 1. PERMIT DESCRIPTION STATES THAT THIS APPLICATION
 REPLACES EXPIRED PERMIT #07030842. SCOPE OF WORK FOR
 THE EXPIRED PERMIT INCLUDED DOORS, WINDOWS, FLOORING,
 CABINETRY, ETC. THIS PERMIT APPLICATION STATES "GENERAL
 REPAIRS." EITHER SUBMIT PLANS SHOWING COMPLETE SCOPE OF
 WORK CONSISTENT WITH THE EXPIRED PERMIT, OR SUBMIT A
 LETTER REQUESTING CANCELLATION OF THE EXPIRED PERMIT
 AND THE REFERENCE TO THE EXPIRED PERMIT WILL BE REMOVED
 FROM THE NEW PERMIT APPLICATION.
  
 2. IF THE REFERENCE TO THE EXPIRED PERMIT IS REMOVED
 FROM THIS PERMIT APPLICATION, AND "GENERAL REPAIRS" ARE
 PROPOSED, PROVIDE A PLAN SHOWING SCOPE OF WORK.
  
 3. FOR THE DOORS TO BE REPLACED, PROVIDE A PLAN SHOWING
 DOOR LOCATIONS INCLUDING DISTANCE FROM CORNER, FBC
 R301. ALSO INCLUDE MEAN ROOF HEIGHT. PLAN SHOULD
 INCLUDE DOOR CONFIGURATION; ARE SIDELIGHTS INCLUDED?
 THE PRODUCT APPROVAL SUBMITTED IS FOR AN OPAQUE DOOR
 WITH OR WITHOUT NON IMPACT SIDELITES.
  
 4. HISTORIC REVIEW REQUIRED AS THE PLANS PREVIOUSLY
 STAMPED BY HISTORIC ARE NO LONGER IN THIS DEPARTMENT.
 WHEN RESUBMITTING, PLEASE BE ADVISED THAT CONTRACTOR
 WILL BE REQUIRED TO OBTAIN HISTORIC REVIEW PRIOR TO
 RESUBMITTAL.
  
 5. CONTRACTOR CURRENTLY HAS APPROXIMATELY 36 EXPIRED
 PERMITS (U-17813, U-20912) IN THIS JURISDICTION. PLEASE
 PROVIDE A LIST OF PERMITS ALREADY ADDRESSED AND PROVIDE
 A PLAN TO SHOW HOW THE REMAINING EXPIRED PERMITS WILL
 BE ADDRESSED. FAILURE TO DO SO WILL RESULT IN FILING OF
 A COMPLIANT AGAINST THE CONTRACTOR'S LICENSE AND MAY
 PREVENT ISSUANCE OF FUTURE PERMITS. THIS IS
 CONTRACTOR'S THIRD WARNING. PLEASE CONTACT ME IF YOU
