 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 THE ELEVATION CERTIFICATE THAT WAS SUBMITTED IS DENIED
 FOR THE FOLLOWING REASONS:
 NOTE: THIS ADDITION WAS PERMITTED WHEN THE FIRM MAP
 DATED MARCH 1, 1979 WAS IN EFFECT. THE VERTICAL DATUM
 ON THE FIRM WAS IN NGVD AND THE CITY'S FLOOD ORDINANCE
 REQUIRED A 6 INCH FREE BOARD ABOVE THE BFE.
  
 1) THE ELEVATION CERTIFICATE THAT WAS SUBMITTED IS NOT
 THE CURRENT ONE, WHICH IS REQUIRED. THE CURRENT ONE HAS
 THE EXPIRATION DATE OF NOVEMBER 30, 2022. SUBMIT THE
 CURRENT FORM.
  
 2) FILL IN THE FORM USING THE INFORMATION FROM THE
 CURRENT FIRM (OCTOBER 5,2017) ADOPTED BY THE CITY OF
 WEST PALM BEACH. USE THE MAP/PANEL NUMBER (12099C0581),
 SUFFIX (F), FIRM INDEX DATE (10/05/2017), FIRM PANEL
 EFFECTIVE/REVISED DATE(10/05/2017), FLOOD ZONE
 DESIGNATION (AE) AND BASE FLOOD ELEVATIONS (6' & 7') IN
 NAVD 88 FROM THE CURRENT MAP. USE THE COMMENT AREA OF
 SECTION D TO PROVIDE THE PAST MAP INFORMATION
 PERTAINING TO THE BUILDING AND PROVIDE THE CONVERSION
 FACTOR FROM NGVD TO NAVD. THIS PROJECT WAS PERMITTED
 WHEN THE PREVIOUS FLOOD MAPS WERE IN EFFECT AND THE
 ELEVATIONS ON THE APPROVED PLANS ARE NGVD RATHER THAN
 NAVD.
  
 3) THE LOWEST ELEVATION OF MACHINERY IS REQUIRED TO BE
 AT OR ABOVE 7.5 FT NGVD CONVERTED TO NAVD ON THE EC.
  
 4) SECTION A9(A) OF THE ELEVATION CERTIFICATE
 INCORRECTLY HAS THE SQUARE FOOTAGE OF THE GARAGE AS
 1477 SQFT. AND THE LAST REVISED PLAN DATED 03/30/2023
 (REV#5) HAS THE GARAGE (1105 SF) AND THE ATTACHED
 STORAGE AREA (504 SF) FOR A TOTAL OF 1609 SF. BOTH OF
 THESE AREAS ARE WET FLOOD PROOFED. REVISE THE E.C. TO
 INCLUDE BOTH AREAS.
  
 5) SECTION A9(B) OF THE ELEVATION CERTIFICATE
 INCORRECTLY INDICATES THAT (5) STACKED ENGINEERED FLOOD
 VENTS WERE INSTALLED. THE LAST APPROVED REVISED PLANS
 DATED 03/30/2023 (REV#5) SPECIFY (5) STACKED ENGINEERED
 FLOOD VENTS ARE IN THE GARAGE FOR A TOTAL OF 2000 SQ IN
 AND (3) STACKED FLOOD VENTS ARE IN THE STORAGE AREA FOR
 A TOTAL OF 1200 SQ IN. IN TOTAL THERE ARE (8) STACKED
 FLOOD VENTS AND 3200 SQ IN OF NET FLOOD VENT OPENINGS.
 PLEASE REVISE THE ELEVATION CERTIFICATE TO INCLUDE THE
 TOTAL NUMBER AND AREA OF THE FLOOD VENTS FOR BOTH THE
 GARAGE AND ATTACHED STORAGE AREA. THE EVALUATION
 REPORTS FOR THE ENGINEERED FLOOD VENTS ARE REQUIRED TO
 BE AN ATTACHMENT TO THE ELEVATION CERTIFICATE.
  
 6) SECTION A9(C) OF THE ELEVATION CERTIFICATE HAS THE
 TOTAL NET OPENINGS OF THE FLOOD OPENINGS AS 2000 SQ IN
 AND THE REVISED PLAN SPECIFIES 3200 SQ IN FOR THE (8)
 STACKED FLOOD VENTS. PLEASE REVISE.
  
 7) THE ELEVATION CERTIFICATE HAS THE LOWEST EQUIPMENT
 AT 5.7' NGVD WHICH IS BELOW THE BFE PLUS THE 6"
 FREEBOARD (7.5' NGVD). THE ELEVATION OF THE LOWEST
 EQUIPMENT SERVICING THE BUILDING IS REQUIRED TO BE AT
 OR ABOVE THE DFE (DESIGN FLOOD ELEVATION). THE DFE FOR
 THIS BUILDING WAS 7.5' NGVD WHEN PERMITTED. THE LOWEST
 EQUIPMENT SERVICING THE BUILDING WILL HAVE TO BE RAISED
 TO BE AT OR ABOVE THE BFE AND FREEBOARD (7.5' NGVD).
  
 8) THE COMMENT SECTION OF THE ELEVATION CERT. MENTIONS
 THAT AN A/C AIR HANDLER LOCATED INSIDE THE GYM IS THE
 LOWEST PIECE OF EQUIPMENT SERVICING THE BUILDING. THE
 ONLY RESIDENTIAL OCCUPANCIES THAT CAN BE WET FLOOD
 PROOFED ARE PARKING AND STORAGE AREAS OR ENTRANCES TO
 THE BUILDING. THIS AREA CAN NOT BE USED AS A GYM.
 PLEASE REMOVE THE NAMING OF THIS AREA AS A GYM FOM THE
 ELEVATION CERTIFICATE AND THE FINAL SURVEY. IT IS TO BE
 CALLED OUT AS EITHER A GARAGE OR STORAGE.
  
 9) A COPY OF THE ENGINEERED FLOOD VENTS' CERTIFICATION
 OR EVALUATION REPORT IS REQUIRED TO BE WITH THE
 ELEVATION CERTIFICATE.
  
 10) TO THE EXTENT POSSIBLE, THE PHOTOGRAPHS SHOULD SHOW
 THE ENTIRE BUILDING. THE PHOTOS ARE REQUIRED TO BE IN
 COLOR AND SHOW THE FLOOD VENTS.
  
 11) SUBMIT A FINAL SURVEY WITH BUILDING ELEVATIONS. THE
 ELEVATIONS ON THE SURVEY AND THE ELEVATION CERTIFICATE
 MUST MATCH. RENAME THE GYM AS STORAGE TO MATCH THE
 REVISED PLAN.
  
 12) IN SECTION B3. OF THE ELEVATION CERTIFICATE THE
 STATE'S TWO (2) LETTER ABBREVIATION (FL) SHOULD BE
 ENTERED AND NOT SPELLED OUT.
  
 13) ALL ITEMS IN SECTIONS A8. A), B) & C) SHOULD BE N/A
 INSTEAD OF 0 PER THE INSTRUCTIONS.
  
 ALL OF THESE REQUIREMENTS ARE LOCATED IN THE 2019
 EDITION OF FEMA'S ELEVATION CERTIFICATE AND
