 DENIED BY BUILDING
 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 1) THE SURVEY SPECIFIES THE PROPERTY AS BEING IN FLOOD
 ZONE B WHEN IT IS ACTUALLY IN FLOOD ZONE C. IF NOT
 CORRECTED, THIS MAY RESULT IN THE HOMEOWNERS PAYING
 MORE FOR FLOOD INSURANCE.
 I DID NOT FIND THE NEW SURVEY THAT WAS REFERENCED IN
 YOUR RESPONSE LETTER IN THE RE-SUBMITTAL.
  
 2) SHOW COMPLIANCE WITH FBC-R806.1 VENTILATION
 REQUIRED. ENCLOSED ATTICS AND ENCLOSED RAFTER SPACES
 FORMED WHERE CEILINGS ARE APPLIED DIRECTLY TO THE
 UNDERSIDE OF ROOF RAFTERS SHALL HAVE CROSS VENTILATION
 FOR EACH SEPARATE SPACE BY VENTILATING OPENINGS
 PROTECTED AGAINST THE ENTRANCE OF RAIN OR SNOW.
 VENTILATION OPENINGS SHALL HAVE A LEAST DIMENSION OF
 1/16 INCH MINIMUM AND 1/4 INCH MAXIMUM. VENTILATION
 OPENINGS HAVING A LEAST DIMENSION LARGER THAN 1/4 INCH
 SHALL BE PROVIDED WITH CORROSION-RESISTANT WIRE CLOTH
 SCREENING, HARDWARE CLOTH, OR SIMILAR MATERIAL WITH
 OPENINGS HAVING A LEAST DIMENSION OF 1/16 INCH MINIMUM
 AND 1/4 INCH MAXIMUM. OPENINGS IN ROOF FRAMING MEMBERS
 SHALL CONFORM TO THE REQUIREMENTS OF SECTION R802.1.8.
 REQUIRED VENTILATION OPENINGS SHALL OPEN DIRECTLY TO
 THE OUTSIDE AIR. CALCULATIONS FOR THE REQUIRED AREA OF
 VENTILATION OPENINGS WERE ADDED TO THE PLANS. I DID NOT
 SEE THE LOCATIONS AND SIZES OF THE VENT OPENINGS SHOWN
 ON THE PLANS.
  
 3) FBC 107.3.4 - PROVIDE TWO SETS OF PRODUCT APPROVALS
 FOR THOSE PRODUCTS WHICH ARE REGULATED BY DCA RULE 9N-3
 THAT HAVE BEEN REVIEWED AND APPROVED IN WRITING BY THE
 DESIGNER OF RECORD. PROVIDE TWO COMPLETE PRODUCT
 APPROVALS WITH FL OR NOA NUMBERS INCLUDING INSTALLATION
 INSTRUCTIONS AND MAXIMUM DESIGN PRESSURES FOR EACH OF
 THE DIFFERENT ROOFING MATERIALS, WINDOW/GLASS TYPES,
 STRUCTURAL MULLIONS AND EXTERIOR DOOR TYPES (INCLUDING
 SECTIONAL GARAGE DOORS).
 THE FOLLOWING IS A LINK TO THE MIAMI - DADE PRODUCT

 4) THE ENERGY PERFORMANCE EVALUATION FORM CONTAINS
 ERRORS AND IS MISSING INFORMATION. THE FORMS REQUIRE
 THE SIGNATURE OF THE OWNER/AGENT. THE STREET IS LAKE
 AVE. AND NOT LAKE WORTH AVE. THE CITY IS WEST PALM
 BEACH AND NOT LANTANA. THE JURIDICTION NUMBER IS
 604700. THE UNITS DO NOT MEET THE CRITERIA IN FBC
 ENERGY CONSERVATION SECTION R405.7.4 TO USE THE CROSS
 VENTILATION OPTION CREDIT. THE FORMS SPECIFY AN
 UNVENTED ATTIC AND THE PLANS SHOW A VENTILATED ATTIC.
 THE FORMS SPECIFY A CONCRETE ROOF AND THE PLANS HAVE A
 PLYWOOD ROOF WITH ASPHALT ROOFING.
  
 5) ACCORDING TO THE DRAWING INDEX ON SHEET CV-1.0,
 SHEET A-6.0 IS MISSING. SHEETS 1, 2 AND 5 FROM FRANCIS
 ENGINEERING ( FIRE SPRINKLER PLANS? ) ARE ALSO MISSING.
  
 6) THE SIGNATURE OF THE STRUCTURAL ENGINEER APPEARS TO
 BE A FASCIMILE. FLORIDA STATUTES REQUIRE THAT ALL
 DRAWINGS AND DOCUMENTS FROM ENGINEERS, AS WELL AS,
 ARCHITECTS THAT ARE SUBMITTED FOR PUBLIC RECORD HAVE
 THEIR HANDWRITTEN SIGNATURES ON THEM IN ADDITION TO
 THEIR SEALS. FAC 61G15-23.002 AND 61G1-16.003
  
 7) IN ORDER TO REDUCE PLAN REVIEW TIME, PLEASE SUBMIT A
 RESPONSE LETTER INDICATING HOW EACH OF THE REVIEW
