 DENIED BY BUILDING
 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 1) THE ENERGY FORMS THAT WERE SUBMITTED CONTAIN
 NUMEROUS ERRORS. THE BUILDING TYPE HAS USER. THE PLANS
 SPECIFY ONLY ONE BEDROOM AND THE FORM LISTS THREE. THIS
 DWELLING HAS A CRAWL SPACE AND THE FORM LISTS SLAB ON
 GRADE. THE PLANS SPECIFY AN UNVENTED ATTIC WITH SPRAY
 FOAM INSULATION AND THE FORM HAS A VENTED ATTIC WITH
 BLOWN INSULATION. THE PLANS SPECIFY A WHITE ASPHALT
 SHINGLE ROOF AND THE FORM LISTS A GRAVEL ROOF. PLEASE
 CORRELATE THE FORMS WITH THE PLANS. FBC 107.2.1
  
 2) SHOW COMPLIANCE WITH FBC-R408.1 VENTILATION. THE
 UNDER-FLOOR SPACE BETWEEN THE BOTTOM OF THE FLOOR
 JOISTS AND THE EARTH UNDER ANY BUILDING (EXCEPT SPACE
 OCCUPIED BY A BASEMENT) SHALL HAVE VENTILATION OPENINGS
 THROUGH FOUNDATION WALLS OR EXTERIOR WALLS. THE MINIMUM
 NET AREA OF VENTILATION OPENINGS SHALL BE NOT LESS THAN
 1 SQUARE FOOT FOR EACH 150 SQUARE FEET OF UNDER-FLOOR
 SPACE AREA, UNLESS THE GROUND SURFACE IS COVERED BY A
 CLASS 1 VAPOR RETARDER MATERIAL. WHERE A CLASS 1 VAPOR
 RETARDER MATERIAL IS USED, THE MINIMUM NET AREA OF
 VENTILATION OPENINGS SHALL BE NOT LESS THAN 1 SQUARE
 FOOT FOR EACH 1,500 SQUARE FEET OF UNDER-FLOOR SPACE
 AREA. ONE SUCH VENTILATING OPENING SHALL BE WITHIN 3
 FEET OF EACH CORNER OF THE BUILDING.
  
 3) SHOW COMPLIANCE WITH FBC-R408.4 ACCESS. ACCESS SHALL
 BE PROVIDED TO ALL UNDER-FLOOR SPACES. ACCESS OPENINGS
 THROUGH THE FLOOR SHALL BE A MINIMUM OF 18 INCHES BY 24
 INCHES. OPENINGS THROUGH A PERIMETER WALL SHALL BE NOT
 LESS THAN 16 INCHES BY 24 INCHES. WHERE ANY PORTION OF
 THE THROUGH-WALL ACCESS IS BELOW GRADE, AN AREAWAY NOT
 LESS THAN 16 INCHES BY 24 INCHES SHALL BE PROVIDED. THE
 BOTTOM OF THE AREAWAY SHALL BE BELOW THE THRESHOLD OF
 THE ACCESS OPENING. THROUGH WALL ACCESS OPENINGS SHALL
 NOT BE LOCATED UNDER A DOOR TO THE RESIDENCE. SEE
 SECTION M1305.1.4 FOR ACCESS REQUIREMENTS WHERE
 MECHANICAL EQUIPMENT IS LOCATED UNDER FLOORS.
  
 4) SHOW COMPLIANCE WITH FBC-R318.7 INSPECTION FOR
 TERMITES. IN ORDER TO PROVIDE FOR INSPECTION FOR
 TERMITE INFESTATION, CLEARANCE BETWEEN EXTERIOR WALL
 COVERINGS AND FINAL EARTH GRADE ON THE EXTERIOR OF A
 BUILDING SHALL NOT BE LESS THAN 6 INCHES.
  
