 PROVIDE WRITTEN RESPONSES TO THE COMMENTS. PLEASE NOTE
 THAT THE SUBMITTAL OF THE REQUIRED ITEMS MAY RESULT IN
 ADDITIONAL COMMENTS
  
 1) REPEAT COMMENT: PROVIDE TWO COPIES OF A SURVEY, LESS
 THAN ONE YEAR OLD, WHICH SHOWS ALL IMPROVEMENTS AND
 EASEMENTS. A SURVEY WAS NOT PROVIDED FOR 1600 NORTH
 DIXIE.
  
 2) REVISED COMMENT: THE CONNECTION BETWEEN THE TWO
 BUILDINGS EXTENDS INTO THE REQUIRED FIVE (5) FOOT SIDE
 SETBACK, WHICH IS NOT PERMITTED. IN ORDER TO CONNECT
 THE TWO BUILDINGS, A RECORDED UNITY OF TITLE IS
 REQUIRED TO REMOVE THE PROPERTY LINE AND THUS THE
 REQUIRED SETBACK ALONG WHAT IS CURRENTLY THE SHARED
 PROPERTY LINE. YOU WILL NEED TO PROVIDE A RECORDED COPY
 OF THE COMPLETED UNITY OF TITLE PRIOR TO ZONING
 APPROVAL OF THIS PERMIT.
  
 3) REVISED COMMENT: THE PERMIT PACKAGE INCLUDES A
 PROPOSED SITE PLAN THAT RECONFIGURES THE EXISTING
 PARKING LOT. THE PARKING LOT WORK REQUIRES PARKING LOT
 PERMIT, WHICH SHALL BE SUBMITTED, WITH PLANS, FOR TO
 THE BUILDING DEPARTMENT PRIOR TO ZONING APPROVAL OF
 THIS BUILDING PERMIT - NO PERMIT CURRENTLY SUBMITTED.
 NOTE: THE PROPOSED PARKING LOT SHOWS ONE LESS PARKING
 SPACE THAN THE APPROVED PLAN FOR THE BUILDING. SINCE
 THE APPROVED PARKING IS DEFICIENT FOR A MEDICAL OFFICE
 BUILDING, NO ADDITIONAL PARKING REDUCTIONS ARE
 PERMITTED.
  
 4) IN ACCORDANCE WITH THE PROVISOS APPROVED AS PART OF
 PERMIT NO. 15100589, THE CURBING AND STRIPING SHOWN ON
 THE ISSUED PARKING LOT PERMIT SHALL BE COMPLETED PRIOR
 TO THE FINAL (104) INSPECTION/CERTIFICATE OF COMPLETION
 OF THE VANILLA BOX PERMIT OR CERTIFICATE OF OCCUPANCY
 FOR THE SPACE, WHICHEVER FIRST OCCURS.
  
 5) REPEAT COMMENT: THE VALUE OF THE CONSTRUCTION
 ($241,020) EXCEEDS 50% OF THE STRUCTURE'S VALUE
 ($339,394) ACCORDING TO THE P.B.C. PROPERTY APPRAISER'S
 OFFICE; THEREFORE, PURSUANT TO SECTION 94-441(C)(2) OF
 THE ZONING AND LAND DEVELOPMENT REGULATIONS (ZLDR), THE
 LANDSCAPE ON THE SITE SHALL BE IN COMPLIANCE WITH ALL
 APPLICABLE REGULATIONS FOUND IN ARTICLE XIV OF THE
 ZLDR. SINCE THE SITE DOES NOT CONFORM TO ARTICLE XIV,
 THEN PRIOR TO THE ZONING APPROVAL OF THIS PERMIT A
 SEPARATE LANDSCAPE PERMIT AND IRRIGATION PERMIT SHALL
 BE SUBMITTED SHOWING MATERIAL TO BRING THE SITE INTO
 CONFORMITY. THE LANDSCAPE WORK SHALL BE COMPLETED PRIOR
 TO A CERTIFICATE OF OCCUPANCY FOR THE BUILDING. NO
 LANDSCAPE PLAN HAS BEEN SUBMITTED.
  
 6) STAFF SUGGESTS REVIEWING THE OPTION OF REMOVING THE
 POWER POLES AND BURYING THE UTILITY LINES AS THIS WILL
 IMPROVE THE AESTHETICS OF THE SITE. IT APPEARS EACH
 POLE ONLY SERVES ONE OF THE BUILDINGS. ANOTHER OPTION
 MAY BE TO CONSOLIDATE THE SERVICES ON ONE POLE.
  
 7) SEPARATE PERMITS, WITH PLANS, ARE REQUIRED FOR:
