 BUILDING
  
 REVIEW FOR RE-ISSUANCE OF PERMIT NUMBER 03120384 FOR
 RE-ROOFING OF WORK THAT WAS PERFORMED IN 2004. THE JOB
 WAS ORIGINALLY APPLIED FOR ON 12/05/2003 AND IS
 DOCUMENTED WITH INSPECTION HISTORY AS FOLLOWS:
 01/23/2004 FAILED SHEATHING BUT NOTED AS IN PROGRESS
 WITH NO ADDITIONAL NOTES, 01/26/2004 PASSED ROOF METAL
 TIN TAG, 07/01/2004 FAILED FINAL WITH NO NOTES AND
 CODED NOT READY. THE WORK DESCRIPTION IS: REPLACE ROOF
 WITH EXISTING IDENTICAL TILE. THERE IS NO UNDERLAYMENT
 SYSTEM SPECIFIED. THERE IS NO BUILT UP OR OTHER ROOFING
 PRODUCT SPECIFIED. SHEET 1 OF 6 FOR MIAMI DADE NOA
 02-1211.08 ROOF TILE IS IN THE RECORD. NO TILE
 ATTACHMENT IS SPECIFIED.
  
 THE INSPECTION HISTORY IS INCOMPLETE. THE OWNER HAS
 ENGAGED A LICENSED ROOFING CONTRACTOR. THE CONTRACTOR
 AND OWNER MUST ENGAGE A LICENSED ENGINEER OR ROOFING
 CONSULTANT TO PERFORM AN INVESTIGATION AND/OR TESTS TO
 ASSURE ALL THE REQUIREMENT OF THE PERMIT WERE
 SATISFACTORILY COMPLETED AND/OR SPECIFY ADDITIONAL WORK
 WHICH MAY BE NECESSARY. PURSUANT TO FBC 110.3.9.1 AND
 FBC 117, A WRITTEN REPORT WITH PHOTOGRAPHIC EVIDENCE IS
 REQUIRED. SUCH REPORT SHALL INCLUDE BUT NOT BE LIMITED
 TO CERTIFYING THE FOLLOWING INSPECTIONS.
  
 ROOF SHEATHING RE-NAILING
 UNDERLAYMENT AND UNDERLAYMENT FASTENER SPACING
 INCLUDING ALL METAL, FLASHING, AND DRIP EDGE
 INSTALLATION IS ON RECORD AS NOTED ABOVE.
 BUILT UP OR OTHER SECONDARY ROOFING SYSTEM INSTALLATION
 ROOFING TILE INSTALLATION AND FASTENING METHOD.
 THE REMAINING USEFUL LIFE OF THE ROOF COVERING SHALL BE
 ESTIMATED AND DOCUMENTED IN THE REPORT.
  
 FBC 110.3.9.1 AFFIDAVIT FOR INSPECTION. WITH SPECIFIC
 PRIOR APPROVAL OF, AND IN A FORMAT ACCEPTABLE TO THE
 BUILDING OFFICIAL, AN AFFIDAVIT FOR CERTIFICATION OF
 INSPECTION MAY BE ACCEPTED FROM THE PERMIT QUALIFIER;
 WHEN ACCOMPANIED BY EXTENSIVE PHOTOGRAPHIC EVIDENCE OF
 SUFFICIENT DETAIL TO DEMONSTRATE CODE COMPLIANCE.
  
 FBC 117 - THE BUILDING OFFICIAL MAY REQUIRE TESTS OR
 TEST REPORTS AS PROOF OF COMPLIANCE. REQUIRED TESTS ARE
 TO BE MADE AT THE EXPENSE OF THE OWNER, OR AGENT, BY AN
 APPROVED TESTING LABORATORY OR OTHER APPROVED AGENCY.
  
 DEPENDING ON THE ACCEPTABILITY OF THE REPORT AND
 INSPECTION AFFIDAVIT, THE MATTER MAY BE RESOLVED
 WITHOUT FURTHER NEED TO SCHEDULE ANY INSPECTION. PLEASE
 COORDINATE WITH ME DIRECTLY UPON FURTHER SUBMITTAL.
  
 ALTERNATIVELY, A RE-SUBMITTAL MAY BE ACCEPTED UNDER
 THIS PERMIT RECORD FOR A NEW ROOFING SYSTEM. ADDITIONAL
 REVIEW AND FEES WILL APPLY.
  
 IN THE EVENT THE ABOVE CANNOT BE ACCOMPLISHED, THE
 ORIGINAL PERMIT WILL REMAIN EXPIRED.
