 ENGINEER'S PLAN; ENGINEER'S PLAN IS TO PROVIDE ENOUGH

 5. GAZEBO FOUNDATION, PROVIDE A STRUCTURAL DETAIL FBC
 107. THE PLAN INDICATES THAT THIS IS "BY OTHERS."
  
 2ND REVIEW, FOUNDATION PLAN FOR THE GAZEBO WAS NOT
 SUBMITTED, SO THERE IS NOTHING FOR EITHER REVIEW OR
 INSPECTION. THIS PORTION IS EXCLUDED FROM THIS PERMIT
 UNTIL A PLAN IS MADE AVAILABLE.
  
 6. FBC 2010 ACCESSIBILITY FAIR HOUSING SECTION 3, ITEM
 C(1) REQUIRES THAT "THE PUBLIC AND COMMON USE AREAS ARE
 READILY ACCESSIBLE AND USABLE BY THE HANDICAPPED
 PERSONS". THE GAZEBO IS GRAPHICALLY SHOWN 6" ABOVE
 GRADE. PROVIDE A DETAIL FOR ACCESSIBLE ENTRY.
  
 2ND REVIEW, NOT ADDRESSED. CONTRACTOR STATED THAT A
 RAMP WILL BE BUILT, AND AN EMAIL BETWEEN TWO
 INDIVIDUALS MAKING REFERENCE TO A RAMP WAS PROVIDED
 (RELATIONSHIP TO JOB UNCLEAR; NEITHER WAS THE
 CONTRACTOR OR THE ENGINEER). REVISION IS TO INCLUDE
 ACCESSIBLE ROUTE TO THE GAZEBO AS WELL AS GAZEBO
 FOUNDATION.
  
 7. FLORIDA STATE OR LOCAL PRODUCT APPROVAL REQUIRED FOR
 THE GAZEBO SHINGLES, FAC9N-3.
  
 2ND REVIEW, A FLORIDA STATE PRODUCT APPROVAL COVER
 SHEET WAS PROVIDED. THIS APPROVAL INCLUDES NINE
 DIFFERENT PRODUCTS; SPECIFIC PRODUCT PROPOSED NOT
 INDICATED AND THE EVALUATION REPORT/INSTALLATION
 INSTRUCTIONS WERE NOT PROVIDED. WITH EITHER THE
 REVISION OR THE ROOF PERMIT, PROVIDE A COMPLETE PRODUCT
 APPROVAL. THIS SHOULD BE REVIEWED BY EOR PRIOR TO
 SUBMITTAL, FBC 107.3.4.
  
 8. ADJUST VALUE TO INCLUDE ALL PROPOSED IMPROVEMENTS.
 PROVIDE A COPY OF THE CONTRACT(S), FBC 109.

 VALUE OF LABOR, MATERIALS, OVERHEAD, PROFIT,
 ENGINEERING - EVERYTHING EXCEPT FOR THE VALUE OF LAND.

 9. PROVIDE SLOPE AND CROSS SLOPE FOR SIDEWALKS, FBC
 2010 ACCESSIBILITY FAIR HOUSING SECTION 3 (ACCESSIBLE

 NEW COMMENTS:
  
 A. PROVIDE AWNING FABRIC REMOVAL INSTRUCTIONS TO SHOW
 COMPLIANCE WITH FBC3105.4.2.1, QUICK RELEASE. THE
 INSTRUCTIONS SHOULD CONSIST OF A SERIES OF PHOTOGRAPHS.
 THIS IS TO BE APPROVED BY THE BUILDING OFFICIAL.
  
 B. PER BUILDING OFFICIAL, WE ARE TO PUT ON THE PERMIT
 FILE A LETTER, OR CONTRACTORS FORM, SIGNED BY THE OWNER
 (NOT THE TENANT) TO ACKNOWLEDGE THEIR RESPONSIBILITY TO
 HAVE THE AWNING COVER REMOVED PRIOR TO ANY WIND EVENT
 WITH PROJECTED WIND SPEEDS OF 75 MPH OR GREATER. THE
 LETTER/FORM MUST BE SIGNED BY THE OWNER BUT IT NEED NOT
 BE NOTARIZED.
  
 C. THE PLANS FOR THE GAZEBO AND PERGOLA ARE
 PHOTOCOPIES. DOCUMENTS PREPARED BY AN ENGINEER AND
 SUBMITTED FOR PUBLIC RECORD ARE TO BE SIGNED, SEALED,
 DATED ORIGINALS FS471. SUBMIT ORIGINALS WITH THE
