 NO NOC IS NEEDED AS THE CITY IS RESPONSIBLE FOR PAYMENT
 OF THE WORK TO BE PERFORMED.
 CALLED CONTRACTOR AT 561-248-8068 AND LEFT MESSAGE ON
 VOICEMAIL REGARDING THE ABOVE INFORMATION. 8/11/15 3:14

 CALLED BRENDA AT FPL - LINE REMOVED ON 7/5/15. CALLED
 DAN KEMP TO DRIVE BY AND VERIFY. DAN VERIFIED 4:03 THAT
 LINE IS DEAD AT THE HOUSE AND DISCONNECTED AT THE BASE
 OF THE POLE.
 CALLED FPUC AT 800-427-7712. SPOKE TO ABEL IN CUSTOMER
 SERVICE. HE VERIFIED METER AND LINE TO STREET HAVE BEEN
 REMOVED.
 FOUND NOTE IN TOM MOORES REVIEW THAT SEWR LINE HAS BEEN
 REMOVED AND CAPPED.
 CUSTOMER SERVICE IN LOBBY VERIFIED WATER SERVICE HAS
 NOT BEEN ACTIVE FOR QUITE SOME TIME AND IS DISCONNECTED
 AT METER

 AND CALLED CONTRACTOR. MOVING ON TO ENG. FOR THEIR
 COMMENTS.
 I WILL NEED FOR BUILDING PLAN REVIEW TO PASS;
  
 1) FPL RELEASE
 2) FPU RELEASE
 3) WATER METER PULL REQUESTED TO WATER DEPARTMENT ON
 THEIR FORM.
 4) SEWER LATERAL CAPPING PERMIT PULLED, (TURNAROUND
 PERMIT), WITH FINAL #703 PASSED.
 5) EXTERMINATOR'S LETTER OF SITE INSPECTION FOR
 RODENTS, AND CONFIRMATION OF ELIMINATION IF FOUND.
 6) N.O.C.
 7) DEBRIS DISPOSAL FORM COMPLETED.
  
 AT OUR END;
 8) ENGINEERING REVIEW AND APPROVAL, LIKELY WILL REQUIRE
 SWPP PLAN.
 9) HISTORIC REVIEW STOP, IF OVERRIDE LETTER REQUIRED,
 (WE WILL HANDLE THAT).
  
 AFTER ISSUANCE;
 PREINSPECT TO REVIEW SIDEWALK CONDITION AND DISCUSS ANY
