 THE BUILDING OFFICIAL, THE CLAIMED VALUATION OF

 MECHANICAL OR PLUMBING SYSTEMS APPEARS TO BE
 UNDERESTIMATED ON THE APPLICATION, THE PERMIT SHALL BE
 DENIED, UNLESS THE APPLICANT CAN SHOW DETAILED,
 QUANTITY ESTIMATES, AND/OR BONA FIDE SIGNED CONTRACTS
 (EXCLUDING LAND VALUE) TO MEET THE APPROVAL OF THE
 BUILDING OFFICIAL. FOR PERMITTING PURPOSES, VALUATION
 OF BUILDINGS AND SYSTEMS SHALL BE TOTAL REPLACEMENT
 VALUE TO INCLUDE STRUCTURAL, ELECTRIC, PLUMBING,
 MECHANICAL, INTERIOR FINISH, RELATIVE SITE WORK,
 ARCHITECTURAL AND DESIGN FEES, MARKETING COSTS,
 OVERHEAD AND PROFIT; EXCLUDING ONLY LAND VALUE.
 VALUATION REFERENCES MAY INCLUDE THE LATEST PUBLISHED
 DATA OF NATIONAL CONSTRUCTION
 COST ANALYSIS SERVICES (MARSHALL-SWIFT, MEANS, ETC.),
 AS PUBLISHED BY INTERNATIONAL CODE CONGRESS.
 FINAL BUILDING PERMIT VALUATION SHALL BE SET BY THE
 BUILDING OFFICIAL.
  
 2. PROVIDE ASTM TEST REPORTS FOR UNDERLAYMENT TO BE
 USED UNDER HARD FLOORING.

 2. TOTAL SQUARE FOOTAGE OF NEW FLOORING. PORCELAIN TILE
 AREAS - PROVIDED ON PLAN
 3. TYPE OF PROPOSED FLOORING
 (TILE/MARBLE/LAMINATE/WOOD/OTHER). PLANS SPECIFY
 PORCELAIN TILE 12 X 24
 4. STATED VALUE OF WORK. N/A THIS WILL BE INCLUDED IN
 THE TOTAL VALUE ON APPLICATION.
 5. EXISTING SITE CONDITIONS: REQUIRED
 A) THICKNESS OF SLAB. B) CEILING ASSEMBLY (OPEN
 CONCRETE/SUSPENDED GYPSUM/ACOUSTICAL)
 6. PROPOSED UNDERLAYMENT. MUST BE FOR PROPOSED
 FLOORING.
 7. TEST REPORTS FOR PROPOSED UNDERLAYMENT BASED ON
 EXISTING SITE CONDITIONS.
 8. TEST REPORTS MAY BE BASED ON THE FOLLOWING ASTM?S,
 E-84, E-90 & E-492.
