 2. IDENTIFY TYPE OF CEILING IN UNIT. (NO CEILING OR

 3. PROPOSED UNDERLAYMENT (MAPEI MAPEGUARD 2) DOES NOT
 MEET THE REQUIREMENTS OF THE IMPACT SOUND TRANSMISSION
 TEST. MINIMUM RATING SHALL NOT BE LESS THAN 50 DB.
 4. PROVIDE UNDERLAYMENT THAT MEETS OR EXCEEDS ASTM 84
 (FLAME SPREAD), ASTM E-90 (AIRBOURNE SOUND) AND ASTM
 E-492 (IMPACT SOUND).
 5. THE INDIVIDUAL TAKING RESPONSIBILITY SHALL PROVIDE
 THEIR PRINTED NAME AND SIGNATURE ON THE SUBMITTED PLANS
 AND SUPPORTING DOCUMENTS PER FBC 107.
  
 THE STATED VALUE APPEARS LOW FOR THE PROPOSED SCOPE OF
 WORK.
 THE PERMIT VALUATION AND PERMIT FEE MUST BE
 COMMENSURATE WITH THE VALUATION AND FEE THAT A LICENSED
 CONTRACTOR WOULD PROVIDE.
 PLEASE INCREASE THE VALUATION ACCORDINGLY.
 YOU MAY PROVIDE A COPY OF THE CONTRACT SIGNED BY THE
 OWNER OR A LIST OF LABOR, MATERIALS AND EQUIPMENT

 UNDERESTIMATED ON THE APPLICATION, THE PERMIT SHALL BE

 QUANTITY ESTIMATES, AND/OR BONA FIDE SIGNED CONTRACTS

 VALUATION REFERENCES MAY INCLUDE THE LATEST PUBLISHED
 DATA OF NATIONAL CONSTRUCTION
 COST ANALYSIS SERVICES (MARSHALL-SWIFT, MEANS, ETC.),
 AS PUBLISHED BY INTERNATIONAL CODE CONGRESS.
 FINAL BUILDING PERMIT VALUATION SHALL BE SET BY THE

 CLASS (STC) OF NOT LESS THAN 50 (45 IF FIELD TESTED)
 FOR AIR-BORNE NOISE WHEN TESTED IN ACCORDANCE WITH ASTM
