 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) THE ENGINEERING PLANS SHOW ELEMENTS OF THE PROJECT
 BEING INSTALLED ON PRIVATE PROPERTY WHICH IS NOT
 PERMITTED FOR FIBER THAT IS PART OF A COMMUNICATION
 FACILITY WITHIN THE RIGHT-OF-WAY.
  
 A. THE AREA THAT IS SHOWN AS THE M CANAL IS A FEE
 PARCEL OWNED BY THE CITY OF WEST PALM BEACH, NOT A
 RIGHT-OF-WAY AND MAY NOT BE USED TO RUN FIBER. THE
 FIBER MUST BE IN THE RIGHT-OF-WAY. NOTE: NO
 VERIFICATION WAS PROVIDED THAT YOU HAVE PERMISSION TO
 ENCROACH ON CITY PROPERTY.
  
 B. 2130 ECHO LAKE DRIVE AND 4441 NORTH CONGRESS AVENUE
 ARE PRIVATE PROPERTIES WHICH MAY NOT HAVE A HANDHOLE
 INSTALLED ON THEM. THE HANDHOLE MUST BE IN THE
 RIGHT-OF-WAY. NOTE: NO VERIFICATION OF THE UTILITY
 EASEMENT SHOWN ON THE ENGINEERING PLAN WAS PROVIDED.
  
 C. 2135 PINEHURST DRIVE AND 4400 NORTH CONGRESS AVENUE
 ARE PRIVATE PROPERTIES WHICH MAY NOT BE USED TO RUN THE
 FIBER THAT IS PART OF A COMMUNICATION FACILITY WITHIN
 THE RIGHT-OF-WAY. THE FIBER MUST BE IN THE
 RIGHT-OF-WAY. NOTE: NO VERIFICATION OF THE UTILITY
 EASEMENT SHOWN ON THE ENGINEERING PLAN WAS PROVIDED.
  
 2) SURVEY - THE SURVEY THAT WAS PROVIDED NEEDS TO BE AT
 A LEGIBLE SIZE. STAFF IS UNABLE TO MAKE OUT ANY
 INFORMATION ON THE SURVEY THAT WAS PROVIDED.
  
 A. THE SURVEY MUST CLEARLY SHOW THE RIGHT-OF-WAY LINE
 AND THE SIDEWALK.
  
 B. ALL UNDERGROUND UTILITIES NEED TO BE PROVIDED ON THE
 SURVEY.
  
 C. THE ENGINEERING PLANS MUST SHOW THE SAME CONDITIONS
 AS THE SURVEY, SO THAT STAFF CAN VERIFY THAT NONE OF
 THE PROPOSED WORK (MARKER POLES, HANDHOLDS OR CONDUIT
 IS BEING INSTALLED ON PRIVATE PROPERTY.
  
 3) TIMETABLE-? YOU ARE REQUIRED TO PROVIDE THE
 ESTIMATED TIMETABLE FOR CONSTRUCTION OF THE PROJECT OR
 EACH PHASE THEREOF.
  
 4) EVIDENCE OF INSURANCE COVERAGE - PROVIDE EVIDENCE OF
 THE INSURANCE COVERAGE REQUIRED UNDER SECTION 78-406 OF
 THE CITY CODE.
  
 5) POLE STYLE - PLEASE NOTE THAT THE FUTURE POLES ARE
 REQUIRED TO MEET THE CITY CONCEALMENT STANDARDS
 OUTLINED IN SECTION 78-411 OF THE CITY CODE.
  
 6) ELECTRONIC FORMAT OF SUBMITTAL - A COPY OF ALL OF
 THE SUBMITTAL MATERIAL MUST BE SUBMITTED IN ELECTRONIC
 FORMAT.
