 CODE AND ISSUE A PERMIT, TO REJECT DESIGN DOCUMENTS
 REQUIRED BY THE CODE THREE OR MORE TIMES FOR FAILURE TO
 CORRECT A CODE VIOLATION SPECIFICALLY AND CONTINUOUSLY
 NOTED IN EACH REJECTION, INCLUDING, BUT NOT LIMITED TO,
 EGRESS, FIRE PROTECTION, STRUCTURAL STABILITY, ENERGY,
 ACCESSIBILITY, LIGHTING, VENTILATION, ELECTRICAL,
 MECHANICAL, PLUMBING, AND GAS SYSTEMS, OR OTHER
 REQUIREMENTS IDENTIFIED BY RULE OF THE FLORIDA BUILDING
 COMMISSION ADOPTED PURSUANT TO CHAPTER 120, THE LOCAL
 GOVERNMENT SHALL IMPOSE, EACH TIME AFTER THE THIRD SUCH
 REVIEW THE PLANS ARE REJECTED FOR THAT CODE VIOLATION,
 A FEE OF FOUR TIMES THE AMOUNT OF THE PROPORTION OF THE
 PERMIT FEE ATTRIBUTED TO PLANS REVIEW.

 OWNER. PLEASE NOTE THE OWNER OF 4300 MILLER AVE. WILL
 NEED TO SIGN THE PERMIT APPLICATION. THE PALM BEACH
 COUNTY PROPERTY APPRAISERS OFFICE LIST TC & MARILYN

 2) 2ND REQUEST. RESPONSE ACKNOWLEDGED SURVEY WILL BE
 SUBMITTED BY OWNER. PLEASE PROVIDE A SURVEY FOR THIS
 LOT IT APPEARS THE BUILDING IS CLOSER THAN 3 FEET TO
 THE PROPERTY LINE. IT APPEARS THE BUILDING MAYBE ON THE
 PROPERTY LINE SEE PALM BEACH COUNTY PROPERTY APPRAISERS
 OFFICE MAP. THE EXTERIOR WALL ADJACENT TO THE PROPERTY
 LINE WILL NEED TO SHOW COMPLIANCE WITH 2014 FBC-B TABLE
 602, 1 HOUR FIRE RATING.

 5) 2ND REQUEST OWNER TO PROVIDE SITE PLAN. SHEET A103
 THE FLOOR PLAN DOES NOT INDICATE IF THE 2 ENTRIES INTO
 THE BUILDING ARE LEVEL WITH THE EXTERIOR OF THE
 BUILDING AND COMPLYING WITH 2014 FBC-ACCESS. CODE 402.2
 COMPONENTS. ACCESSIBLE ROUTES SHALL CONSIST OF ONE OR
 MORE OF THE FOLLOWING COMPONENTS: WALKING SURFACES WITH
 A RUNNING SLOPE NOT STEEPER THAN 1:20, DOORWAYS, RAMPS,
 CURB RAMPS EXCLUDING THE FLARED SIDES, ELEVATORS, AND
 PLATFORM LIFTS. ALL COMPONENTS OF AN ACCESSIBLE ROUTE
 SHALL COMPLY WITH THE APPLICABLE REQUIREMENTS OF
 CHAPTER 4 AND 208.3.1.
  
 6) 2ND REQUEST. OWNER TO PROVIDE SITE PLAN. PLEASE
 SUBMIT A SITE PLAN AND SHOW COMPLIANCE WITH AN
 ACCESSIBLE PARKING SPACE AND ACCESS AISLE WHERE THE
 OCCUPANT WILL BE ABLE TO TRAVEL FROM THE VEHICLE TO THE
 FRONT ENTRY. ACCESS. CODE 502.2 VEHICLE SPACES. EACH
 PARKING SPACE MUST BE AT LEAST 12 FEET (3658 MM) WIDE,
 SHALL BE MARKED TO DEFINE THE WIDTH, AND SHALL HAVE AN
 ADJACENT ACCESS AISLE COMPLYING WITH 502.3.
  
