 1515 SOUTH FLAGLER DRIVE
 PHASE I FOR PERMITTING
  
 1. ALL DEMOLITION ACTIVITIES SHALL COMPLY WITH NFPA
 241, THE STANDARD FOR SAFEGUARDING CONSTRUCTION,
 ALTERATION, AND DEMOLITION OPERATIONS.
  
 2. THE REMOVAL OF VERTICAL AND HORIZONTAL FIRE CUT-OFFS
 FOR THE BUILDING SHALL REMAIN UNTIL THE END OF THE
 DEMOLITION ACTIVITIES. BOTH EXIT STAIRS SHALL REMAIN
 CLEAR AND USEABLE DURING THE DEMOLITION ACTIVITIES.
  
 3. THE BUILDING DURING DEMOLITION ACTIVITIES WILL BE
 CONSIDERED NON-SMOKING AND NO SMOKING PLANS SHALL BE
 INSTALLED AND MAINTAINED DURING DEMOLITION ACTIVITIES.
  
 4. ALL INTERIOR SHAFTWAYS SHALL BE ADEQUATELY PROTECTED
 TO PREVENT PERSONS FROM FALLING INTO AN UNPROTECTED
 SHAFT. SHAFTS SHALL BE PROPERLY IDENTIFIED.
  
 5. GASOLINE POWERED GENERATORS SHALL NOT BE USED IN
 ENCLOSED AREAS OF THE BUILDING DUE TO THE POSSIBILITY
 OF CARBON MONOXIDE (CO) ACCUMULATION. CARBON MONOXIDE
 IS A COLORLESS, ODORLESS, GAS THAT DISPLACES OXYGEN.
 CARBON MONOXIDE CAN ALSO BE A FLAMMABLE MIXTURE IN THE
 CORRECT CONCENTRATION.
  
 6. THE EXISTING STANDPIPE SYSTEM SHALL REMAIN IN
 SERVICE AS THE INTERIOR OF THE BUILDING IS BEING
 DEMOLISHED. NO FIRE SPRINKLERS CURRENTLY EXIST AT THIS
 LOCATION.
  
 7. PORTABLE FIRE EXTINGUISHING EQUIPMENT SHALL BE
 PROVIDED DURING DEMOLITION ACTIVITIES.
  
 8. THE PLANS INDICATE THAT HANDRAILS AND GUARDRAILS ARE
 TO BE REMOVED. TEMPORARY HANDRAILS AND GUARDRAILS ARE
 NECESSARY IF THE PERMANENT ONES ARE REMOVED.
  
 9. EMERGENCY CONTACT NUMBERS ARE TO BE PROVIDED TO THE
 APPROPRIATE EMERGENCY SERVICES ORGANIZATIONS (ESO'S).
  
 10. TRASH AND DEBRIS SHALL BE REMOVED DAILY FROM THE
 SITE.
  
 11. DID NOT LOCATE ANY UTILITY COMPANY SIGN-OFFS
 RELATED TO THE DISCONNECTION OF SERVICES.
  
 12. A FIRE SAFETY PLAN AND PROGRAM SHALL BE DOCUMENTED
 TO THE APPROPRIATE EMERGENCY SERVICE ORGANIZATIONS
 (ESO'S)
  
 13. SUITABLE ACCESS WILL BE NECESSARY FOR THE
 APPROPRIATE EMERGENCY SERVICE ORGANIZATIONS (ESO'S)
  
 14. ALL EMERGENCIES ON THE SITE ARE TO BE REPORTED.
  
 15. A COMMAND POST NEEDS TO BE ESTABLISHED SHOULD ANY
 EMERGENCIES OCCUR ON THE SITE.
  
 16. ALL ASBESTOS RELATED MATERIALS SHALL BE REMOVED,
 HANDLED, AND DISPOSED OF IN ACCORDANCE WITH FEDERAL,
 STATE, AND LOCAL REQUIREMENTS.
  
 17. THE PROPOSED IMPLOSION SHALL COMPLY WITH NFPA 495,
 THE EXPLOSIVES MATERIALS CODE. AN INCIDENT ACTION PLAN
 (IAP) SHALL BE DEVELOPED FOR THIS ACTIVITY. A UNIFIED
 COMMAND SYSTEM AND INCIDENT COMMAND POST WILL BE
 ESTABLISHED FOR THIS ACTIVITY. ALL REQUIREMENTS OF THE
 VARIOUS CITY DEPARTMENTS SHALL BE MET PRIOR TO THIS
 ACTIVITY RECEIVING FINAL APPROVAL. INSURANCE
 CERTIFICATES NAMING THE CITY OF WEST PALM BEACH AND ITS
 AGENTS AS ADDITIONAL INSURED SHALL BE SUBMITTED AS PART
 OF THE OVERALL COMPREHENSIVE REQUEST PACKAGE.
 SUFFICIENT PERSONNEL FROM THE APPROPRIATE EMERGENCY
 SERVICE ORGANIZATIONS (ESO'S) WILL BE REQUIRED FOR
 EMERGENCY STANDBY ACTIVITIES PRIOR TO, DURING AND AFTER
 THE PROPOSED IMPLOSION SHOULD THIS ACTIVITY RECEIVE
 FINAL APPROVAL. ALL APPROPRIATE SAFETY PROCEDURES AND
 BENCHMARKS SHALL BE FOLLOWED.
  
 18. ADDITIONAL MEETINGS WILL BE NECESSARY FOR ANY
 PROPOSED IMPLOSION ACTIVITIES.
  
 19. PROPER CLEARANCES TO COMBUSTIBLE MATERIALS SHALL BE
 PROVIDED WHEN ANY CUTTING OR WELDING ACTIVITIES OCCUR.
