| (4) DATE PREPARED. |
| (5) A SPACE FOR THE SIGNATURE AND DATED SEAL. |
| (6) THE PRINTED NAME AND THE LICENSE/REGISTRATION |
| NUMBER OF THE PERSON SEALING THE DOCUMENT. |
| (7) THE DATE OF PLANS REVISION, IF THE PLANS ARE |
| REVISED. |

| FBC 109.3 BUILDING PERMIT VALUATION. FOR PERMITTING |
| PURPOSES, VALUATION OF BUILDINGS AND SYSTEMS SHALL BE |
| TOTAL REPLACEMENT VALUE TO INCLUDE STRUCTURAL, |
| ELECTRIC, PLUMBING, MECHANICAL, INTERIOR FINISH, |
| RELATIVE SITE WORK, ARCHITECTURAL AND DESIGN FEES, |
| MARKETING COSTS, OVERHEAD AND PROFIT; EXCLUDING ONLY |
| LAND VALUE. VALUATION REFERENCES MAY INCLUDE THE LATEST |
| PUBLISHED DATA OF NATIONAL CONSTRUCTION COST ANALYSIS |
| SERVICES (MARSHALL-SWIFT, MEANS, ETC.), AS PUBLISHED BY |
| INTERNATIONAL CODE CONGRESS. FINAL BUILDING PERMIT |
| VALUATION SHALL BE SET BY THE BUILDING OFFICIAL. |
| FAILURE TO PROVIDE DOCUMENTATION OF ESTIMATED COSTS MAY |
| DELAY THE APPLICATION PROCESS. |