 7) 2ND REQUEST. RESPONSE INDICATES DISCUSSION WITH
 STRUCTURAL ENGINEER, WE WILL NEED A SIGNED AND SEALED
 LETTER FOR THE STRUCTURAL ANALYSIS FOR THE OPENING OF
 THE ROOF DECK, AND IF REINFORCEMENT IS NECESSARY, HOW
 THE ROOF TOP CURB WILL BE ATTACHED TO THE ROOF DECK AND
 THE ROOF TOP UNIT TO THE CURB.
 COMPUTER CALCULATIONS FOR WIND DESIGN, MWFRS AND
 COMPONENTS AND CLADDING WERE SUBMITTED BY LARRY VAN
 DYK, WHO IS THIS THE CALCULATIONS ARE NOT SIGNED NOR
 SEALED.

 SPECIFICATIONS MAY BE USED AND THAT SHEET MUST BE

 SPECIFICATION WITH SUFFICIENT INFORMATION ON THE COVER

 AT EXISTING ROOF. THIS APPEARS TO BE A NEW ROOF TOP
 OPENING THAT WILL BE CUT INTO THE CONCRETE ROOF DECK.
 THERE IS NO DETAILS AS TO REQUIRED REINFORCEMENT NOR
 LOCATION IN REGARDS TO THE CONCRETE JOIST. THE DETAIL
 WILL ALSO NEED TO PROVIDE A WIND ANALYSIS FOR THE
 ANCHORING OF THE CURB TO THE ROOF DECK AS WELL AS THE
 RTU TO THE CURB. 2014 FBC-B 1604.4 ANALYSIS. LOAD
 EFFECTS ON STRUCTURAL MEMBERS AND THEIR CONNECTIONS
 SHALL BE DETERMINED BY METHODS OF STRUCTURAL ANALYSIS
 THAT TAKE INTO ACCOUNT EQUILIBRIUM, GENERAL STABILITY,
 GEOMETRIC COMPATIBILITY AND BOTH SHORT- AND LONG-TERM
 MATERIAL PROPERTIES. EVERY STRUCTURE SHALL BE DESIGNED
 TO RESIST THE OVERTURNING EFFECTS CAUSED BY THE LATERAL
 FORCES SPECIFIED IN THIS CHAPTER. SEE SECTION 1609 FOR
 WIND LOADS.

 PROVIDE SUPERSEDED NOA, THIS ONE IS EXPIRED, NEEDS TO
 BE REVIEWED BY THE DESIGNER OF RECORD. PLEASE IDENTIFY
 THE FOLLOWING REQUIREMENTS CALCULATING THE AREA (SIDE
 VIEW) OF THE ROOF TOP COMPRESSORS, CONFIGURATION ON
 STAND, THE MEAN ROOF HEIGHT OF THE STAND AND
 COMPRESSORS, WEIGHT, CIRCLED THE DESIGN PARAMETERS IN
 THE TABLES, ANCHORS AND UPLIFT VERSES MOMENT FOR THE
 UNIT TO THE STAND AND THE STAND TO THE ROOF. THE
 PRODUCT APPROVAL IS TO BE REVIEWED BY THE DESIGNER OF
 RECORD, APPROVED BEFORE SUBMISSION TO THE BUILDING

 FOR THE ROOF TOP A/C STAND IS A CARRIER ATTACHMENT
 DETAIL BY ROBERT SAMARA P.E. PLEASE NOTE THIS SIGNED
 AND SEALED DOCUMENT IS DATED 11/11/15. THIS SEEMS TO BE
 A GENERIC DETAIL. THE SEAL IS MINIATURIZED AND A COPY
 OF SIGNATURE.
 61G15-23.001 ENGINEERS SEALS ACCEPTABLE TO THE BOARD.
 SAID SEAL SHALL BE A MINIMUM OF 1 7/8 INCHES IN
