| PLEASE NOTE THAT THE LIGHT POLE FIXTURES MUST BE |
| SETBACK A MINIMUM OF 25 FT. FROM THE FRONT PROPERTY |
| LINE; 5 FT. FROM THE SIDE AND REAR PROPERTY LINES. THE |
| LIGHT FIXTURES DO NOT SEEM TO COMPLY WITH THE SETBACK |
| REQUIREMENTS. AN ALTERNATIVE MAY BE TO PROVIDE LIGHTING |
| ON THE BUILDING IF THE MINIMUM REQUIRED SETBACKS CANNOT |

| 2.) THE PHOTOMETRIC PLAN SHALL CONTAIN THE FOLLOWING |

| 3.) PROVIDED PLANS IN THE PERMIT ARE INCONSISTENT WITH |
| THE PROPOSED CHANGES AS PART OF APPLICATION FSPR |
| 16-03B, SUBMITTED TO THE PLANNING DIVISION FOR A MINOR |
| SITE PLAN AMENDMENT (PENDING REVIEW). PLANS SHALL BE |
| CONSISTENT WITH THE PROPOSED CHANGES TO THE SITE. |

| 4.) APPROVAL OF FSPR 16-03B IS REQUIRED PRIOR TO THE |
| APPROVAL OF THIS PERMIT. |
