 FAILED AS FOLLOWS:
  
 THE RECORD INDICATES 25 WINDOWS & DOORS AND STORM
 PANELS WERE PERMITTED AND PARTIALLY INSTALLED PURSUANT
 TO 04080618, BUT THE INSPECTION FAILED AND THE PERMIT
 EXPIRED FOR LACK OF PRODUCT APPROVALS AND THE STORM
 PANELS NOT BEING INSTALLED. THE APPLICANT HAS SUBMITTED
 FOR REVIEW, WINDOW AND DOOR PRODUCTS FROM THE RECORD
 RETRIEVED FROM RECORDS STORAGE, FILENET, AND NEW
 PRODUCT APPROVALS FOR STORM PANELS AS NONE WERE EVER
 INSTALLED. THE PERMIT DESCRIPTION REFLECTS THIS.
  
 ADDRESS THE FOLLOWING COMMENTS AND RE-SUBMIT
  
 1. THE VALUATION APPEARS LOW. THE PERMIT VALUATION AND
 PERMIT FEE MUST BE COMMENSURATE WITH THE VALUATION AND
 FEE THAT A LICENSED CONTRACTOR WOULD PROVIDE.
 PLEASE INCREASE THE VALUATION ACCORDINGLY.
 YOU MAY PROVIDE A COPY OF THE CONTRACT SIGNED BY THE
 OWNER OR A LIST OF LABOR, MATERIALS AND EQUIPMENT
 RENTAL TO VERIFY VALUE.
 2. CONTRIBUTING HISTORIC PROPERTY. ADDRESS HISTORIC
 COMMENTS.
  
  
 THIS SUBMITTAL SHALL BE SPLIT - THE BELOW WORK SHALL BE
 ON SEPERATE PERMIT 4/26/17 CT
  
 ADDITIONALLY, THERE IS A LETTER INCLUDED ATTESTING THAT
 NO WORK WAS DONE ON EXPIRED APPLICATION 13010325. THAT
 APPLICATION WAS VAGUE AND THEREFORE NEVER APPROVED.
 THERE WAS TO BE $10,000.00 OF DRYWALL, PAINTING AND
 BASEBOARD WORK PERFORMED. THIS SUBMITTAL SHOULD OFFER A
 REPORT OF INSPECTION BY A RESPONSIBLE PERSON,
 CONTRACTOR OR ENGINEER, WITH AN EVALUATION OF THE
 APPLICATION AND THE BUILDING REGARDING THE FACTS OF THE
 MATTER OF 13010325 AND IT SHOULD INCLUDE REFERENCE TO
 CODE CASE CE13010208 WHICH CITED VIOLATIONS THAT MAY BE
 RELATED.
  
 FBC-107.2.1 INFORMATION ON CONSTRUCTION DOCUMENTS. ALL
 INFORMATION, DRAWINGS, SPECIFICATIONS AND ACCOMPANYING
 DATA SHALL BEAR THE NAME AND SIGNATURE OF THE PERSON
 RESPONSIBLE FOR THE DESIGN.
  
 FBC107.1 GENERAL. SUBMITTAL DOCUMENTS CONSISTING OF
 CONSTRUCTION DOCUMENTS, STATEMENT OF SPECIAL
 INSPECTIONS, GEOTECHNICAL REPORT AND OTHER DATA SHALL
 BE SUBMITTED IN TWO OR MORE SETS WITH EACH PERMIT
 APPLICATION.
  
 FBC 109.3 BUILDING PERMIT VALUATION. FOR PERMITTING
 PURPOSES, VALUATION OF BUILDINGS AND SYSTEMS SHALL BE
 TOTAL REPLACEMENT VALUE TO INCLUDE STRUCTURAL,
 ELECTRIC, PLUMBING, MECHANICAL, INTERIOR FINISH,
 RELATIVE SITE WORK, ARCHITECTURAL AND DESIGN FEES,
 MARKETING COSTS, OVERHEAD AND PROFIT; EXCLUDING ONLY
 LAND VALUE. VALUATION REFERENCES MAY INCLUDE THE LATEST
 PUBLISHED DATA OF NATIONAL CONSTRUCTION COST ANALYSIS
 SERVICES (MARSHALL-SWIFT, MEANS, ETC.), AS PUBLISHED BY
 INTERNATIONAL CODE CONGRESS. FINAL BUILDING PERMIT
 VALUATION SHALL BE SET BY THE BUILDING OFFICIAL.
  
 FBC 107.2.1 INFORMATION ON CONSTRUCTION DOCUMENTS.
 CONSTRUCTION DOCUMENTS SHALL BE DIMENSIONED AND DRAWN
 UPON SUITABLE MATERIAL. CONSTRUCTION DOCUMENTS SHALL BE
 OF SUFFICIENT CLARITY TO INDICATE THE LOCATION, NATURE
 AND EXTENT OF THE WORK PROPOSED AND SHOW IN DETAIL THAT
 IT WILL CONFORM TO THE PROVISIONS OF THIS CODE AND
 RELEVANT LAWS, ORDINANCES, RULES AND REGULATIONS, AS
 DETERMINED BY THE BUILDING OFFICIAL. SUCH DRAWINGS AND
 SPECIFICATIONS SHALL CONTAIN INFORMATION, IN THE FORM
 OF NOTES OR OTHERWISE, AS TO THE QUALITY OF MATERIALS,
 WHERE QUALITY IS ESSENTIAL TO CONFORMITY WITH THE
 TECHNICAL CODES.
  
 FBC 117 - THE BUILDING OFFICIAL MAY REQUIRE TESTS OR
 TEST REPORTS AS PROOF OF COMPLIANCE. REQUIRED TESTS ARE
 TO BE MADE AT THE EXPENSE OF THE OWNER, OR AGENT, BY AN
 APPROVED TESTING LABORATORY OR OTHER APPROVED AGENCY.
