 THE FOUNDATION PERMIT HAS BEEN DENIED FOR THE FOLLOWING
 REASONS.
  
 1) THE PERMIT TO CONSTRUCT THE BUILDING HAS NOT BEEN
 APPLIED FOR AT THIS TIME. TYPICALLY THE PERMIT TO
 CONSTRUCT THE BUILDING HAS BEEN APPLIED FOR AND THE
 PLANS FOR CONSTRUCTION HAVE REVIEWED BEFORE A PERMIT
 FOR A FOUNDATION ONLY PERMIT IS APPLIED FOR. UNLESS THE
 PLANS FOR THE BUILDING HAVE BEEN REVIEWED AND APPROVED,
 THE FOUNDATION ONLY PERMIT WILL BE ISSUED AS A PROCEED
 AT YOUR ON RISK PERMIT. FBC 107.3.3 PHASED APPROVAL.
 (SEE ALSO SECTION 105.13.) THE BUILDING OFFICIAL IS
 AUTHORIZED TO ISSUE A PERMIT FOR THE CONSTRUCTION OF
 FOUNDATIONS OR ANY OTHER PART OF A BUILDING OR
 STRUCTURE BEFORE THE CONSTRUCTION DOCUMENTS FOR THE
 WHOLE BUILDING OR STRUCTURE HAVE BEEN SUBMITTED,
 PROVIDED THAT ADEQUATE INFORMATION AND DETAILED
 STATEMENTS HAVE BEEN FILED COMPLYING WITH PERTINENT
 REQUIREMENTS OF THIS CODE. THE HOLDER OF SUCH PERMIT
 FOR THE FOUNDATION OR OTHER PARTS OF A BUILDING OR
 STRUCTURE SHALL PROCEED AT THE HOLDER'S OWN RISK
 WITH THE BUILDING OPERATION AND WITHOUT ASSURANCE THAT
 A PERMIT FOR THE ENTIRE STRUCTURE WILL BE GRANTED. THIS
 PROVISION ONLY APPLIES TO THE FLORIDA BUILDING CODE;
 ALL OTHER AGENCY
 APPROVALS NECESSARY FOR CONSTRUCTION MUST BE SECURED
 PRIOR TO THIS PROVISION BEING APPLIED.
  
 2)A PORTION OF THE PROPERTY IS LOCATED IN AN AE FLOOD
 ZONE WHICH IS A SPECIAL FLOOD HAZARD AREA. IT APPEARS
 THAT THE BUILDING AND THE EQUIPMENT SERVICING IT ARE
 NOT LOCATED IN THE AE FLOOD ZONE. IT APPEARS THAT SHEET
 C5.00 HAS THE X AND X500 FLOOD ZONES REVERSED. FROM
 EAST TO WEST THE FLOOD ZONES ARE AE, X500 AND X. PLEASE
 CORRECT.
  
 3) ON SHEET 00.200 UNDER BUILDING TYPE THAT REFERENCES
 THE 2010 FBC AND THIS PROJECT FALLS UNDER THE 2017 FBC.
  
 4) THIS PROJECT EXPANDS OVER MULTIPLE LOTS OR PARCELS
 OF LAND. THIS WILL HAVE TO BE ADDRESSED PRIOR TO PERMIT
 ISSUANCE. FBC 107.2.1
  
 5) AN MOT PLAN IS REQUIRED TO BE APPROVED BEFORE PERMIT
 ISSUANCE. IN ADDITION TO TRAFFIC CONTROL, THE PLAN
 SHOULD INCLUDE LOCATION OF STAGING AREAS, CONSTRUCTION
 TRAILER LOCATIONS, EMPLOYEE PARKING DURING
 CONSTRUCTION, EMERGENCY FIRE DEPARTMENT ACCESS, SITE
 FENCING (SAFEGUARDS DURING CONSTRUCTION), HOURS OF
 WORK, ETC. FBC 107
  
 6) THIS IS A THRESHOLD BUILDING. SUBMIT THE REQUIRED
 THRESHOLD DOCUMENTS WITH ORIGINAL SIGNATURES AND COMPLY
 WITH THE OTHER THRESHOLD REQUIREMENTS. THRESHOLD
 BUILDING / FL S. S. 553.71(7) "THRESHOLD BUILDING"
 MEANING ANY BUILDING WHICH IS GREATER THAN (3) STORIES
 OR 50 FT. IN HEIGHT, OR WHICH HAS AN ASSEMBLY OCCUPANCY
 CLASSIFICATION AS DEFINED IN THE FLORIDA BUILDING CODE
 WHICH EXCEEDS 5,000 SQ. FT. IN AREA AND AN OCCUPANT
 CONTENT OF GREATER THAN 500 PERSONS.
  
