 ELECTRONICALLY
 UPON APPLICATION AND FEE PAYMENT DESIGN DOCUMENTS
 SUBMISSION OPTIONS ARE AS FOLLOWS:
 1 UPLOAD VIA THE PROJECTDOX PORTAL WITH DIGITAL
 SIGNATURES OF DESIGN PROFESSIONALS AS APPLICABLE; USE
 THE REQUIRED NAMING CONVENTION FOR EACH DOCUMENT
 2 INCLUDE ON A CD IF DESIGN PROFESSIONAL DIGITAL
 SIGNATURES ARE PROVIDED AS REQUIRED; USE THE REQUIRED
 NAMING CONVENTION FOR EACH DOCUMENT
 3 INCLUDE ON A CD WITH ONE SET OF TRADITIONAL PAPER
 DESIGN DOCUMENTS SIGNED AND SEALED AS REQUIRED; USE THE
 REQUIRED NAMING CONVENTION FOR EACH DOCUMENT
 4 ONE SET OF TRADITIONAL PAPER DESIGN DOCUMENTS SUBJECT
 TO ADMINISTRATIVE FEES TO DIGITIZE THE DOCUMENTS
 ADDITIONAL INFORMATION WILL BE PROVIDED WHEN IT BECOMES

 1) THIS PERMIT IS FOR THE TEMPORARY INSTALLATION OF A
 CONSTRUCTION MODULAR BUILDING FOR 6 MONTHS,
 (TEMPORARY). THE 2010 WEST PALM BEACH AMENDMENTS TO THE
 FLORIDA BUILDING CODE, CHAPTER 1, 102.2 BUILDING. THE
 FOLLOWING BUILDINGS, STRUCTURES AND FACILITIES ARE
 EXEMPT FROM THE FLORIDA BUILDING CODE AS PROVIDED BY
 LAW, AND ANY FURTHER EXEMPTIONS SHALL BE AS DETERMINED
 BY THE LEGISLATURE AND PROVIDED BY LAW:
  
 (E) MOBILE OR MODULAR STRUCTURES USED AS TEMPORARY
 OFFICES, EXCEPT THAT THE PROVISIONS OF PART V (SECTION
 553.501-553.513, FLORIDA STATUTES) RELATING TO
 ACCESSIBILITY BY PERSONS WITH DISABILITIES, AND PERMITS
 SHALL BE REQUIRED FOR STRUCTURAL SUPPORT AND TIE DOWN,
 ELECTRICAL SUPPLY, AND ALL UTILITY CONNECTIONS TO SUCH
 MOBILE OR MODULAR STRUCTURES.
  
 NO PLANS WERE SUBMITTED FOR THE ACCESSIBLE PARKING
 SPACE, ACCESS AISLE, ACCESSIBLE ROUTE TO THE OFFICE,
 RAMP, STAIRS, LANDING, GAURDRAIL/ HANDRAIL SYSTEM.
 SEE ACC CODE, 405.1-10.
  
 2) IN LEU OF SHUTTERING THE MANUFACTURED BUILDING
 BECAUSE OF BEING LOCATED IN A WIND BORN DEBRIS REGION
 IF HEDRICK BROTHER WILL PROVIDE A LETTER ON THEIR
 LETTER HEAD STATING IF THE MANUFACTURED BUILDING IS
 STILL IN USE AND A HURRICANE WARNING OCCURS FOR THIS
 AREA THEY WILL EITHER MOVE THE UNIT FROM THE SITE OR
 APPLY PLYWOOD STORM PANELS OVER THE GLAZED OPENINGS.
