| APPLICATION STATES THAT THE REROOF WAS INSULATION OVER |
| A CONCRETE DECK. THE DOCUMENTS SUBMITTED BY CONTRACTOR |
| STATE THAT THIS IS A WOOD DECK, SO NO MOISTURE SURVEY |
| IS REQUIRED. PLEASE RESOLVE THIS DISCREPANCY. |