 A)FBC (CITY AMENDMENTS)110.8.1 THRESHOLD INSPECTION
 PLAN. THE ENFORCING AGENCY SHALL REQUIRE A SPECIAL
 INSPECTOR TO PERFORM STRUCTURAL INSPECTIONS ON A
 THRESHOLD BUILDING PURSUANT TO A STRUCTURAL INSPECTION
 PLAN PREPARED BY THE ENGINEER OF RECORD. THE STRUCTURAL
 INSPECTION PLAN MUST BE SUBMITTED TO THE ENFORCING
 AGENCY PRIOR TO THE ISSUANCE OF A BUILDING PERMIT FOR
 THE CONSTRUCTION OF A THRESHOLD BUILDING. THE PURPOSE
 OF THE STRUCTURAL INSPECTION PLAN IS TO PROVIDE
 SPECIFIC INSPECTION PROCEDURES AND SCHEDULES SO THAT
 THE BUILDING CAN BE ADEQUATELY INSPECTED FOR COMPLIANCE
 WITH THE PERMITTED DOCUMENTS. (I SAW THAT A THRESHOLD
 INSPECTION PLAN IS PROVIDED ON SHEET S0-003.)
  
 B) THRESHOLD INSPECTOR: WPB AMENDMENT 110.8.1 W.P.B.
 CONSTRUCTION SERVICES DEPARTMENT REQUEST FOR THRESHOLD
 BUILDINGS A SPECIAL INSPECTOR AS REQUIRED BY 553.79(5)
 FLORIDA STATUTES TO THE MINIMUM INSPECTIONS REQUIRED BY
 THIS CODE. THE THRESHOLD INSPECTION FORMS ARE REQUIRED
 TO BE FILED OUT BY ALL PARTIES WITH INTEREST, NOTARIZED
 AND RETURNED TO THIS OFFICE, AND REVIEWED AND APPROVED
 BEFORE PERMIT ISSUANCE WILL TAKE PLACE.
  
 C) THRESHOLD BLDG. REQUIRED STATEMENT: W.P.B. AMENDMENT
 110.8.4.4 ALL PLANS FOR THE BUILDING WHICH ARE REQUIRED
 TO BE SIGNED AND SEALED BY THE ARCHITECT OR ENGINEER OF
 RECORD CONTAIN A STATEMENT THAT, TO THE BEST OF THE
 ARCHITECT'S OR ENGINEER'S KNOWLEDGE, THE PLANS AND
 SPECIFICATIONS COMPLY WITH THE APPLICABLE MINIMUM
 BUILDING CODES AND THE APPLICABLE FIRE-SAFETY STANDARDS
 AS DETERMINED BY THE LOCAL AUTHORITY IN ACCORDANCE WITH
 THIS SECTION AND CHAPTER 633 FLORIDA STATUTES.
  
 D) FOR THE SPECIAL INSPECTOR PLEASE PROVIDE A RESUME OF
 INSPECTION EXPERIENCE ON PREVIOUS PROJECT. THE RESUME
 MUST BE ACCOMPANIED BY ALL CERTIFICATES AS SPECIFIED
 HEREIN. THE BUILDING OFFICIAL OR HIS DESIGNEE WILL
 REVIEW THE RESUME. AFTER THE RESUME IS REVIEWED, AN
 INTERVIEW WILL BE SCHEDULED. UNDER NO CIRCUMSTANCES
 WILL AN INSPECTOR BE INTERVIEWED FOR WORK WITHOUT FIRST
 MEETING THE ABOVE CRITERIA.
  
 E) THE CONTRACTOR WILL PRODUCE A JOB SUMMARY OR HISTORY
 FOR THIS TYPE OF CONSTRUCTION.
  
