 ADDITION
  
 DENIED BY BUILDING
  
 1) THE WRONG ENERGY CALCULATION WAS SUBMITTED. THE FORM
 REQUIRED FOR THE 2004 FBC IS 600C-04. PLEASE SUBMIT TWO
 COPIES OF THE CORRECT FORM.
  
 2) THE PRESCRIPTIVE METHOD C REQUIRES R-5 INSULATION ON
 THE CONCRETE BLOCK WALLS. SEE THE ENERGY CALCULATION
 FORM. THE PLANS SPECIFY R-4.2. PLEASE CORRECT.
  
 3) SHOW COMPLIANCE WITH R408.1 VENTILATION.
 THE UNDER-FLOOR SPACE BETWEEN THE BOTTOM OF THE FLOOR
 JOISTS AND THE EARTH UNDER ANY BUILDING (EXCEPT SPACE
 OCCUPIED BY A BASEMENT OR CELLAR) SHALL BE PROVIDED
 WITH VENTILATION OPENINGS THROUGH FOUNDATION WALLS OR
 EXTERIOR WALLS. THE MINIMUM NET AREA OF VENTILATION
 OPENINGS SHALL NOT BE LESS THAN 1 SQUARE FOOT FOR EACH
 150 SQUARE FEET (0.67 M2 FOR EACH 100 M2) OF
 UNDER-FLOOR SPACE AREA. ONE SUCH VENTILATING OPENING
 SHALL BE WITHIN 3 FEET (914 MM) OF EACH CORNER OF SAID
 BUILDING.
  
 4) SHOW COMPLIANCE WITH R408.3 ACCESS.
 ACCESS SHALL BE PROVIDED TO ALL UNDER-FLOOR SPACES.
 ACCESS OPENINGS THROUGH THE FLOOR SHALL BE A MINIMUM OF
 18 INCHES BY 24 INCHES (457 MM BY 610 MM). OPENINGS
 THROUGH A PERIMETER WALL SHALL BE 16 INCHES BY 24
 INCHES (407 MM BY 610 MM). WHEN ANY PORTION OF THE
 THROUGH WALL ACCESS IS BELOW GRADE, AN AREAWAY OF NOT
 LESS THAN 16 INCHES BY 24 INCHES (407 MM BY 610 MM)
 SHALL BE PROVIDED. THE BOTTOM OF THE AREAWAY SHALL BE
 BELOW THE THRESHOLD OF THE ACCESS OPENING. THROUGH WALL
 ACCESS OPENINGS SHALL NOT BE LOCATED UNDER A DOOR TO
 THE RESIDENCE. SEE M1305.1.4 FOR ACCESS REQUIREMENTS
 WHERE MECHANICAL EQUIPMENT IS LOCATED UNDER FLOORS.
  
 5) FBC 106.3* CITY AMENDMENTS PRODUCT APPROVALS. THOSE
 PRODUCT WHICH ARE REGULATED BY DCA RULE 9B-72 SHALL BE
 REVIEWED AND APPROVED IN WRITING BY THE DESIGNER OF
 RECORD PRIOR TO SUBMITTAL FOR JURISDICTIONAL APPROVAL.
 PGT WINDOWS HAVE NOT BEEN REVIEWED BY THE ARCHITECT.
  
 6) IMPACT FEES MUST BE PAID TO PALM
 BEACH COUNTY, PLANS STAMPED BY THEM AND
 COPY OF RECEIPT SUBMITTED TO CITY OF
 WEST PALM BEACH BUILDING DEPARTMENT,
 BEFORE A BUILDING PERMIT CAN BE ISSUED.
  
 7) NOTE: A RECORDED COPY OF THE NOTICE OF COMMENCEMENT
 WILL BE REQUIRED.
