 FOR YOUR CONVENIENCE, A SUMMARY OF THE DOCUMENTS
 SUBMITTED ARE AT THE END OF THIS PLAN REVIEW.
  
 1. DUE TO THE NUMBER OF PERMITS, IT IS RECOMMENDED THAT
 A "MASTER" BE GENERATED. THE PERMIT DOCUMENTS WOULD BE
 STORED UNDER THE MASTER, PERMIT CARDS WOULD REFERENCE
 THE MASTER, AND CONTRACTOR WOULD PROVIDE A FEW SETS OF
 DOCUMENTS - ONE FOR THIS DEPARTMENT TO RETAIN, AND A
 FEW SETS FOR INSPECTION PURPOSES (THE NUMBER WOULD
 DEPEND ON THE NUMBER OF BUILDINGS FOR WHICH INSPECTIONS
 WOULD BE CALLED IN ON ONE DAY AS IT IS A REQUIREMENT TO
 HAVE THE PLANS PROVIDED ON THE JOBSITE AT TIME OF
 INSPECTION). A MINIMUM OF THREE INSPECTION SETS ARE
 RECOMMENDED.
  
 IF CONTRACTOR ELECTS NOT TO USE A MASTER, EACH PERMIT
 PACKAGE IS TO BE REVISED TO ADDRESS ALL COMMENTS. THE
 NOA WAS SUBMITTED WITH SHEET 2 ON THE TOP OF THE
 PACKAGE. WHEN RESUBMITTING, PLEASE PUT NOA PAGES IN
 ORDER. THIS HAS BEEN DONE FOR YOU FOR APPROXIMATELY
 HALF OF THE APPLICATIONS.
  
 2. PROVIDE INFORMATION REGARDING THE EXISTING ROOF. THE
 PERMIT DOCUMENTS SHOULD INCLUDE ALL INFORMATION
 REQUIRED TO DEMONSTRATE CODE COMPLIANCE, FBC 107.
  
 BASED ON TWO RECORDS, (PERMIT #01041400 FOR ADDRESSES
 7113,7114 71ST WAY AND 7223 7224 72ND WAY, ALSO SEE
 01041405, 01041403), IT APPEARS THAT THE EXISTING ROOF
 SYSTEM IS TPO, INSULATED DECK (NOA 98-0305.02, JPS
 ELASTOMERIC CORPORATION, STEVENS ROOFING SYSTEMS,
 SINGLE PLY ROOFING SYSTEM OVER CONCRETE DECK, NOA
 SUB-TYPE TPO). DESCRIPTIONS OF ADDRESSES INCLUDED IN
 THESE THREE PERMITS ARE AVAILABLE ON THE INTERNET. I
 HAVE VERIFIED THAT BOTH 01041400 AND 01041405 WERE
 ISSUED WITH THE TPO ROOF NOA REFERENCED. EXISTING ROOF
 TYPE SHOULD BE FIELD VERIFIED BY CONTRACTOR.
  
 3. SEE PAGE 3, PVC SINGLE PLY ROOF SYSTSMS IS
 UNDERLINED. BASED ON OUR PERMIT RECORDS FOR THIS
 PROJECT, THE ROOF IS TPO. TPO IS NOT ONE OF THE SYSTEMS
 INCLUDED IN THE MIAMI DADE NOA. PLEASE ADDRESS BY
 PROVIDING INFORMATION FROM THE MANUFACTURER WHICH
 DOCUMENTS THE SUITABILITY OF THIS APPLICATION.
  
 4. PROVIDE SPEC SHEETS FOR THE GACOWASH. THIS SHOULD
 PROVIDE INFORMATION ABOUT THE PRODUCT AS WELL AS
 RECOMMENDED DILUTION RATE, FBC 107.
  
