 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) ENGINEERING PLANS
  
 A. THE ENGINEERING PLANS MUST SHOW THE COMPLETED
 CLEMATIS STREET IMPROVEMENTS; THE FINAL ALLEY
 RECONSTRUCTION SOUTH OF CLEMATIS STREET, BETWEEN OLIVE
 AVENUE AND SOUTH DIXIE HIGHWAY; AND THE PROPOSED
 RECONSTRUCTION OF BANYAN BOULEVARD. STAFF NEEDS TO
 VERIFY THAT THE PROPOSED FIBER CONDUIT, MARKER POLES,
 HANDHOLDS AND FUTURE COMMUNICATION POLES DO NOT
 INTERFERE WITH THESE CITY RIGHT-OF-WAY PROJECTS.
  
 B. THE ENGINEERING PLAN INCORRECTLY SHOWS PRIVATE
 PROPERTY AS AN ALLEY ON THE WEST SIDE OF NORTH OLIVE
 AVENUE, NORTH OF BANYAN BOULEVARD. THIS PORTION OF THE
 FIBER NETWORK REQUIRES A BUILDING PERMIT APPLICATION AS
 IT IS NOT A RIGHT-OF-WAY.
  
 C. YOU ARE NOT PERMITTED TO INSTALL ABOVE GRADE MARKER
 POLES IN THE CITY RIGHTS-OF-WAY.
  
 2) SURVEY OF UNDERGROUND FACILITIES - NO SURVEY WAS
 PROVIDED.
  
 A. THE PROPERTY LINES AND ALL UNDERGROUND UTILITIES,
 TREES (AND THE AREA OF THE SOIL CELLS ON CLEMATIS
 STREET), AND UTILITY/LIGHT POLE FOOTERS WITHIN THE
 RIGHT-OF-WAY ALONG THE FIBER CONDUIT PATH NEED TO BE
 PROVIDED ON A SURVEY PREPARED BY A FLORIDA LICENSED
 SURVEYOR.
  
 B. THE ENGINEERING PLANS MUST SHOW THE SAME CONDITIONS
 AS THE SURVEY, SO THAT STAFF CAN VERIFY THAT NONE OF
 THE PROPOSED WORK (MARKER POLES, HANDHOLDS OR CONDUIT
 IS BEING INSTALLED ON PRIVATE PROPERTY AND THAT NO
 TREES (SPECIFICALLY THEIR ROOT SYSTEM) ARE OR UTILITY
 POLE FOOTERS ARE IN LINE OF THE PROPOSED BORE. PLEASE
 NOTE THAT SHEETS 22, 24 AND 25, WHEN COMPARED TO THE
 STREET CONDITIONS APPEAR TO SHOW MULTIPLE CONFLICTS
 WITH EXISTING TREES AND POLES.
  
 3) CONSTRUCTION TIMETABLE - YOU MUST PROVIDE AN
 ESTIMATED TIMETABLE FOR CONSTRUCTION OF THE PROJECT.
  
 4) RESTORATION ESTIMATE - YOU MUST PROVIDE A
 RESTORATION ESTIMATE, WHICH IS THE COST FOR THE
 RESTORATION BASED ON THE CONDITIONS AT EACH LOCATION
 WHERE YOU WILL BE INSTALLING A HANDHOLD OR DOING OTHER
 LAND DISTURBING ACTIVITY.
  
 5) EVIDENCE OF INSURANCE COVERAGE - YOU ARE REQUIRED TO
 SUBMIT VERIFICATION OF INSURANCE IN ACCORDANCE WITH THE
 PROVISIONS OF SECTION 78-406 OF THE CITY CODE OF
 ORDINANCES.
  
 6) PUBLIC UTILITY LETTERS, TELEPHONE, NATURAL GAS,
 ELECTRIC SERVICE, AND TELEVISION CABLE - THE PUBLIC
 UTILITY LETTERS WERE NOT PROVIDED. PROVIDE COPIES OF
 THE PUBLIC UTILITY LETTERS.
  
 7) ELECTRONIC FORMAT OF SUBMITTAL - ONLY THE PLANS WERE
 PROVIDED ON THE DISC. A COPY OF ALL OF THE SUBMITTAL
 MATERIAL MUST BE SUBMITTED IN ELECTRONIC FORMAT.
  
 8) FEES PAID - NO FEES WERE SUBMITTED. PLEASE PROVIDE
 THE JUSTIFICATION AS TO WHY YOU SHOULD NOT PAY FEES.
 PLEASE SEE SECTION A ON PAGE 4 OF THE APPLICATION FOR
 THE EXEMPTIONS. IF YOU DO NOT MEET THE EXEMPTION, THE
 REQUIRED FEES MUST BE SUBMITTED.
