 PLEASE ADDRESS THE FOLLOWING. RESPOND TO THE COMMENTS
 IN WRITING AND/OR ON THE PLANS WHERE APPLICABLE:
  
 1.) THERE IS AN EXISTING HAVERHILL COMMERCE PARK
 INDUSTRIAL PLANNED DEVELOPMENT (IPD) COVENANT THAT
 REQUIRES ARCHITECTURAL REVIEW AND APPROVAL FOR THE
 PROPOSED MONUMENT SIGN. AN APPROVAL LETTER FROM THE
 ARCHITECTURAL REVIEW COMMITTEE OF THIS IPD IS REQUIRED
 AS PART OF THE APPROVAL PROCESS.
  
 2.) THIS APPLICATION REQUIRES A MINOR PLANNED
 DEVELOPMENT (PD) AMENDMENT PRIOR TO PROCEEDING TO THE
 SIGN PERMITTING PROCESS. PLEASE SUBMIT AN APPLICATION
 FOR A MINOR PD AMENDMENT AFTER THE ARCHITECTURAL
 APPROVAL PROCESS HAS BEEN COMPLETED.
  
 3.) PROVIDE A FULL SIZE MEASURABLE SURVEY OF THE
 PROPERTY. THE LENGTH OF THE FRONTAGE IS NEEDED TO
 DETERMINE THE MAXIMUM ALLOWABLE SIGN AREA.
  
 4.) THE HEIGHT OF THE PROPOSED MONUMENT SIGN CANNOT
 EXCEED 10 FT. (ZLDR. S. 94-408(E))
  
 5.) PROVIDE A SITE PLAN SHOWING ALL SETBACK DIMENSIONS
 OF THE PROPOSED MONUMENT SIGN FROM THE PROPERTY LINE.
