 NOTE:
 MECHANICAL REVIEW MAY BE APPROVED WITH PROVISO. PROVISO
 MEANS THAT CONTRACTOR WILL PROVIDE REQUIRED INFORMATION
 AFTER THE PERMIT IS ISSUED BUT BEFORE INSPECTIONS TO
 AVOID CONFLICTS/DELAYS. REVISION FEES WILL APPLY.
 CONTRACTOR TO ADVICE IF PROVISO OPTION IS PREFERRED AT
 THIS TIME.
  
 1- EQUIPMENT SIZING CALCULATIONS ARE DONE BASED ON THE
 INFORMATION SUBMITTED IN THE ENERGY CALCULATIONS.
 ENERGY CALCULATIONS NEED TO BE REVISED. SEE BUILDING
 REVIEW COMMENTS BELOW. REVISE THE EQUIPMENT SIZING
 CALCULATIONS ONCE THE ENERGY CALCULATIONS ARE REVISED.

 1- ENERGY CALCULATIONS FORM R405-2014 SUBMITTED:
 A) OWNER OR LEGALLY DESIGNATED AGENT TO CERTIFY AT THE
 BOTTOM OF THE FORM AS REQUIRED BY SEC. R103.1.1.2
 FBC-ENERGY CONSERVATION, 5TH EDITION (2014).
  
 B) PROVIDE COMPLETE INFORMATION ON TOP OF FORM,
 INCLUDING THE OWNER'S NAME AND BUILDER'S NAME. SEC.
 R101.5.1.1.2 FBC-ENERGY CONSERVATION.
  
 C) CONCRETE BLOCK WALL INSULATION R-VALUE SPECIFIED ON
 ITEM #9 DOESN'T MATCH ARCHITECT'S PLANS. REVISE AS
 REQUIRED TO MATCH. SEC. R103.2 FBC-ENERGY CONSERVATION.
  
 D) CEILING INSULATION R-VALUE AND AREA SPECIFIED ON
 ITEM #10 DOESN'T MATCH PLANS. REVISE AS REQUIRED TO
 MATCH. SEC. R103.2 FBC-ENERGY CONSERVATION.
  
 E) PROVIDE COMPLETE INSPECTION CHECKLIST DOCUMENTING
 THE BUILDING COMPONENT CHARACTERISTICS OF THE PROPOSED
 DESIGN AS REQUIRED BY SEC. R405.4.2 FBC-ENERGY
 CONSERVATION.
  
 F) PROVIDE THE NAME OF THE PERSON COMPLETING THE
 COMPLIANCE REPORT AS REQUIRED BY SEC. R405.4.2
 FBC-ENERGY CONSERVATION.
  
 G) THE ENERGY PERFORMANCE LEVEL (EPL) DISPLAY CARD
 SUBMITTED NEEDS TO BE COMPLETED BY THE BUILDER AS
 REQUIRED BY SEC. R405.4.3 FBC-ENERGY CONSERVATION.
  
 H) THE "SPACES" INFORMATION ON 2 OF 4 SPECIFIES THAT
 THERE IS A KITCHEN. ACCORDING TO PLANS SUBMITTED THERE
 IS NOT KITCHEN IN THE GUEST HOUSE. THERE IS SUPPOSED TO
 BE NO COOKING. NOTE: ZONING DOESN'T ALLOW A KITCHEN FOR
 THE GUEST HOUSE. REVISE ENERGY CALCULATIONS AS
 REQUIRED. SEC. R103.2 FBC-ENERGY CONSERVATION.
  
 I) THE ROOF TYPE AND MATERIALS SPECIFIED ON PAGE 2 OF 4