 F) NOTE: FOR DIGITAL PLANS, AFTER PERMIT ISSUANCE,
 PLEASE E-MAIL THE PERMIT LIBRARIAN OF THE PERMIT BEING
 ISSUED, THE PERMIT LIBRARIAN WILL THEN E-MAIL YOU THE
 APPROVED AND BATCH STAMPED PLANS SO YOU MAY PRINT YOUR
 PLANS, TWO SETS OF PLANS WILL NEED TO BE PRINTED IN
 COLOR, (1) FOR THE THRESHOLD INSPECTOR, THE OTHER FOR
 THE MEP INSPECTORS OTHER PLANS YOU WISH TO PRINT MAYBE
 IN BLACK & WHITE.
  
 G) 110.8.4.3 ALL SHORING AND RESHORING PROCEDURES,
 PLANS AND DETAILS SHALL BE SUBMITTED TO THE ENFORCEMENT
 AGENCY FOR RECORDKEEPING. EACH SHORING AND RESHORING
 INSTALLATION SHALL BE SUPERVISED, INSPECTED AND
 CERTIFIED TO BE IN COMPLIANCE WITH THE SHORING
 DOCUMENTS BY THE CONTRACTOR.
  
 H) PLEASE CALL THE CITY OF WEST PALM BEACH'S AUTOMATED
 TELEPHONE INSPECTIONS LINE (561) 805-6700 FOR ALL
 BUILDING INSPECTIONS FOR AUDITING OF JOBSITE
 INSPECTIONS WITHOUT THIS THE PERMIT EXPIRES IN 6
 MONTHS, ABANDONMENT OF JOB, NO INSPECTION ACTIVITY.
  
 I) 110.8 THRESHOLD BUILDINGS, OPERATING GUIDELINES FOR
 THRESHOLD INSPECTORS #25. ASSURE THAT THE DESIGNER HAS
 REQUIRED ON THE PLANS OR SPECIFICATIONS ONE MATERIAL,
 APPROVED FOR FIRE STOPPING PENETRATIONS, FOR WALLS AND
 CEILINGS.
  
 J) PLEASE CONTACT ME WITH AN EMAIL ADDRESS AND I WILL
 BE HAPPY TO SEND YOU A COPY OF THE CURRENT (OCTOBER

 CLEARANCE ABOVE PARKING STALLS IS 7'-0" AFF. FBC
 ACCESSIBILITY SECTION 502.5 REQUIRES THE VERTICAL
 CLEARANCE FOR ACCESSIBILE VAN PARKING SPACES, ACCESS
 AISLES AND VECHICULAR ROUTES IS 98" OR 8'- 2".
  
 11) SHOW THAT THERE IS AT LEAST ONE ELEVATOR THAT IS
 CAPBALE OF ACCOMODATING AN AMBULANCE STRETCHER IN THE
 HORIZONTAL OPEN POSITION AS REQUIRED BY FBC 3002.4
  
 12) FBC 3007.6 REQUIRES A FIRE SERVICE ACCESS ELEVATOR
 TO OPEN INTO A FIRE SERVICE ACCESS ELEVATOR LOBBY. FBC
 3007.6.4 REQUIRES THE LOBBY SIZE TO BE A MINIMUM OF 150
 SQUARE FEET WITH NO DIMENSION LESS THAN 8 FEET IN
 DIMENSION. THE FIRE SERVICE ACCESS ELEVATOR LOBBIES
 HAVE A DIMENSION OF 6 FEET. DOES THIS DESIGN USE FBC
 3007.6 EXCEPTION #2 WHICH ALLOWS A 6 FT WIDE, 150 SQFT
 EXIT ACCESS CORRIDOR WITH THE PRESSURIZED FIRE ACCESS
 ELEVATORS AND FLOOR TO FLOOR SMOKE CONTROL? PLEASE
 CLARIFY.
  
 13) IN ORDER TO PREVENT BEING OVER CHARGED FOR THE
 BUILDING PERMITS, PROVIDE A COST BREAKDOWN OF THE
 PROJECT. SEPARATE THE COSTS ASSOCIATED WITH THE
 FOUNDATION PERMIT FROM THE ITEMS THAT REQUIRE SEPARATE

 14) IN ORDER TO DETERMINE PERCENTAGE OF ALLOWABLE
 OPENINGS, SUBMIT A SITE PLAN THAT SHOWS THE DISTANCE

 15) WHEN RESUBMITTING, IT IS HELPFUL TO PROVIDE A
 RESPONSE LETTER ADDRESSING EACH ITEM ALONG WITH THE
