 1) CIVIL PLAN SHOULD HAVE BEEN REVIEWED AND APPROVED
 FIRST THRU: ENGINEERING SERVICES DEPARTMENT, 1000 -
 45TH STREET, SUITE 15, WEST PALM BEACH FL 33407. PLEASE
 SUBMIT UPDATED PLANS WITH WRITTEN EXPLANATION OF
 COMMENTS. WHEN CONTACTING THE REVIEWER PLEASE REFERENCE
 THE PERMIT NUMBER. IF YOU HAVE ANY QUESTIONS PLEASE
 FEEL FREE TO CONTACT THE REVIEWER: DEAN S. LAUDERMILK,
 PROJECT COORDINATOR.
  
 2) POLLUTION CONTROL PLAN IS MISSING, PLEASE SUPPLY
 PLAN FOR ALL AREAS TO BE EXCAVATED AND THE SURROUNDING
 CATCH BASINS. ONLY SOME DETAILS ARE SHOWN ON SHEET C9.
  
 3) SHEET C2:
 3A) I AM CONFUSED AS TO WHAT IS PHASE 1A AND PHASE 1B.
 THIS SHEET SHOWS THE LARGE AREA TO BE 1A, C3 SHOWS IT
 AS 1B, WHILE C4 SHOWS THE LARGE AREA TO AGAIN BE 1A AND
 THE SMALL 2/3 BUILDING AREA TO BE 1A. PLEASE CORRECT
 THE DRAWINGS AND SUBMIT AN EXPLANATION OF THE PHASING
 AND WHAT IS TO TAKE PLACE. AS YOU WILL READ IN LATER
 COMMENTS IT IS CONFUSING WHEN THE DIFFERENT PHASES WILL
 TAKE PLACE AND WHAT THEY INVOLVE.
 3B) NOT ALL OF THE REVIEWING STAFF UNDERSTANDS THAT THE
 FIRST FLOOR DOORS TO THE LIBRARY ARE IN LINE WITH THE
 DEMOLITION AND CONSTRUCTION. SO, AT SOME TIME, FOR
 SOMETIME, THE DOORS TO THE GROUND FLOOR OF THE LIBRARY
 WILL BE COMPLETELY BLOCKED. PLEASE SHOW THE DOORS AND
 EXPLAIN HOW ACCESS WILL BE MAINTAINED.
 3C) THIS PLAN SHOWS THE REMOVAL OF THE CATCH BASIN AND
 THE PIPING TO THE CATCH BASIN TO THE EAST. PLEASE
 EXPLAIN OR SHOW HOW THIS AREA THAT NORMALLY FLOODS WILL
 BE PROTECTED FROM FLOODING WHILE THE LIBRARY REMAINS
 OPEN. THE FLOOR OF THE LIBRARY IS THE LOWEST POINT IN
 THE AREA (4.8"+- PER SHEET C4).
 3D) SHOULDN'T THE LIMITS OF THE DEMOLITION ALSO INCLUDE
 THE AREA TO THE EAST TO THE EXISTING CATCH BASIN, SINCE
 THE STORM SEWER IS TO BE REMOVED TO THE EAST TO THE
 NEXT CATCH BASIN?
 3E) DURING LOW TIDE YOU CAN SEE THAT THERE IS A LARGE
 PIPE (PERHAPS 24") MUDDED UP GOING NORTH FROM THE
 EXISTING CATCH BASIN TO BE REMOVED, NOTE IT ON THE PLAN
 THAT THERE MAY STILL BE PIPE IN THE GROUND TO THE NORTH
 THAT NEEDS TO BE REMOVED.
 3F) THE PLAN SHOWS THE REMOVAL OF THE DRIVEWAY SOUTH OF
 THE SIDEWALK, THIS CONCRETE AREA HAS 3 DUMPSTERS ON IT
 AND IS SIGNED; "PERMIT PARKING FOR THE LIBRARY". PLEASE
 EXPLAIN WHERE THE DUMPSTERS ARE TO BE RELOCATED TO
 DURING THE TIME THE LIBRARY IS OPEN AND WHERE THE
 LIBRARY STAFF WILL PARK (THERE WERE TWO CARS IN THIS
 SPACE TODAY, 1-20-09) AND ONE OF THEM WAS FOR AN ADA
 EMPLOYEE).
 3G) REMOVE NOTE ON ALL PLAN SHEETS (EAST OF FP&L
 MANHOLE) "CONTRACTOR TO FIELD VERIFY W.M. SIZE AND
 LOCATION PRIOR TO ORDERING TAP". THE REASON IS
 EXPLAINED ON C12.
  
