 FLORIDA STATUTES STATES THAT A LOCAL GOVERNMENT SHALL
 IMPOSE A FEE OF FOUR TIMES THE FEE FOR PLAN REVIEW, IF
 PLANS ARE REJECTED (3) OR MORE TIMES FOR REPEATED
 FAILURE TO CORRECT A CODE VIOLATION.

 THE ARCHITECT OF RECORDS OWN SEAL, BUT THE SHEETS 1 OF
 2 FOR WINDOW PLACEMENT, CUSTOM MULLION AND WIND DESIGN
 NOT REPLACED, STILL HAS THE CERTIFICATE OF
 AUTHORIZATION SEAL. L DESIGN FOR WIND BY RAYOMND
 CHLADNY ARCHITECT, NOTE THE DESIGNER OF RECORD SEALED
 THE PLANS WITH HIS AA CERTIFICATE OF AUTHORITY SEAL. 61
 G 1-16.003 USES OF SEAL. EFFECTIVE 04/18/2000 THE
 PERSONAL SEAL, SIGNATURE AND DATE OF THE ARCHITECT OR
 INTERIOR DESIGNER SHALL APPEAR ON ALL ARCHITECTURAL OR
 INTERIOR DESIGN DOCUMENTS TO BE FILED FOR PUBLIC RECORD
 AND SHALL BE CONSTRUED TO OBLIGATE HIS PARTNERS OR HIS
 CORPORATION. A CORPORATE SEAL ALONE IS INSUFFICIENT.
 DOCUMENTS SHALL BE SIGNED PERSONALLY AND SEALED BY THE
 RESPONSIBLE ARCHITECT
  
 2) 2ND REQUEST. PLEASE NOTE THE WIND DESIGGN CRITERIA
 WAS PROVIDED, WHAT WAS NOT PROVIDED WAS THE SIZE OF THE
 WINDOWS AND DOORS NOR WHERE ZONE 4 AND 5 START AND END.
 THE PRESSUES NEED TO ALIGN WITH THE SIZE OF OPENING AND
 ZONE EITHER 4 OR 5. PLEASE PROVIDE THE ACTUAL SIZES OF
 THE WINDOWS AND SLIDERS TO BE INSTALLED. NOTE, FOR THE
 SLIDDING GLASS DOORS, IDENTIFY THE SIZES OF PANELS
 LOCATED ON SHEET 7 OF THE PRODUCT APPROVAL. CIRCLE THE
 SIZE OF THE PANEL, TYPE OF GLAZING, AND SILL HEIGHT
 THAT GOVERNS EACH CHART.

 THOSE PRODUCTS WHICH ARE REGULATED BY THE DCA RULE
 9N-03 SHALL BE REVIEWED AND APPROVED IN WRITING BY THE
 DESIGNER OF RECORD PRIOR TO SUBMITTAL FOR
 JURISDICTIONAL APPROVAL.

 SUPERVISING AND REVIEWING ALL PROJECT DATA, REPORTS,

 SUBMITTED, NO TESTING, SUPER SAM 125 HAS A IIC RATING
 OF 49, MINIMUM REQUIRED IS 50. PLEASE PROVIDE TESTING
 FOR CODE COMPLIANCE FOR FLOOR INSULATION: BLDG TYPE:
 TYPE I/ TYPE II FLOOR INSULATION
  
 4A) FBC-B 2010 1207.3 STRUCTURE-BORNE SOUND.
 FLOOR/CEILING ASSEMBLIES BETWEEN DWELLING UNITS OR
 BETWEEN A DWELLING UNIT AND A PUBLIC OR SERVICE AREA
 WITHIN THE STRUCTURE SHALL HAVE AN IMPACT INSULATION
 CLASS (IIC) RATING OF NOT LESS THAN 50 (45 IF FIELD
 TESTED) WHEN TESTED IN ACCORDANCE WITH ASTM E 492.
  
 4B) FBC-B 2010 1207.2 1207.2 AIR-BORNE SOUND.
 WALLS, PARTITIONS AND FLOOR/CEILING ASSEMBLIES
 SEPARATING DWELLING UNITS FROM EACH OTHER OR FROM
 PUBLIC OR SERVICE AREAS SHALL HAVE A SOUND TRANSMISSION
 CLASS (STC) OF NOT LESS THAN 50 (45 IF FIELD TESTED)
 FOR AIR-BORNE NOISE WHEN TESTED IN ACCORDANCE WITH ASTM
 E 90.
  
