 1)A PORTION OF THE PROPERTY IS LOCATED IN AN AE FLOOD
 ZONE WHICH IS A SPECIAL FLOOD HAZARD AREA. IT APPEARS
 THAT THE BUILDING AND THE EQUIPMENT SERVICING IT ARE
 NOT LOCATED IN THE AE FLOOD ZONE. IT APPEARS THAT SHEET
 C5.00 HAS THE X AND X500 FLOOD ZONES REVERSED. FROM
 EAST TO WEST THE FLOOD ZONES ARE AE, X500 AND X. PLEASE
 CORRECT.
  
 2) ON SHEET 00.200 UNDER BUILDING TYPE THAT REFERENCES
 THE 2010 FBC AND THIS PROJECT FALLS UNDER THE 2017 FBC.
  
 3) AN MOT PLAN IS REQUIRED TO BE APPROVED BEFORE PERMIT
 ISSUANCE. IN ADDITION TO TRAFFIC CONTROL, THE PLAN
 SHOULD INCLUDE LOCATION OF STAGING AREAS, CONSTRUCTION
 TRAILER LOCATIONS, EMPLOYEE PARKING DURING
 CONSTRUCTION, EMERGENCY FIRE DEPARTMENT ACCESS, SITE
 FENCING (SAFEGUARDS DURING CONSTRUCTION), HOURS OF
 WORK, ETC. FBC 107
  
 4) THIS IS A THRESHOLD BUILDING. SUBMIT THE REQUIRED
 THRESHOLD DOCUMENTS WITH ORIGINAL SIGNATURES AND COMPLY
 WITH THE OTHER THRESHOLD REQUIREMENTS. THRESHOLD
 BUILDING / FL S. S. 553.71(7) "THRESHOLD BUILDING"
 MEANING ANY BUILDING WHICH IS GREATER THAN (3) STORIES
 OR 50 FT. IN HEIGHT, OR WHICH HAS AN ASSEMBLY OCCUPANCY
 CLASSIFICATION AS DEFINED IN THE FLORIDA BUILDING CODE
 WHICH EXCEEDS 5,000 SQ. FT. IN AREA AND AN OCCUPANT
 CONTENT OF GREATER THAN 500 PERSONS.

 CONSTRUCTION SERVICES DEPARTMENT REQUEST FOR THRESHOLD
 BUILDINGS A SPECIAL INSPECTOR AS REQUIRED BY 553.79(5)
 FLORIDA STATUTES TO THE MINIMUM INSPECTIONS REQUIRED BY
 THIS CODE. THE THRESHOLD INSPECTION FORMS ARE REQUIRED
 TO BE FILED OUT BY ALL PARTIES WITH INTEREST, NOTARIZED
 AND RETURNED TO THIS OFFICE, AND REVIEWED AND APPROVED
 BEFORE PERMIT ISSUANCE WILL TAKE PLACE.
  
 C) THRESHOLD BLDG. REQUIRED STATEMENT: W.P.B. AMENDMENT
 110.8.4.4 ALL PLANS FOR THE BUILDING WHICH ARE REQUIRED
 TO BE SIGNED AND SEALED BY THE ARCHITECT OR ENGINEER OF
 RECORD CONTAIN A STATEMENT THAT, TO THE BEST OF THE
 ARCHITECT'S OR ENGINEER'S KNOWLEDGE, THE PLANS AND
 SPECIFICATIONS COMPLY WITH THE APPLICABLE MINIMUM
 BUILDING CODES AND THE APPLICABLE FIRE-SAFETY STANDARDS
 AS DETERMINED BY THE LOCAL AUTHORITY IN ACCORDANCE WITH
 THIS SECTION AND CHAPTER 633 FLORIDA STATUTES.
  
