 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 1) THE ELEVATION CERTIFICATE THAT WAS SUBMITTED SHOWS
 THAT THE PROJECT IS IN VIOLATION OF THE CITY OF WEST
 PALM BEACH'S FLOOD ORDINANCE AND FEMA REQUIREMENTS FOR
 CONSTRUCTION IN A SPECIAL FLOOD HAZARD AREA. BASED ON
 THE ELEVATION CERTIFICATE FOR FINISHED CONSTRUCTION
 THAT WAS SUBMITTED, THE REQUIREMENTS OF APPROVED PLANS
 FOR THE ADDITION OF A SECOND FLOOR ABOVE THE EXISTING
 GARAGE (#14011176) AND THE REVISION TO ADD THE TWO
 STORY ADDITION BETWEEN THE GARAGE AND THE DWELLING
 (#15050172) HAVE NOT BEEN COMPLIED WITH.
  
 2) PER THE APPROVED PLANS, THE EXISTING FIRST FLOOR
 GARAGE AND THE ONE CAR GARAGE ON THE FIRST FLOOR OF THE
 TWO STORY ADDITION ARE BELOW THE REQUIRED DESIGN FLOOD
 ELEVATION OF 7.5 FEET NGVD AND ARE REQUIRED TO HAVE
 FLOOD OPENINGS. ONLY FLOOD RESISTANT MATERIALS AND
 EQUIPMENT CAN BE INSTALLED BELOW THE 7.5 FEET DFE. THE
 CITY OF WEST PALM BEACH'S FLOOD ORDINANCE AND FEMA
 REQUIRE THAT THESE AREAS CAN ONLY BE USED FOR THE
 PARKING OF VECHICLES, BUILDING ACCESS AND STORAGE
 (LOW-VALUE ITEMS).
  
 3) THE ELEVATION CERTIFICATE INDICATES THAT THE FORMER
 GARAGE (ELEVATION 5.6 FEET) IS A PERSONAL GYM AND THAT
 THERE IS EQUIPMENT SERVICING THE BUILDING THAT IS BELOW
 THE REQUIRED DESIGN FLOOD ELEVATION OF 7.5 FEET NGVD.
  
 4) THE ELEVATION CERTIFICATE IS REQUIRED TO INCLUDE
 PHOTOGRAPHS OF THE FLOOD OPENINGS OR VENTS.
