 ATTACHED TO THE SECONDARY ADDRESS OF 3551 HAMILTON BAY
 RD (LAKE 3) ON PCN# 74424315030120000, SO I MOVED IT
 INSTEAD TO BE CORRECTLY REFERENCED TO 3301 BOLLARD RD
 ON PCN# 74424315020120000. I ALSO ADJUSTED THE ADDRESS
 TO 3301 BOLLARD RD ON THE ORIGINAL PERMIT# 09100560
 APPLICATION. PLEASE NOTE THAT THE SITE PLAN SHOWS THE
