION CLASS (IIC)
 TEST DONE IN ACCORDANCE WITH ASTM E 492 AS REQUIRED BY
 SEC. 1207.3 OF FBC 7TH EDITION (2020).
  
 NOTE: TEST SAMPLES TO MATCH BUILDING FLOOR/CEILING
 CONSTRUCTION AND PROPOSED FLOOR FINISH. ALSO, THERE ARE
 TESTS FOR 6" OR 8" CONCRETE SLABS WITH OR WITHOUT
 ACOUSTICAL CEILING. TYPICALLY THERE ARE NOT SUSPENDED
 ACOUSTICAL CEILINGS IN THESE BUILDINGS.
