 ENGINEER LICENSED UNDER CHAPTER 471, AN ARCHITECT
 LICENSED UNDER CHAPTER 481 OR A STANDARD INSPCTOR
 LICENSED UNDER CHAPTER 468. PLEAS REMOVE THE EIT, MR
 ADAM COON FROM THE AGREEMENT.
 NOTE. MR COON MAY BE USED TO OBSERVE THE APPLICATION OF
 WATERPROOFING ON THE BUILDING.
 1B. OWNER MUST SIGN AND DATE AGREEMENT (SEE PAGE 4 OF
 6).
 1C. CONTRACTOR MUST SIGN AND DATE AGREEMENT (SEE PAGE 5
 OF 6).

 CONSTRUCTION SERVICES DEPARTMENT REQUESTS FOR THRESHOLD
 BUILDINGS, A SPECIAL INSPECTOR IS REQUIRED BY F.S.
 553.79 (5) TO THE MINIMUM INSPECTIONS REQUIRED BY THIS
 CODE. THE THRESHOLD INSPECTION FORMS ARE REQUIRED TO BE
 FILLED OUT BY ALL PARTIES WITH INTEREST, NOTARIZED AND
 RETURNED TO THIS OFFICE AND REVIEWED AND APPROVED
 BEFORE PERMIT ISSUANCE WILL TAKE PLACE. CONTACT KEN

 6. JOB INSPECTOR ? FBC 110.3.7.6 THE BUILDING
 DEPARTMENT MAY ALLOW A SPECIAL INSPECTOR TO CONDUCT THE
 MINIMUM STRUCTURAL INSPECTION OF THRESHOLD BUILDINGS
 REQUIRED BY THIS CODE, SECTION 553.73,FLORIDA STATUTES,
 WITHOUT DUPLICATIVE INSPECTION BY THE BUILDING
 DEPARTMENT. THE BUILDING OFFICIAL IS RESPONSIBLE FOR
 ENSURING THAT ANY PERSON CONDUCTING INSPECTIONS IS
 QUALIFIED AS A BUILDING INSPECTOR UNDER PART XII OF
 CHAPTER 468, FLORIDA STATUTES, OR CERTIFIED AS A
 SPECIAL INSPECTOR UNDER CHAPTER 471 OR 481, FLORIDA
 STATUTES. INSPECTIONS OF THRESHOLD BUILDINGS REQUIRED
 BY SECTION 553.79(5), FLORIDA STATUTES, ARE IN ADDITION
 TO THE MINIMUM INSPECTIONS REQUIRED BY THIS CODE.
 FBC 110.3.7.4.1 THE SPECIAL INSPECTOR, UPON COMPLETION
 OF THE BUILDING AND PRIOR TO THE ISSUANCE OF A
 CERTIFICATE OF OCCUPANCY OR COMPLETION, FILE A SIGNED
 AND SEALED STATEMENT WITH THE ENFORCEMENT AGENCY IN
 SUBSTANTIALLY THE FOLLOWING FORM: ?TO THE BEST OF MY
 KNOWLEDGE AND BELIEF, THE CONSTRUCTION OF ALL
 STRUCTURAL LOAD-BEARING COMPONENTS DESCRIBED IN THE
 THRESHOLD INSPECTION PLAN COMPLIES WITH THE PERMITTED
 DOCUMENTS AND THE SPECIALTY SHORING DESIGN PROFESSIONAL
 ENGINEER HAS ASCERTAINED THAT THE SHORING AND RESHORING
 CONFORMS WITH THE SHORING AND RESHORING PLANS SUBMITTED
 TO THE ENFORCEMENT AGENCY?.
  
