 SEC. 506 FBC-EXISTING BUILDING AND SEC. R101.5
 FBC-ENERGY CONSERVATION.
 NOTE: PLEASE MAKE SURE FORMS ARE PROPERLY SIGNED BY ALL
 REQUIRED PARTIES AND IS COORDINATED WITH PLANS TO AVOID
 CONFLICTS/DELAYS.

 1(A)OWNER OR OWNER'S ARCHITECT OR OTHER OWNER LEGALLY
 AUTHORIZED AGENT NEEDS TO SIGN AND DATE THE BOTTOM OF
 FORM AS REQUIRED BY SEC. R103.1.1.2 FBC-ENERGY
 CONSERVATION.
 NOTE: PERMIT CAN NOT BE ISSUED WITHOUT THE OWNER'S OR
 AUTHORIZED AGENT CERTIFICATION.

 4- DESIGNER OF RECORD TO REVIEW AND APPROVE IN WRITING
 (NOT SIGNING AND SEALING) ALL PRODUCT APPROVALS AS
 REQUIRED BY SEC. 107.3.4.1 CITY AMENDMENTS.
  
 RESPONSE:
 THERE IS NO WAY OF KNOWING WHICH PRODUCT APPROVALS THE
 ARCHITECT REVIEWED/APPROVED. PRODUCT APPROVALS
 SUBMITTED DON'T SHOW ANY INDICATION THAT THEY WERE
 REVIEWED AND APPROVED BY THE ARCHITECT OF RECORD. THE
 ONLY RESPONSE FROM ARCHITECT WAS "SEE APPROVED PRODUCT
 APPROVALS FOR INSTALLATION". THERE IS NO REVIEW STAMP
 ON THE PRODUCT APPROVAL OR ANY LIST OF SPECIFIC PRODUCT
 APPROVAL NUMBERS AND MODELS THAT WERE REVIEWED/APPROVED
 BY THE ARCHITECT.
 ARCHITECT TO PROVIDE REVIEW STAMP ON EACH PRODUCT
 APPROVAL OR PROVIDE LETTER CLEARLY LISTING EACH PRODUCT
 APPROVAL SPECIFIC NUMBER AND MODEL.