 D) FOR THE SPECIAL INSPECTOR PLEASE PROVIDE A RESUME OF
 INSPECTION EXPERIENCE ON PREVIOUS PROJECT. THE RESUME
 MUST BE ACCOMPANIED BY ALL CERTIFICATES AS SPECIFIED
 HEREIN. THE BUILDING OFFICIAL OR HIS DESIGNEE WILL
 REVIEW THE RESUME. AFTER THE RESUME IS REVIEWED, AN
 INTERVIEW WILL BE SCHEDULED. UNDER NO CIRCUMSTANCES
 WILL AN INSPECTOR BE INTERVIEWED FOR WORK WITHOUT FIRST
 MEETING THE ABOVE CRITERIA.
  
 E) THE CONTRACTOR WILL PRODUCE A JOB SUMMARY OR HISTORY
 FOR THIS TYPE OF CONSTRUCTION.

 2018) GUIDELINES AND POLICIES FOR PROJECTS CONCERNING

 SUBMITTED AND APPROVED BY THE ENGINEER OF RECORD FOR
 THE PROJECT PRIOR TO COMMENCEMENT OF OPERATIONS. FBC

 CLEARANCE FOR ACCESSIBILE VAN PARKING SPACES, ACCESS

 9) IN ORDER TO PREVENT BEING OVER CHARGED FOR THE
 BUILDING PERMITS, PROVIDE A COST BREAKDOWN OF THE
 PROJECT. SEPARATE THE COSTS ASSOCIATED WITH THE
 FOUNDATION PERMIT FROM THE ITEMS THAT REQUIRE SEPARATE

 10) IN ORDER TO DETERMINE PERCENTAGE OF ALLOWABLE
 OPENINGS, SUBMIT A SITE PLAN THAT SHOWS THE DISTANCE
 FROM THE EXTERIOR WALLS OF THE BUILDING TO PROPERTY
 LINES. FBC 705.8
  
 11) WHEN RESUBMITTING, IT IS HELPFUL TO PROVIDE A
 RESPONSE LETTER ADDRESSING EACH ITEM ALONG WITH THE
 CITY RE-SUBMITTAL FORM. ADDITIONAL COMMENTS MAY BE
 GENERATED BY THE RESPONSES.
  
 12) SOME FLOOR PLANS NEED TO COMPLY WITH FBC 1016.2
 EGRESS THROUGH INTERVENING SPACES.
 EGRESS THROUGH INTERVENING SPACES SHALL COMPLY WITH
 THIS SECTION.
 1.EXIT ACCESS THROUGH AN ENCLOSED ELEVATOR LOBBY IS
 PERMITTED. ACCESS TO NOT LESS THAN ONE OF THE REQUIRED
 EXITS SHALL BE PROVIDED WITHOUT TRAVEL THROUGH THE
 ENCLOSED ELEVATOR LOBBIES REQUIRED BY SECTION 3006.

 14) WILL NEED BUILDING OFFICIAL APPROVAL FOR THE
 FOUNDATION ONLY - FBC WPB 105.13 PHASED PERMIT
 APPROVAL. AFTER SUBMITTAL OF THE APPROPRIATE
 CONSTRUCTION DOCUMENTS,
 THE BUILDING OFFICIAL IS AUTHORIZED TO ISSUE A PERMIT
 FOR THE CONSTRUCT ION OF FOUNDATIONS OR ANY OTHER
 PART OF A BUILDING OR STRUCTURE BEFORE THE CONSTRUCTION
 DOCUMENTS FOR THE WHOLE BUILDING OR STRUCTURE
 HAVE BEEN SUBMITTED. THE HOLDER OF SUCH PERMIT FOR THE
 FOUNDATION OR OTHER PARTS OF A
 BUILDING OR STRUCTURE SHALL PROCEED AT THE HOLDER'S OWN
 RISK WITH THE BUILDING OPERATION AND
 WITHOUT ASSURANCE THAT A PERMIT FOR THE ENTIRE
 STRUCTURE WILL BE GRANTED. CORRECTIONS MAY BE
 REQUIRED TO MEET THE REQUIREMENTS OF THE TECHNICAL
 CODES. THIS PROVISION IS ONLY FOR THE
 FLORIDA BUILDING CODE. ALL OTHER AGENCY APPROVALS
 NECESSARY FOR CONSTRUCTION MUST BE SECURED
