| BE A COPY OF A SURVEY THAT IS NOT TO SCALE. THE BASE |
| SITE PLAN (OR SURVEY) SHALL BE TO SCALE WITH THE |
| WALKWAY INFORMATION DRAWN ON IT AT THE SAME SCALE AS |
| THE BASE SITE PLAN. SCALED DIMENSIONS SHALL BE INCLUDED |
| ON THE PLAN. |

| B. ADDRESSING WHY THE WALKWAY CANNOT BE A MINIMUM OF |
| SIX (6) FEET IN WIDTH TO ALLOW ADEQUATE ROOM FOR TWO |
| PEOPLE WITH BAGS CONTAINING WORKOUT GEAR TO PASS EACH |
| OTHER WITHOUT FORCING ONE OF THE PEOPLE INTO THE |
| VEHICLE TRAVEL LANE. |

| C. SPECIFIC RESPONSE WITH DETAILS ON HOW THE WALKWAY |
| WILL BE LIGHTED (MORE THAN JUST "LIT PER FBC 2010") AND |
| WHAT ADDITIONAL SECURITY PROCEDURES WILL BE IN PLACE. |
| THIS AREA IS ISOLATED AT NIGHT AND ON WEEKENDS WHEN THE |
| OFFICE TOWER IS CLOSED WITH NO NATURAL SURVEILLANCE |
| FROM INSIDE OR OUTSIDE OF CITYPLACE. SAFE SIGHTING |
| SHALL BE PROVIDED FOR THE ENTIRE WALKWAY, NOT JUST |
| AGAINST THE WALL. |

| THERE HAD BEEN FOLLOW UP DISCUSSIONS WITH THE MINOR PD |
| AMENDMENT APPLICANT AFTER STAFF NOTED THAT THE MELLOW |
| MUSHROOM IS USING THE DRIVE AREA ADJACENT TO THE |
| PROPOSED STRIPED ADA WALKWAY AS AN AREA FOR TAKEOUT |
| PICK UP. IT WAS CONVEYED TO THE MINOR PD AMENDMENT |
| APPLICANT THAT A TURNING RADIUS IS REQUIRED TO SHOW |
| THAT THERE IS SUFFICIENT AREA THAT A CUSTOMER TURNING |
| INTO THE PICKUP LANE WILL NOT ENCROACH INTO THE STRIPED |
| ADA WALKWAY. PLANS SHALL BE SHOWN TO SCALE. |

| 2) UPON SUBMITTAL OF PLANS COMPLYING WITH COMMENT NO. |
| 1, ZONING STAFF WILL PROVIDE A FULL PLAN REVIEW. STAFF |
| HAS NOTED THAT THERE IS A DIMENSION LINE INCLUDED FOR A |
| 21 FOOT DRIVE, WHICH INCLUDES A PORTION OF THE CURB. |
| CURBS MAY NOT BE SHOWN AS PART OF THE DRIVE AISLE. |

| 3) PLEASE PROVIDE A COPY OF A RECORDED CROSS ACCESS |
| AGREEMENT/EASEMENT FOR THE PORTIONS OF THE ACCESS AND |
| EGRESS ROUTES WHICH CROSS ONTO THE OFFICE TOWER PARCEL. |

| 4) SEPARATE PERMIT APPLICATIONS WILL BE REQUIRED FOR |
| THE WORK ON THE OFFICE TOWER PARCEL, TO INCLUDE WALKWAY |
| STRIPING AND LIGHTING. THE SAME PLANS MAY BE USED FOR |
| BOTH APPLICATIONS. |

| 5) THE NEW FENCING SHOWN ON THE SITE PLAN REQUIRES A |
| SEPARATE PERMIT, INCLUDING FENCE DETAILS. |