 5. E5320 IS SPECIFIED AS A "PRIMER COAT" IN THE
 PROPOSAL. PROVIDE MANUFACTURER SPEC SHEETS/INSTALLATION
 INSTRUCTIONS, FBC 107. APPLICATION PROPOSES "PRIMER" AT
 A RATE OF 1 GALLON / 250 SF. SINCE THIS PRODUCT DOES
 NOT APPEAR TO HAVE A PRODUCT APPROVAL (IT IS ONLY
 REFERENCED IN THE ROOF COATING SYSTEMS IN THE NOA),
 PROVIDE MANUFACTURER SPEC SHEETS WHICH INCLUDE
 INFORMATION ABOUT THE PRODUCT AND THE RECOMMENDED
 APPLICATION METHOD AND RATE. ALTHOUGH IT DOES NOT HAVE
 A PRODUCT APPROVAL, IT APPEARS THAT THIS IS THE PRODUCT
 WHICH ASSURES THE BOND REQUIRED FOR WIND RESISTANCE FBC
 1609.
  
 6. APPLICATION AND PROPOSAL SPECIFY GACO FLEX S2000,
 1.5 GALLONS PER 100 SF. SEE PAGE 3 OF THE NOA; TWO
 COATS AT A MINIMUM RATE OF .75 GAL / 100' EACH ARE
 REQUIRED. ALTHOUGH THE TOTAL GALLONS PER SF IS
 CONSISTENT WITH THE NOA, THE APPLICATION METHOD
 PROPOSED ("A TOP COAT") DOES NOT COMPLY WITH THE
 REQUIREMENTS OF THE NOA.
  
 7. ONLY ONE PAGE OF A THREE PAGE CONTRACT WAS PROPOSED.
 PROVIDE THE COMPLETE CONTRACT, FBC 109.
  
 8. IF METHOD OF INSTALLATION AS PROPOSED ON THE
 APPLICATION IS NOT APPROVED, NEW APPLICATIONS WILL BE
 REQUIRED. IT IS RECOMMENDED TO HAVE ONLY INFORMATION
 DESCRIBING THE PROJECT ON THE PERMIT APPLICATION (WHICH
 SHOULD INCLUDE ROOF AREA, TYPE OF ROOF PROPOSED, MEAN
 ROOF HEIGHT, DECK TYPE), AND HAVE SPECIFICATIONS FOR
 INSTALLATION IN THE PERMIT DOCUMENTS. IT IS NOT TYPICAL
 TO SPEC INSTALLATION IN THE PERMIT DESCRIPTION. AS THE
 PERMIT DESCRIPTION ON THE PERMIT APPLICATION DOES NOT
 APPEAR TO BE CODE COMPLIANT, A NEW PERMIT APPLICATION
 WILL BE REQUIRED UNLESS IT IS DETERMINED THAT THE
 INSTALLATION METHOD PROPOSED ON THE APPLICATION IS
 ACCEPTABLE AND A PERMIT IS ISSUED WITH THIS SPEC.
  
 9. PERMIT 02090676 FOR 7404 74TH WAY IS EXPIRED (DID
 NOT OBTAIN A FINAL INSPECTION). IF THIS PROJECT
 INCLUDES THIS ADDRESS, THIS PERMIT IS TO BE RESOLVED
 PRIOR TO ISSUANCE OF THE PERMIT FOR THAT BUILDING.
  
 TO RESOLVE THE EXPIRED PERMIT:
  
 APPLICATION WITH THE DESCRIPTION: "FINAL INSPECTION
 ONLY TO CLOSE EXPIRED PERMIT #02090676" TO BE SUBMITTED
 AT THE BUILDING DIVISION USING THE STANDARD COUNTY WIDE
 BUILDING PERMIT APPLICATION. A RECORDS RESEARCH REQUEST
 SHOULD BE SUBMITTED AT THE SAME TIME TO OBTAIN A COPY
 OF THAT PERMIT. IF AVAILABLE, IT WILL BE SUPPLIED FOR
 YOU.
