 FLORIDA BUILDING CODE WHICH EXCEEDS 5,000 SQ. FT. IN
 AREA AND AN OCCUPANT CONTENT OF GREATER THAN 500
 PERSONS.

 FILED OUT BY ALL PARTIES WITH INTEREST, NOTARIZED AND
 RETURNED TO THIS OFFICE, AND REVIEWED AND APPROVED
 BEFORE PERMIT ISSUANCE WILL TAKE PLACE. CONTACT KEN

 BLDG., REQUIRED STATEMENT: W.P.B. AMENDMENT 110.3.7.4.4
 ALL PLANS FOR THE BUILDING WHICH ARE REQUIRED TO BE
 SIGNED AND SEALED BY THE ARCHITECT OR ENGINEER OF
 RECORD CONTAIN A STATEMENT THAT, TO THE BEST OF THE
 ARCHITECT'S OR ENGINEER'S KNOWLEDGE, THE PLANS AND
 SPECIFICATIONS COMPLY WITH THE APPLICABLE FIRE-SAFETY
 STANDARDS AS DETERMINED BY THE LOCAL AUTHORITY IN
 ACCORDANCE WITH THIS SECTION AND 633 FLORIDA STATUTE.

 SUMMARY OR HISTORY FOR THIS TYPE OF CONSTRUCTION.
  
 1F) NOTE: FOR PAPER PLANS 3 SETS OF DOCUMENTS ARE
 REQUIRED. FOR DIGITAL PLANS, AFTER PERMIT ISSUANCE,
 PLEASE E-MAIL THE PERMIT LIBRARIAN OF THE PERMIT BEING
 ISSUED, THE PERMIT LIBRARIAN WILL THEN E-MAIL YOU THE
 APPROVED AND BATCH STAMPED PLANS SO YOU MAY PRINT YOUR
 PLANS, TWO SETS OF PLANS WILL NEED TO BE PRINTED IN
 COLOR, (1) FOR THE THRESHOLD INSPECTOR, THE OTHER FOR
 THE MEP INSPECTORS OTHER PLANS YOU WISH TO PRINT MAYBE
 IN BLACK & WHITE..

 6 MONTHS, ABANDONMENT OF JOB, NO INSPECTION ACTIVITY.

 BUILDINGS, OPERATING GUIDELINES FOR THRESHOLD
 INSPECTORS #26. ASSURE THAT THE DESIGNER HAS REQUIRED
 ON THE PLANS OR SPECIFICATIONS ONE MATERIAL, APPROVED
 FOR FIRE STOPPING PENETRATIONS, FOR WALLS AND CEILINGS.
  
 2) 2ND REQUEST, SEMI-COMPLIED. SOILS REPORT WAS
 SUBMITTED. PLEASE NOTE NEITHER THE ARCHITECT OF RECORD
 NOR THE STRUCTURAL ENGINEER INDICATE THEY HAVE REVIEWED
 THE SOILS REPORT AND THEIR FINDINGS OF LOOSE SOILS,
 VIBRO-COMPACTION, UNDERPINNING THE EXISTING BUILDINGS
 OR THE POSSIBLE USE OF DEEP FOUNDATION AUGER-CAST PILE.
 DEPENDING ON THE FINAL DECISION IT APPEARS THERE WILL
 BE ADDITIONAL DOCUMENTS ON THE UNDER-PINNING PROCESS OR
 THE AUGER CAST PILE.

 2014 FBC-B 1803.5.2 QUESTIONABLE SOIL.
 WHERE THE CLASSIFICATION, STRENGTH OR COMPRESSIBILITY
 OF THE SOIL IS IN DOUBT OR WHERE A LOAD-BEARING VALUE
 SUPERIOR TO THAT SPECIFIED IN THIS CODE IS CLAIMED, THE
 BUILDING OFFICIAL SHALL BE PERMITTED TO REQUIRE THAT A
 GEOTECHNICAL INVESTIGATION BE CONDUCTED.
  
 3) COMPLIED. RELOCATION OF HEDRICK BROTHERS STAMP
 RECEIVED.
  
 4) 2ND REQUEST. IMPACT FEES BEFORE A PERMIT TO
 CONSTRUCT, MAY BE ISSUED, IMPACT FEES MUST BE PAID TO
 PALM BEACH COUNTY. THE ACTUAL PERMIT SET OF PLANS MUST
 BE STAMPED BY THAT OFFICE, AND A COPY OF THE PAID
 RECEIPT ATTACHED TO THE PERMIT APPLICATION. PLEASE CALL

 5A) SHEET 031_LS-0.1 CODE DATA FBC INDICATES THE GARAGE
 ADDITION TO BE ONLY A 5 STORY IN HEIGHT. PLEASE PROVIDE
 THE ELEVATION SHEETS. IF THE ROOF LEVEL OF THE PARKING
 GARAGE IS USEABLE DECK THEN IT IS CONSIDERED A STORY IF
 OVER 30 PER CENT OF THE ROOF DECK. FBC-B 1509.2. ALSO
 SEE THE DEFINITION OF STORY.
  
 5B) I DO NOT SEE THE LIVE LOAD DESIGN LOADS IN NEITHER
 THE LIFE SAFETY PLAN NOR THE STRUCTURAL PLANS, THE 6TH
 FLOOR /ROOF LEVEL PLAN TO REVIEW THE FLOOR LOADING IN
 ACCORDANCE WITH FBC-B TABLE 1607.1 TO INSURE THE PROPER
 FOUNDATION SIZE HAS BEEN FIGURED FOR THIS BUILDING.
  
 6) A TRANSMITTAL LETTER LISTING THE ORIGINAL REVIEW
 COMMENT NUMBER, WITH A DESCRIPTION OF THE REVISION
 MADE, IDENTIFYING THE SHEET OR SPECIFICATION PAGE WHERE
 THE CHANGES CAN BE FOUND WILL HELP TO EXPEDITE YOUR