 5) FLAME SPREAD- FBC-B 2010 603.1.2 EXCEPTION 2.
 INSULATION INSTALLED BETWEEN A FINISHED FLOOR AND SOLID
 DECKING WITHOUT INTERVENING AIRSPACE SHALL BE ALLOWED
 TO HAVE A FLAME SPREAD INDEX OF NOT MORE THAN 200.
  
 FLAME SPREAD INDEX. A COMPARATIVE MEASURE, EXPRESSED AS
 A DIMENSIONLESS NUMBER, DERIVED FROM VISUAL
 MEASUREMENTS OF THE SPREAD OF FLAME VERSUS TIME FOR A
 MATERIAL TESTED IN ACCORDANCE WITH ASTM E 84 OR UL 723.
  
 703.4 NONCOMBUSTIBILITY TESTS.
 THE TESTS INDICATED IN SECTIONS 703.4.1 AND 703.4.2
 SHALL SERVE AS CRITERIA FOR ACCEPTANCE OF BUILDING
 MATERIALS AS SET FORTH IN SECTIONS 602.2, 602.3 AND
 602.4 IN TYPE I, II, III AND IV CONSTRUCTION. THE TERM
 ?NONCOMBUSTIBLE? DOES NOT APPLY TO THE FLAME SPREAD
 CHARACTERISTICS OF INTERIOR FINISH OR TRIM MATERIALS. A
 MATERIAL SHALL NOT BE CLASSIFIED AS A NONCOMBUSTIBLE
 BUILDING CONSTRUCTION MATERIAL IF IT IS SUBJECT TO AN
 INCREASE IN COMBUSTIBILITY OR FLAME SPREAD BEYOND THE
 LIMITATIONS HEREIN ESTABLISHED THROUGH THE EFFECTS OF
 AGE, MOISTURE OR OTHER ATMOSPHERIC CONDITIONS.
  
 703.4.1 ELEMENTARY MATERIALS.
 MATERIALS REQUIRED TO BE NONCOMBUSTIBLE SHALL BE TESTED
 IN ACCORDANCE WITH ASTM E 136.
  
 703.4.2 COMPOSITE MATERIALS.
 MATERIALS HAVING A STRUCTURAL BASE OF NONCOMBUSTIBLE
 MATERIAL AS DETERMINED IN ACCORDANCE WITH SECTION
 703.4.1 WITH A SURFACING NOT MORE THAN 0.125 INCH (3.18
 MM) THICK THAT HAS A FLAME SPREAD INDEX NOT GREATER
 THAN 50 WHEN TESTED IN ACCORDANCE WITH ASTM E 84 OR UL
 723 SHALL BE ACCEPTABLE AS
 NONCOMBUSTIBLE MATERIALS.
  
 6) 2010 WEST PALM BEACH AMENDMENTS TO THE FLORIDA
 BUILDING CODE, CHAPTER 1, ADMINISTRATION, 107.3.5
 MINIMUM PLAN REVIEW CRITERIA FOR BUILDINGS. THE
 EXAMINATION OF THE DOCUMENTS BY THE BUILDING OFFICIAL
 SHALL INCLUDE THE FOLLOWING MINIMUM CRITERIA AND
 DOCUMENTS: A FLOOR PLAN.
  
 7) WHEN RESUBMITTING PLANS PLEASE INDICATE THE REVISION
 & REMOVE ANY VOIDED SHEETS & REPLACE ANY PAGES AS
 NECESSARY. A TRANSMITTAL LETTER LISTING THE ORIGINAL
 REVIEW COMMENT NUMBER, WITH A DESCRIPTION OF THE
 REVISION MADE, IDENTIFYING THE SHEET OR SPECIFICATION
 PAGE WHERE THE CHANGES CAN BE FOUND WILL HELP TO
 EXPEDITE YOUR PERMIT. THANK YOU FOR YOUR ANTICIPATED
 COOPERATION.