 7. NOTICE ONLY. PLEASE CALL THE CITY OF WEST PALM
 BEACH?S AUTOMATED TELEPHONE INSPECTION LINE @ (561)
 805-6700 FOR ALL BUILDING INSPECTIONS FOR AUDITING OF
 JOBSITE INSPECTIONS. WITHOUT THIS, THE PERMIT EXPIRES
 IN 6 MONTHS WHICH WILL RESULT IN ?ABANDONMENT OF JOB,
 NO INSPECTION ACTIVITY?. THE PERMIT WILL BECOME
 EXPIRED.
  
 8. PLEASE PROVIDE A SITE PLAN SHOWING THE PROPERTY
 LINES, CONSTRUCTION AREAS, STAGING AREAS, HOW THE
 OCCUPANTS OF THE BUILDING WILL BE SAFE GUARDED DURING
 THEIR ENTRY AND EXIT OF THE BUILDING. SHOW COMPLIANCE
 WITH 2014 FBC 3306.7 ? COVERED WALKWAYS, INCLUDING
 LIGHTING FOR THE EXIT DISCHARGE. 2014 FBC 3310.2 ?
 MAINTENANCE OF MEANS OF EGRESS. REQUIRED MEANS OF
 EGRESS SHALL BE MAINTAINED AT ALL TIMES DURING
 CONSTRUCTION, DEMOLITION, REMODELING OR ALTERATIONS AND
 ADDITIONS TO ANY BUILDING.
  
 9. IF SCAFFOLDING EXTENDS BEYOND THE PRIVATE PROPERTY
 AND ONTO CITY RIGHT OF WAY, ENGINEERING WILL REQUIRE
 RIGHT OF WAY PERMITS. PLEASE CONTACT MANNY GONZALEZ AT
 THE ENGINEERING DEPARTMENT ? (561) 494-1085.
 9A. IF SCAFFOLDING IS TO BE USED FOR THE HEIGHT OF THE
 BUILDING, PLEASE PROVIDE HOW THE SCAFFOLDING WILL BE
 BUILT AS WELL AS HOW THE SCAFFOLDING WILL BE ANCHORED
 TO RESIST LATERAL WIND LOADING. THERE APPEARS SAND
 BLASTING WILL ALSO BE AN ELEMENT OF THE OPERATION, HOW
 WILL THE SANDBLASTING BE CONTAINED, WILL THERE BE WIND
 SCREENS, IF SO PLEASE PROVIDE WHAT ELEMENTS WILL BE
 INTRODUCED TO KEEP THE SCAFFOLDING FROM MOVING WITH THE
 LATERAL EFFECT OF WIND ON THE WIND SCREENS. AT WHAT
 SAFETY FACTOR ARE THESE ELEMENTS FACTORED? 107.2.1.3
 ADDITIONAL INFORMATION. 2014 FBC 3302.1 & 3306.1.
  
 10. PROVIDE COMPLIANCE WITH FBC 3309.1 WHERE REQUIRED.
 ALL STRUCTURES UNDER CONSTRUCTION, ALTERATION OR
 DEMOLITION SHALL BE PROVIDED WITH NO FEWER THAN ONE
 APPROVED PORTABLE FIRE EXTINGUISHER IN ACCORDANCE WITH
 SECTION 906 AND SIZED FOR NOT LESS THAN ORDINARY HAZARD
 AS FOLLOWS:
 10A. AT EACH STAIRWAY ON ALL FLOOR LEVELS WHERE
 COMBUSTIBLE MATERIALS HAVE ACCUMULATED.
 10B. IN EVERY STORAGE AND CONSTRUCTION SHED.
 10C. ADDITIONAL PORTABLE FIRE EXTINGUISHERS SHALL BE
 PROVIDED WHERE SPECIAL HAZARDS EXIST, SUCH AS THE
 STORAGE AND USE OF FLAMMABLE AND COMBUSTIBLE LIQUIDS.
  
 11. DURING RESTORATION WORK ON BALCONIES, DEMONSTRATE
 HOW THE TENANTS WILL BE SAFE GUARDED FROM THE WORK AREA
 AND EGRESS ONTO THE BALCONIES. FBC 3306.1 PROTECTION
 REQUIRED.
