 ALL PERMIT APPLICATION PLAN REVIEWS WILL BE PERFORMED
 ELECTRONICALLY
 UPON APPLICATION AND FEE PAYMENT DESIGN DOCUMENTS
 SUBMISSION OPTIONS ARE AS FOLLOWS:
 1 UPLOAD VIA THE PROJECT DOX PORTAL WITH DIGITAL
 SIGNATURES OF DESIGN PROFESSIONALS AS APPLICABLE; USE
 THE REQUIRED NAMING CONVENTION FOR EACH DOCUMENT
 2 INCLUDE ON A CD IF DESIGN PROFESSIONAL DIGITAL
 SIGNATURES ARE PROVIDED AS REQUIRED; USE THE REQUIRED
 NAMING CONVENTION FOR EACH DOCUMENT
 3 INCLUDE ON A CD WITH ONE SET OF TRADITIONAL PAPER
 DESIGN DOCUMENTS SIGNED AND SEALED AS REQUIRED; USE THE
 REQUIRED NAMING CONVENTION FOR EACH DOCUMENT
 4 ONE SET OF TRADITIONAL PAPER DESIGN DOCUMENTS SUBJECT
 TO ADMINISTRATIVE FEES TO DIGITIZE THE DOCUMENTS
 ADDITIONAL INFORMATION WILL BE PROVIDED WHEN IT BECOMES
 AVAILABLE

 1) A LETTER FROM THE QUALIFIER OF BOTH THE PRIME/
 MASTER CONTRACTOR (HEDRICK BROTHERS) AND MEZZANINE
 CONTRACTOR MUST BE PROVIDED BEFORE THE MEZZINE PERMIT
 CAN BE ISSUED, IF THE MEZZANINE CONTRACTOR WISHES TO
 START WORK BEFORE THE SHELL / MASTER/ PRIME PERMIT WORK
 HAS RECEIVED A CERTIFICATE OFOCCUPANCY, AGREEING THEY
 WILL WORK TOGETHER TO ACHIEVE CODE COMPLIANCE. THESE
 DOCUMENTS WILL NEED TO BE SIGNED BY THE QUALIFIER.
 PLEASE PROVIDE THE PERMIT NUMBER, ADDRESS WITH SUITE
 NUMBER WITHIN THE DOCUMENT.
  
 2) THE SUBMITTED MEZININE PLANS WERE NOT REVIEWED BY
 THE DESIGNER OF RECORD. FL 61G1-23.015 (2) THE
 ARCHITECT IS RESPONSIBLE FOR SUPERVISING AND REVIEWING
 ALL PROJECT DATA, REPORTS, SHOP DRAWINGS ETC...
  
 3A) THE MEZZININE PLANS DO NOT SHOW ANY GUARD RAIL
 SYSTEM AROUND THE OPEN AREA AT THE PART DELIVERY AREA
 OR THE VLM. 2010 FBC-B 1013.1 WHERE REQUIRED.
 GUARDS SHALL BE LOCATED ALONG OPEN-SIDED WALKING
 SURFACES, INCLUDING MEZZANINES, EQUIPMENT PLATFORMS,
 STAIRS, RAMPS AND LANDINGS THAT ARE LOCATED MORE THAN
 30 INCHES (762 MM) MEASURED VERTICALLY TO THE FLOOR.
  
 3B) SHOW COMPLIANCE WITH; 1013.3 OPENING LIMITATIONS.
 EXCEPTIONS: 4. IN AREAS THAT ARE NOT OPEN TO THE PUBLIC
 WITHIN OCCUPANCIES IN GROUP I-3, F, H OR S, AND FOR
 ALTERNATING TREAD DEVICES AND SHIP LADDERS, GUARDS
 SHALL NOT HAVE OPENINGS WHICH ALLOW PASSAGE OF A SPHERE
 21 INCHES (533 MM) IN DIAMETER.
  
 WHEN RESUBMITTING PLANS PLEASE INDICATE THE REVISION &
 REMOVE ANY VOIDED SHEETS & REPLACE ANY PAGES AS
 NECESSARY. A TRANSMITTAL LETTER LISTING THE ORIGINAL
 REVIEW COMMENT NUMBER, WITH A DESCRIPTION OF THE
 REVISION MADE, IDENTIFYING THE SHEET OR SPECIFICATION
 PAGE WHERE THE CHANGES CAN BE FOUND WILL HELP TO
 EXPEDITE YOUR PERMIT. THANK YOU FOR YOUR ANTICIPATED
