 LIST THE PROJECT ADDRESS PAGE 0400-1 AS 525 OKEECHOBEE
 BLVD. PLEASE CORRECT THE DISCREPANCY IN THE ADDRESS.

 AMENDMENTS TO THE FLORIDA BUILDING CODE, CHAPTER 1,
 ADMINISTRATION 107.2.1 INFORMATION ON CONSTRUCTION
 DOCUMENTS. CONSTRUCTION DOCUMENTS SHALL BE OF
 SUFFICIENT CLARITY TO INDICATE THE LOCATION OF THE
 PROJECT. 107.2.1.3 ADDITIONAL INFORMATION IS REQUIRED,
 DISCREPANCY.
  
 2) THIS BUILDING MEETS THE DEFINITION OF: THRESHOLD
 BUILDING FL S. S. 553.71(7) ?THRESHOLD BUILDING"
 MEANING ANY BUILDING WHICH IS GREATER THAN (3) STORIES
 OR 50 FT. IN HEIGHT, OR WHICH HAS AN ASSEMBLY OCCUPANCY
 CLASSIFICATION AS DENIED IN THE
 FLORIDA BUILDING CODE WHICH EXCEEDS 5,000 SQ. FT. IN
 AREA AND AN OCCUPANT CONTENT OF GREATER THAN 500
 PERSONS.
  
 2A) UPDATE THE THRESHOLD INSPECTION PLAN, TO THE
 CORRECT ADDRESS. 109.3.6.1 THRESHOLD INSPECTION PLAN.
 THE ENFORCING AGENCY SHALL REQUIRE A (WRITTEN/ TYPED)
 SPECIAL INSPECTOR TO PERFORM STRUCTURAL INSPECTIONS ON
 A THRESHOLD BUILDING PURSUANT TO A STRUCTURAL
 INSPECTION PLAN PREPARED BY THE ENGINEER OF RECORD. THE
 STRUCTURAL INSPECTION PLAN MUST BE SUBMITTED TO THE
 ENFORCING AGENCY PRIOR TO THE ISSUANCE OF A BUILDING
 PERMIT FOR THE CONSTRUCTION OF A THRESHOLD BUILDING.
 THE PURPOSE OF THE STRUCTURAL INSPECTION PLAN IS TO
 PROVIDE SPECIFIC INSPECTION PROCEDURES AND SCHEDULES SO
 THAT THE BUILDING CAN BE ADEQUATELY INSPECTED FOR
 COMPLIANCE WITH THE PERMITTED DOCUMENTS.

 REQUIRED BY THIS CODE. THE THRESHOLD INSPECTION FORMS
 ARE REQUIRED TO BE FILED OUT BY ALL PARTIES WITH
 INTEREST, NOTARIZED AND RETURNED TO THIS OFFICE, AND
 REVIEWED AND APPROVED BEFORE PERMIT ISSUANCE WILL TAKE

 DEPARTMENT ARE NOT COMPLETE, MISSING THE NOTARIZED
 SIGNATURES OF OWNER & CONTRACTOR.
  
 2C) MISSING STATEMENT. THRESHOLD BLDG., REQUIRED
 STATEMENT: W.P.B. AMENDMENT 110.3.7.4.4 ALL PLANS FOR
 THE BUILDING WHICH ARE REQUIRED TO BE SIGNED AND SEALED
 BY THE ARCHITECT OR ENGINEER OF RECORD CONTAIN A
 STATEMENT THAT, TO THE BEST OF THE ARCHITECT'S OR
 ENGINEER'S KNOWLEDGE, THE PLANS AND SPECIFICATIONS
 COMPLY WITH THE APPLICABLE FIRE-SAFETY STANDARDS AS
 DETERMINED BY THE LOCAL AUTHORITY IN ACCORDANCE WITH
 THIS SECTION AND 633 FLORIDA STATUTE.
  
 2D) NOT COMPLETED. FOR THE SPECIAL INSPECTOR PLEASE
 PROVIDE A RESUME' OF INSPECTION EXPERIENCE ON PREVIOUS
 PROJECT. THE RESUME' MUST BE ACCOMPANIED BY ALL
 CERTIFICATES AS SPECIFIED HEREIN. THE BUILDING OFFICIAL
 OR HIS DESIGNEE WILL REVIEW THE RESUME'. AFTER THE
 RESUME' IS REVIEWED, AN INTERVIEW WILL BE SCHEDULED.
  
 2E) NOT COMPLETED. THE CONTRACTOR WILL PRODUCE A JOB
 SUMMARY OR HISTORY FOR THIS TYPE OF CONSTRUCTION.