 5) PROVIDE DETAILS FOR THE RELOCATED INTERIOR STAIRS.
 FBC-R311.7 STAIRWAYS, R311.7.1 WIDTH, R311.7.5 STAIR
 TREADS AND RISERS, R311.7.6 LANDINGS FOR STAIRWAYS,
 R311.7.8 HANDRAILS AND R312.1 GUARDS.
  
 6)SHOW COMPLIANCE WITH FBC-R803.2.3.1 SHEATHING
 FASTENINGS. WOOD STRUCTURAL PANEL SHEATHING SHALL BE
 FASTENED TO ROOF FRAMING WITH RSRS-01 (2 3/8" X 0.113")
 NAILS AT 6 INCHES ON CENTER AT EDGES AND 6 INCHES ON
 CENTER AT INTERMEDIATE FRAMING, UNLESS ROOF DIAPHRAGM
 DESIGN REQUIRES A CLOSER SPACING. RSRS-01 IS RING SHANK
 ROOF SHEATHING NAIL MEETING THE SPECIFICATIONS IN ASTM
 F1667.
 WHERE ROOF FRAMING WITH A SPECIFIC GRAVITY, 0.42 < OR =
 G < 0.49 IS USED, SPACING OF RING-SHANK FASTENERS SHALL
 BE 4 INCHES ON CENTER IN NAILING ZONE 3 IN ACCORDANCE
 WITH FIGURE R803.2.3.1 WHERE VULT IS 165 MPH OR
 GREATER.
  
 7) UPDATE THE IMPACT GLAZING NOTES ON SHEET S-200 FROM
 THE 2014 TO THE 2017 FBC.
  
 8) SUBMIT TWO COPIES OF THE PRODUCT EVALUATION REPORT
 AND INSTALLATION INSTRUCTIONS FOR THE SPRAY FOAM
 INSULATION. FBC-R316
  
 9) FBC 107.3.4 - PROVIDE TWO SETS OF PRODUCT APPROVALS
 FOR THOSE PRODUCTS WHICH ARE REGULATED BY FAC
 61G20-3.001 PER FLORIDA STATUTE 553.842 (5) THAT HAVE
 BEEN REVIEWED AND APPROVED IN WRITING BY THE DESIGNER
 OF RECORD. PROVIDE TWO COMPLETE PRODUCT APPROVALS WITH
 FL OR NOA NUMBERS INCLUDING INSTALLATION INSTRUCTIONS
 AND MAXIMUM DESIGN PRESSURES FOR EACH OF THE DIFFERENT
 ROOFING MATERIALS, WINDOW(GLASS TYPES), STRUCTURAL
 MULLIONS AND EXTERIOR DOOR TYPES.
  
 10) THE PERMIT VALUATION OF $167,000.00 ON THE
 APPLICATION IS LOW BASED ON THE SCOPE OF WORK AND HAS
 BEEN ADJUSTED TO $190,533.00. FBC 109.3 BUILDING PERMIT
 VALUATION. FOR PERMITTING PURPOSES, VALUATION OF
 BUILDINGS AND SYSTEMS SHALL BE TOTAL REPLACEMENT VALUE
 TO INCLUDE STRUCTURAL, ELECTRIC, PLUMBING, MECHANICAL,
 INTERIOR FINISH, RELATIVE SITE WORK, ARCHITECTURAL AND
 DESIGN FEES, MARKETING COSTS, OVERHEAD AND PROFIT;
 EXCLUDING ONLY LAND VALUE. VALUATION REFERENCES MAY
 INCLUDE THE LATEST PUBLISHED DATA OF NATIONAL
 CONSTRUCTION COST ANALYSIS SERVICES (MARSHALL-SWIFT,
 MEANS, ETC.), AS PUBLISHED BY INTERNATIONAL CODE
 CONGRESS. FINAL BUILDING PERMIT VALUATION SHALL BE SET
 BY THE BUILDING OFFICIAL.
  
 11) IN ORDER TO REDUCE PLAN REVIEW TIME, PLEASE SUBMIT
 A RESPONSE LETTER INDICATING HOW EACH OF THE REVIEW
