 CHECKLIST.
 CODE: FBC 5TH EDITION (2014).
  
 ORIGINAL COMMENT:
 1- SOUND INSULATION MATERIAL SUBMITTED:
 A) PROVIDE 2 COPIES OF THE FLAME SPREAD INDEX AND SMOKE
 DEVELOPED TEST DONE IN ACCORDANCE WITH ASTM E 84 OR UL
 723 AS REQUIRED BY SEC. 720.1 OF FBC 5TH EDITION
 (2014).
 B) PROVIDE 2 COPIES OF THE SOUND TRANSMISSION CLASS
 (STC) TEST DONE IN ACCORDANCE WITH ASTM E 90 AS
 REQUIRED BY SEC. 1207.2 OF FBC 5TH EDITION (2014).
 NOTE: TEST SAMPLE TO MATCH ACTUAL BUILDING
 CONSTRUCTION. TECHNICAL DATA SUBMITTED SHOWS THAT THE
 TESTS WERE CONDUCTED ON 8" CONCRETE SLAB WITH
 ACOUSTICAL CEILING. THIS ASSEMBLY DOESN'T MATCH THE
 ASSEMBLY ON THIS BUILDING. THIS BUILDING HAS A 7"
 CONCRETE SLAB AND THERE IS NO ACOUSTICAL CEILING. THERE
 ARE TESTS FOR 6" CONCRETE SLABS WITHOUT ACOUSTICAL
 CEILING. PROVIDE INFORMATION THAT MORE CLOSELY MATCHES
 THIS BUILDING ASSEMBLY.
  
 RESPONSE TO ITEMS (A) AND (B) ABOVE:
 ALL TESTS SUBMITTED ARE FOR 8" CONCRETE SLAB WITH
 ACOUSTICAL CEILING. THAT'S WHY THE RESULTS ARE SO HIGH.
 THIS BUILDING HAS A 7" CONCRETE SLAB AND THERE IS NO
 ACOUSTICAL CEILING. SUBMIT COPIES FOR TESTS DONE ON THE
 TYPE OF ASSEMBLY THAT CLOSELY MATCHES THIS BUILDING
 ASSEMBLY. REPEAT COMMENT.

 (IIC) TEST DONE IN ACCORDANCE WITH ASTM E 492 AS
 REQUIRED BY SEC. 1207.3 OF FBC 5TH EDITION (2014).
 NOTE: TEST SAMPLE TO MATCH ACTUAL BUILDING
 CONSTRUCTION. TECHNICAL DATA SUBMITTED SHOWS THAT THE
 TESTS WERE CONDUCTED ON 8" CONCRETE SLAB WITH

 2- VALUATION DECLARED IN THE PERMIT APPLICATION IS TOO
 LOW. VALUATION TO INCLUDE ALL LABOR, MATERIALS AND
 CONTRACTOR'S OVERHEAD AND PROFIT AS REQUIRED BY SEC.
 109.3 CITY AMENDMENTS. NOTE: PROVIDE COPY OF SIGNED
 CONTRACT FOR ALL SCOPE OF WORK TO BE DONE FOR BUILDING
 OFFICIAL APPROVAL.
  
 RESPONSE:
 IT LOOKS LIKE THE ESTIMATE SUBMITTED IS ONLY FOR
 INSTALLATION AND OWNER IS SUPPLYING THE MATERIALS. NEED
 TO INCLUDE THE COST OF ALL MATERIALS IN THE VALUATION.