 PROCEDURES, PLANS AND DETAILS SHALL BE SUBMITTED TO THE
 ENFORCEMENT AGENCY FOR RECORDKEEPING. EACH SHORING AND
 RESHORING INSTALLATION SHALL BE SUPERVISED, INSPECTED
 AND CERTIFIED TO BE IN COMPLIANCE WITH THE SHORING
 DOCUMENTS BY THE CONTRACTOR.

 805-6700) FOR ALL BUILDING INSPECTIONS FOR AUDITING OF
 JOBSITE INSPECTIONS WITHOUT THIS THE PERMIT EXPIRES IN
 6 MONTHS, ABANDONMENT OF JOB, NO INSPECTION ACTIVITY.
  
 3) PLEASE PROVIDE A SITE PLAN SHOWING THE PROPERTY
 LINES, CONSTRUCTION AREAS, STAGING AREAS, HOW THE
 OCCUPANTS OF THE BUILDING WILL BE SAFE GUARDED DURING
 THEIR ENTRY AND EXIT OF THE BUILDING. SHOW COMPLIANCE
 WITH 2014 FBC-B 3306.7 COVERED WALKWAYS INCLUDING
 LIGHTING FO THE EXIT DISCHARGE.
 2014 FBC-B 3310.2 MAINTENANCE OF MEANS OF EGRESS.
 REQUIRED MEANS OF EGRESS SHALL BE MAINTAINED AT ALL
 TIMES DURING CONSTRUCTION, DEMOLITION, REMODELING OR
 ALTERATIONS AND ADDITIONS TO ANY BUILDING.
  
 4A) IF SCAFFOLDING EXTENDS BEYOND THE PRIVATE PROPERTY
 AND ONTO CITY RIGHT OF WAY ENGINEERING WILL REQUIRE
 RIGHT OF WAY PERMITS. PLEASE CONTACT MANNY GONZALEZ AT
 THE ENGINEERING DEPARTMENT 494-1085.
  
 4B) IF SCAFFOLDING IS TO BE USED FOR THE HEIGHT OF THE
 BUILDING, PLEASE PROVIDE HOW THE SCAFFOLDING WILL BE
 BUILT AS WELL AS HOW THE SCAFFOLDING WILL BE ANCHORED
 TO RESIST LATERAL WIND LOADING. THERE APPEARS SAND
 BLASTING WILL ALSO BE A ELLEMENT OF THE OPERATION, HOW
 WILL THE SANDBLASTING BE CONTAINED, WILL THERE BE WIND
 SCREENS, IF SO PLEASE PROVIDE WHAT ELEMENTS WILL BE
 INTRODUCED TO KEEP THE SCAFFOLDING FROM MOVING WITH THE
 LATERAL AFFECT OF WIND ON THE WIND SCREENS. AT WHAT
 SAFETY FACTOR ARE THESE ELEMENTS FACTORED? 107.2.1.3
 ADDITIONAL INFORMATION. 2014 FBC-B 3302.1 & 3306.1.
  
 5) PLEASE PROVIDE COMPLIANCE WITH 2014 FBC-B 3309.1
 WHERE REQUIRED.
 ALL STRUCTURES UNDER CONSTRUCTION, ALTERATION OR
 DEMOLITION SHALL BE PROVIDED WITH NO FEWER THAN ONE
 APPROVED PORTABLE FIRE EXTINGUISHER IN ACCORDANCE WITH
 SECTION 906 AND SIZED FOR NOT LESS THAN ORDINARY HAZARD
 AS FOLLOWS:
  
 1. AT EACH STAIRWAY ON ALL FLOOR LEVELS WHERE
 COMBUSTIBLE MATERIALS HAVE ACCUMULATED.
  
 2. IN EVERY STORAGE AND CONSTRUCTION SHED.
  
 3. ADDITIONAL PORTABLE FIRE EXTINGUISHERS SHALL BE
 PROVIDED WHERE SPECIAL HAZARDS EXIST, SUCH AS THE
 STORAGE AND USE OF FLAMMABLE AND COMBUSTIBLE LIQUIDS.
  
 [F] 3309.2 FIRE HAZARDS.
 THE PROVISIONS OF THIS CODE AND THE FLORIDA FIRE
 PREVENTION CODE SHALL BE STRICTLY OBSERVED TO SAFEGUARD
 AGAINST ALL FIRE HAZARDS ATTENDANT UPON CONSTRUCTION
 OPERATIONS.
  
 6) DURING THE RESTORATION WORK ON THE BALCONIES HOW
 WILL THE TENANTS BE SAFE GUARDED FROM THE WORK AREA AND
 EGRESS ONTO THE BALCONIES? 3306.1 PROTECTION REQUIRED.
  
 7) ALL ADD-MIXTURES RHINOOPLASTIC CONCRETE ADDITIVES
 ARE TO BE REVIEWED BY THE ENGINEER OF RECORD AS ALL
 OTHER ADDITIVES SHOP DRAWING STAMP OF APPROVAL.
