 REPEAT COMMENT: IN ACCORDANCE WITH SECTIONS 78-402(B)
 AND 78-411(B)(4) OF THE CITY OF WEST PALM BEACH CODE OF
 ORDINANCES, THE CITY OF WEST PALM BEACH IS REQUESTING
 THAT THIS PROPOSED SMALL WIRELESS FACILITY BE MOVED TO
 ANOTHER LOCATION IN THE RIGHT-OF-WAY AND PLACED ON AN
 ALTERNATIVE UTILITY POLE OR ON A NEW UTILITY POLE TO BE
 INSTALLED WITHIN THE RIGHT-OF-WAY. PLEASE NOTE THAT
 THIS IS A RESIDENTIAL NEIGHBORHOOD AND SECTION
 78-411(B)(2) STATES THAT WIRELESS FACILITIES SHALL BE
 LOCATED ON THE OPPOSITE SIDE OF THE RIGHT-OF-WAY FROM
 RESIDENTIAL STRUCTURES AND NOT DIRECTLY IN FRONT OF A
 RESIDENTIAL STRUCTURE.
  
 PLEASE REMEMBER THAT POLES SHALL BE LOCATED SO AS TO
 ALIGN WITH EXISTING POLES IN THE RIGHT-OF-WAY AND HAVE
 EQUAL SETBACK DISTANCES WITH EXISTING POLES FROM THE
 CURBLINE OF THE RIGHT-OF-WAY.
  
 PLEASE SEE THE LIST BELOW FOR AN ANALYSIS OF THE
 DEFICIENCIES IN INFORMATION REQUIRED BY SECTION 78-401
 OF THE CITY OF WEST PALM BEACH CODE AS PART OF THE
 COMMUNICATION FACILITIES PLACEMENT PERMIT APPLICATION.
 THE DEFICIENT INFORMATION MUST BE PROVIDED FOR STAFF TO
 REVIEW THE PERMIT APPLICATION. PLEASE NOTE THAT STAFF
 HAS ALSO INCLUDED SOME SUBSTANTIVE REVIEW COMMENTS,
 THAT WHILE NOT RESULTING IN A DEFICIENT APPLICATION,
 NEED TO BE ADDRESSED OR THE PERMIT WILL NOT BE
 APPROVED.
  
 1) REPEAT COMMENT: PROVIDE A SURVEY, PREPARED BY A
 FLORIDA LICENSED SURVEYOR, THAT SHOWS:
  
 A. THE POLE LOCATION, THE RIGHT-OF-WAY LINE, AND THE
 UNDERGROUND ELECTRIC TO THE POLE SO THAT STAFF CAN
 VERIFY THAT THE UNDERGROUND ELECTRIC AND ANY REQUIRED
 BORE PITS AND HANDHOLES ARE WITHIN THE RIGHT-OF-WAY AND
 WHICH JURISDICTION'S RIGHT-OF-WAY THEY ARE IN.
 B. THE HEIGHT OF THE PROPOSED POLE AND THE HEIGHT OF
 THE UTILITY POLE WITHIN THE RIGHT-OF-WAY THAT IS BEING
 USED TO ESTABLISH THE HEIGHT OF THE PROPOSED WIRELESS
 COMMUNICATION FACILITY POLE.
 C. THE UNDERGROUND UTILITIES FOR THE EXTENT OF THE
 PROJECT, INCLUDING THE UNDERGROUND POWER ROUTE.
 D. ABOVEGROUND FEATURES, INCLUDING TREES.
 E. IDENTIFY ALL ABOVEGROUND COMMUNICATION FACILITIES IN
 THE AREA TO INCLUDE AN INVENTORY OF THE REGISTRANT'S
 EXISTING FACILITIES - NO INFORMATION ON EXISTING
 FACILITIES WAS PROVIDED. SHOW THIS INFORMATION ON THE
 PLANS AND PROVIDE A NARRATIVE. IF THERE ARE NO
 FACILITIES, THIS SHALL BE NOTED ON THE SURVEY.
  
 2) ENGINEERING PLAN (BASED OFF THE SURVEY REQUIRED IN
 COMMENT #1)
  
 A. REPEAT COMMENT: THE ENGINEERING PLAN WAS SUBMITTED
 AT A 1 INCH = 30 FOOT SCALE, WHICH DOES NOT COMPLY WITH
 THE CITY CODE SCALE REQUIREMENT OF 10FT, 20FT, OR 40FT
 SCALE. PROVIDE THE PLANS AT THE REQUIRED SCALE.
 B. SHOW THE UNDERGROUND POWER ROUTE ON THE ENGINEERING
 PLAN.
 C. PROVIDE THE CROSS SECTION OF THE UNDERGROUND
 ELECTRIC SO THAT STAFF CAN VERIFY THAT THE BORE IS DEEP
 ENOUGH TO AVOID EXISTING UTILITIES AND DAMAGING THE
 ROOT SYSTEMS OF THE TREES OVER THE ELECTRIC PATH.
  
 3) THANK YOU FOR SHOWING AS STEALTH POLE DESIGN. PLEASE
 PROVIDE VERIFICATION THAT THE METER AND ALL EQUIPMENT
 WILL BE HOUSED WITHIN THE POLE. THE POLE DETAIL SHEET
 DOES NOT SHOW A METER LOCATION.
  
 4) REPEAT COMMENT: RADIO FREQUENCY INTERFERENCE - NO
 INFORMATION WAS PROVIDED THAT THE PROPOSED FACILITIES
 WILL NOT CAUSE RADIO FREQUENCY INTERFERENCE WITH THE
 CITY'S PUBLIC SAFETY COMMUNICATIONS EQUIPMENT. PROVIDE
 A STATEMENT REGARDING RADIO FREQUENCY INTERFERENCE.
  
 5) REPEAT COMMENT: PUBLIC UTILITY LETTERS - PROVIDE A
 COPY THE UTILITY LETTERS OF NO OBJECTION FROM THE
 PROVIDERS THAT ARE SHOWN ON THE SURVEY WITH FACILITIES
 IN THE AREA. THE ENGINEERING PLAN SHOWS FLORIDA POWER
 AND LIGHT IN THE AREA, BUT THE NO OBJECTION LETTER FROM
 THEM HAS NOT BEEN PROVIDED.
  
 6) THE RIGHTS-OF-WAY IN THIS AREA ARE CONTROLLED BY
 WEST PALM BEACH. REMOVE THE REFERENCES TO PALM BEACH
 COUNTY FROM THE PLANS.
