 1) THE COVER SHEET INCORRECTLY LISTS THE PROPERTY OWNER
 AS FPL. THE OWNER OF THE THE PROPERTY IS THE VILLAGE OF
 PALM BEACH LAKES PROPERTY OWNER'S ASSOCIATION. THE POLE
 AND BASE LOCATION IS WITHIN AN FPL EASEMENT.
  
 2) THE COVER SHEET INCORRECTLY LISTS PALM BEACH COUNTY
 AS THE BUILDING DEPARTMENT AND ZONING IN THE PROJECT
 DATA BOX. THE SITE IS UNDER THE CITY OF WEST PALM
 BEACH'S JURISDICTION.
  
 3) THE MOUNTING OF THE NEW ANTENNA TO A POLE IS
 CONSIDERED A COMMUNICATION TOWER UNDER THE ZONING AND
 LAND DEVELOPMENT REGULATIONS. THE PROPERTY IS ZONED
 RESIDENTIAL PLANNED DEVELOPMENT, WHICH REQUIRES CITY
 COMMISSION APPROVAL PRIOR TO THE RESUBMITTAL OF THE
 BUILDING PERMIT. THE WIRELESS COMMUNICATION/PLANNED
 DEVELOPMENT AMENDMENT REVIEW APPLICATION IS AVAILABLE
 FROM THE PLANNING AND ZONING DEPARTMENT.
  
 4) A SURVEY OF THE SITE, LESS THAN ONE YEAR OLD IS
 REQUIRED. NOTE: A NEW BUILDING HAS BEEN BUILT AND
 ADDITIONAL LANDSCAPE INSTALLED THAT ARE NOT SHOWN ON
 THE SITE PLAN. THE PROPOSED ACCESS EASEMENT IS SHOWN
 THROUGH THE BUILDING AND THE UTILITY EASEMENT IS SHOWN
 THROUGH THE NEW BUILDING AND ITS REQUIRED LANDSCAPE.
 THE EASEMENTS MAY NOT OCCUR AT THESE LOCATIONS.
  
 5) PROVIDE THE FULL HEIGHT OF THE ANTENNA, THE NEW
 EQUIPMENT CABINETS AND ALL OTHER NEW ABOVE GROUND
 ITEMS. NOTE: THE MIDPOINT OF THE ANTENNA EXCEEDS THE
 HEIGHT ALLOWED WITHIN A RESIDENTIAL PLANNED DEVELOPMENT
 AND WOULD REQUIRE A WAIVER APPROVAL FROM THE CITY
 COMMISSION.
  
 6) ALL EQUIPMENT, CABINETS, THE GENERATOR, ETC. ARE
 REQUIRED TO BE SCREENED BY AN OPAQUE FENCE/WALL AND
 LANDSCAPE HEDGE TO THE FULL HEIGHT OF THE EQUIPMENT.
 CHAIN LINK IS NOT APPROPRIATE SCREENING MATERIAL. NOTE:
 IF THE EQUIPMENT ON THE PLATFORM REMAINS ATTACHED TO
 THE TOWER, THEN IT WILL NEED SCREENING TO ITS FULL 20
 FOOT HEIGHT.
  
 7) THE GENERATOR, THE OPAQUE FENCE/WALL AND THE
 LANDSCAPE HEDGE REQUIRE SEPARATE PERMIT APPLICATIONS
 PRIOR TO THE APPROVAL OF THIS PERMIT BY THE ZONING
 DIVISION.
  
 8) THE 15 FOOT WIDE ACCESS EASEMENT DOES NOT FOLLOW THE
 INGRESS/EGRESS LIMITS OF THE DRIVEWAY. THE ACCESS
 EASEMENT CAN NOT BE DESIGNED IN A MANNER TO FORCE
 VEHICLES TO DRIVE IN AN OPPOSITE DIRECTION LANE.
  
 9) THE LITE PLAN SHOWS THE PROPOSED ROUTE OF THE
 ELECTRIC AND TELCO SERVICE GOING THROUGH REQUIRED
 LANDSCAPE AREAS, SOME CONTAINING TREES AND SHRUBS.
 THESE NEW LINES SHALL BE INSTALLED UNDER THE PARKING
 AND DRIVEWAYS TO MINIMIZE LANDSCAPE CURRENT AND FUTURE
 LANDSCAPE CONFLICTS.
  
 10) ALL PLANS SHALL BE DRAWN TO SCALE AND INCLUDE THE
 SCALE AT WHICH THEY ARE DRAWN.
  
 11) BARBED WIRE IS NOT ALLOWED WITHIN RESIDENTIAL
 ZONING DISTRICTS. THIS MAY BE WAIVED BY THE CITY
 COMMISSION AS PART OF THE MAJOR AMENDMENT.
  
 12) REVISED PLANS MAY RESULT IN ADDITIONAL COMMENTS.
