 61G1-16.004 TITLE BLOCK. ARCHITECT.
 A TITLE BLOCK MUST APPEAR ON ALL ARCHITECTURAL OR
 INTERIOR DESIGN DRAWINGS AND SPECIFICATION
 IDENTIFICATION SHEETS THAT ARE REQUIRED TO BE SIGNED
 AND SEALED. THE TITLE BLOCK MUST, AT A MINIMUM, CONTAIN
 THE FOLLOWING INFORMATION:
 (5) A SPACE FOR THE SIGNATURE AND DATED SEAL.
 (6) THE PRINTED NAME AND THE LICENSE/REGISTRATION
 NUMBER OF THE PERSON SEALING THE DOCUMENT.
  
 2) THE SUITE NUMBER IS 1003, IT IS NOT CLEAR IF THIS
 UNIT IS ON THE 1ST FLOOR OR THE 10TH FLOOR. FOR THIS

 DRAWN ELECTRICAL SYSTEMS ON WHAT APPEARS TO BE AN
 ARCHITECTS PLAN SINCE THERE IS AN ARCHITECTURAL FIRMS
 NAME IN THE TITLE BLOCK BUT NOT SIGNED AND SEALED. WITH
 THE ARCHITECTS FIRM NAME AND LICENSE NUMBER IT IS
 ASSUMED THIS PLAN WAS A DESIGN OF THE ARCHITECTURAL
 FIRM AND CANNOT BE WRITTEN OR DRAWN ON WITH THE
 EXCEPTION OF THE ARCHITECT OF RECORD.

 EITHER PLEASE PROVIDE TEST RESULTS FOR THE FOLLOWING:

 FOR WOOD AND WOOD LAMINATE FLOORING ALL NEED TO HAVE

 THE FOLLOWING STANDARDS AND MEET THE QUALIFICATIONS
 LISTED UNDER EACH OF THE STANDARDS FOR A 6 INCH
 CONCRETE FLOOR ASSEMBLY/ NO DROPPED CEILING:
 PLEASE PROVIDE TEST REPORTS SHOWING COMPLIANCE WITH:
  
 3A) FBC-B 2017 1207.3 STRUCTURE-BORNE SOUND.
 FLOOR/CEILING ASSEMBLIES BETWEEN DWELLING UNITS OR
 BETWEEN A DWELLING UNIT AND A PUBLIC OR SERVICE AREA
 WITHIN THE STRUCTURE SHALL HAVE AN IMPACT INSULATION
 CLASS (IIC) RATING OF NOT LESS THAN 50 (45 IF FIELD
 TESTED) WHEN TESTED IN ACCORDANCE WITH ASTM E 492.
  
 3B) FBC-B 2017 1207.2 AIR-BORNE SOUND. WALLS,
 PARTITIONS AND FLOOR/CEILING ASSEMBLIES SEPARATING

 INSULATION INSTALLED BETWEEN A FINISHED FLOOR AND SOLID
 DECKING WITHOUT INTERVENING AIRSPACE SHALL BE ALLOWED
 TO HAVE A FLAME SPREAD INDEX OF NOT MORE THAN 200.
  
 FLAME SPREAD INDEX. A COMPARATIVE MEASURE, EXPRESSED AS
 A DIMENSIONLESS NUMBER, DERIVED FROM VISUAL
 MEASUREMENTS OF THE SPREAD OF FLAME VERSUS TIME FOR A
 MATERIAL TESTED IN ACCORDANCE WITH ASTM E 84 OR UL 723.
  
 4) WHEN RESUBMITTING PLANS PLEASE INDICATE THE REVISION
 & REMOVE ANY VOIDED SHEETS & REPLACE ANY PAGES AS
 NECESSARY. A TRANSMITTAL LETTER LISTING THE ORIGINAL
 REVIEW COMMENT NUMBER, WITH A DESCRIPTION OF THE
 REVISION MADE, IDENTIFYING THE SHEET OR SPECIFICATION
 PAGE WHERE THE CHANGES CAN BE FOUND WILL HELP TO
 EXPEDITE YOUR PERMIT. THANK YOU FOR YOUR ANTICIPATED

 A THOROUGH REVIEW OF THE PLANS COULD NOT HAPPEN SINCE
 THERE WAS A GREAT DEFICIENCY IN PLANS AND REQUIRED
 DOCUMENTATION. WITH THE NEXT PLAN REVIEW CYCLE THERE
 MAYBE ADDITIONAL REVIEW COMMENTS BECAUSE OF NEW PLANS
 AND INFORMATION LACKING UNDER THE PRIOR REVIEW.
