 ADDRESS THE FOLLOWING COMMENTS AND RE-SUBMIT:
  
 THE ELEVATION CERTIFICATE THAT WAS SUBMITTED IS DENIED
 FOR THE FOLLOWING REASONS:
 NOTE: THIS ADDITION WAS PERMITTED WHEN THE FIRM MAP
 DATED MARCH 1, 1979 WAS IN EFFECT. THE VERTICAL DATUM
 ON THE FIRM WAS IN NGVD AND THE CITY'S FLOOD ORDINANCE
 REQUIRED A 6 INCH FREE BOARD ABOVE THE BFE.
  
  
 1. [ THE NOTE ABOUT THE STORAGE AREA IN THE COMMENTS
 FOR SECTION D, SHOULD STATE THAT THE STORAGE AREA IS
 BELOW THE DFE AND HAS (3) THREE ENGINEERED FLOOD VENTS
 WITH 1200 SQ IN OF TOTAL NET AREA OF FLOOD OPENINGS.
 YOU WILL ALSO NEED TO REVISE THE NUMBER OF ENGINEERED
 FLOOD OPENINGS TO (5) FIVE FOR THE ATTACHED GARAGE WITH
 A TOTAL NET FLOOD OPENINGS AREA OF 2000 SQ IN.] SECTION
 A9(A) OF THE ELEVATION CERTIFICATE INCORRECTLY HAS THE
 SQUARE FOOTAGE OF THE GARAGE AS 1477 SQFT. AND THE LAST
 REVISED PLAN DATED 03/30/2023 (REV#5) HAS THE GARAGE
 (1105 SF) AND THE ATTACHED STORAGE AREA (504 SF) FOR A
 TOTAL OF 1609 SF. BOTH OF THESE AREAS ARE WET FLOOD
 PROOFED. REVISE THE E.C. TO INCLUDE BOTH AREAS.
  
  
 2. [ THE PHOTOS ARE NOT LABELED CORRECTLY AND ARE NOT
 DATED. SEE THE INSTRUCTIONS NEAR THE TOP OF THE
 BUILDING PHOTOGRAPHS PAGE. THE PHOTOGRAPHS ARE TO BE
 IDENTIFIED WITH THE DATE TAKEN AND LABELED "FRONT
 VIEW", "REAR VIEW", "RIGHT SIDE VIEW" AND "LEFT SIDE
 VIEW".] TO THE EXTENT POSSIBLE, THE PHOTOGRAPHS SHOULD
 SHOW THE ENTIRE BUILDING. THE PHOTOS ARE REQUIRED TO BE
 IN COLOR AND SHOW THE FLOOD VENTS.
  
 3. [ THE ELEVATION OF THE GARAGE FLOOR IS 5.45' NGVD ON
 THE FINAL SURVEY AND THE ELEVATION CERTIFICATE HAS 4.0'
 NAVD OR 5.5' NGVD. THE ELEVATIONS NEED TO MATCH.]
 SUBMIT A FINAL SURVEY WITH BUILDING ELEVATIONS. THE
 ELEVATIONS ON THE SURVEY AND THE ELEVATION CERTIFICATE
 MUST MATCH. RENAME THE GYM AS STORAGE TO MATCH THE
 REVISED PLAN.
  
 4. [ THE INSTRUCTIONS REQUIRE THE STATE'S ABBREVIATION
 OF FL IN THE BOX. ] IN SECTION B3. OF THE ELEVATION
 CERTIFICATE THE STATE'S TWO (2) LETTER ABBREVIATION
 (FL) SHOULD BE ENTERED AND NOT SPELLED OUT.
  
 5. [ PLEASE CORRECT.] ALL ITEMS IN SECTIONS A8. A), B)
 & C) SHOULD BE N/A INSTEAD OF 0 PER THE INSTRUCTIONS.
  
 ALL OF THESE REQUIREMENTS ARE LOCATED IN THE 2019
 EDITION OF FEMA'S ELEVATION CERTIFICATE AND
 INSTRUCTIONS AND IN FEMA TECHNICAL BULLETINS.
  
 PLEASE NOTE THAT SUBMITTAL OF ADDITIONAL AND/OR REVISED
 MATERIALS MAY RESULT IN NEW PLAN REVIEW COMMENTS.