 4) SHEET C3:
 4A) NOTES ARE INCORRECT, REMOVE NOTES ON ALL APPLICABLE
 PLAN SHEETS AND REVIEW REASONS UNDER SHEET C12:
 4A.1) UNDER NOTE FOR EXISTING WATER SERVICE "TO BE
 CONSTRUCTED DURING PHASE 1A". ALSO REMOVE THE ARROW.
 4A.2) UNDER NOTE FOR EXISTING SANITARY SERVICE "TO BE
 CONSTRUCTED DURING PHASE 1A". ALSO REMOVE THE ARROW.
 4B) ADD NOTE "PROTECT EXISTING WATER SERVICE FROM
 EXISTING METER TO THE BACKFLOW PREVENTERS, AND THEN
 INTO THE LIBRARY." WATER METER SERVING BUILDING IS OUT
 BY THE LIGHT POLE WEST OF SITE, NOT PICKED UP BY
 SURVEYOR.
 4C) ADD NOTE "PROTECT EXISTING WATER SERVICE FROM
 EXISTING METER TO THE BACKFLOW PREVENTERS, AND THEN
 INTO THE EXISTING IRRIGATION SYSTEM." SECOND WATER
 METER SERVING IRRIGATION SYSTEM IS OUT BY THE LIGHT
 POLE WEST OF SITE, NOT PICKED UP BY SURVEYOR.
 4D) ADD NOTE: "PROTECT EXISTING SANITARY SEWER
 SERVICE." EXISTING SANITARY SEWER SERVICE CROSSES THRU
 THE SITE FROM THE MANHOLE OUT IN THE STREET THRU THE
 CLEAN-OUT IN THE STREET TO THE LIBRARY CLEAN-OUT AT THE
 ENTRANCE DOOR.
  
 5) SHEET C4:
 5A) THE LAY-OUT OF THE RAMPS SHOWN ON THE CIVIL DO NOT
 MATCH THE DESIGN SHOWN ON A-1.1. PLEASE CORRECT ONE OR
 BOTH DRAWINGS. I AM ASSUMING THAT A-1.1 IS CORRECT,
 WHICH WILL STILL NOT WORK.
 5B) THERE IS NOT ENOUGH ROOM ON THE LANDING IN FRONT OF
 THE BATHROOM DOORS (THIS SHEET SHOWS IT AS A RAMP), FOR
 THE DOORS TO OPEN OUT FROM THE BATHROOMS AND SOMEONE
 WILL NOT BE ABLE TO GET BY IN A WHEEL CHAIR. PLEASE
 CORRECT.
 5C) THE EXISTING DRIVEWAY CAN NOT BE UTILIZED.
 5C.1) EXPLAIN WHY THE DRIVEWAY IS REQUIRED, THERE IS
 MORE THAN ENOUGH SPACE FOR ON STREET PARKING.
 5C.2) AS LAID OUT, IF A TRUCK BACKS INTO THIS SPACE IT
 WILL RUN INTO THE BUILDING.
  
 6) SHEET C5:
 6A) EXPLAIN HOW LONG THE BATHROOMS (AND THE FOUNTAIN)
 WILL BE OUT OF SERVICE SINCE THE WOOD PLATFORM WILL
 NEED TO BE REMOVED AND THE CONCRETE RAMP AND PLATFORM
 BE BUILT?
  
 7) SHEET C6:
 7A) NONE OF THE DETAILS FOR STORMWATER ON THIS SHEET
 WILL BE BUILT. REMOVE IT.
  
 8) SHEET C7:
 8A) NONE OF THE DETAILS FOR STORMWATER ON THIS SHEET
 WILL BE BUILT. REMOVE IT.
  
 9) SHEET C8:
 9A) NONE OF THE DETAILS FOR STORMWATER ON THIS SHEET
 WILL BE BUILT. REMOVE IT.
  
 10) SHEET C9:
 10A) NO DETAILS FOR PROTECTING CATCH BASINS ARE SHOWN
 ON THIS SHEET, PLEASE ADD MORE POLLUTION CONTROL
 DETAILS.
  
 11) SHEET C12:
 12A) THERE IS NO NEED FOR A 4" TAP TO SERVE A 5/8"
 WATER METER. TAKE IT OFF THE PLAN. INSTEAD SHOW TYING
 INTO THE BACK OF THE EXISTING 2" WATER SERVICE TO THE
 LIBRARY AND SERVING THE BUILDING THRU THAT LINE. THE
 SURVEYOR DID NOT PICK IT UP BUT THERE IS A 2" METER BOX
 NEXT TO THE LIGHT POLE AT THE CURB, USE IT. ALSO SHOW
 ABANDONING THE REST OF THE PIPE INTO THE LIBRARY WHEN
 IT IS DEMOLISHED.
  
 12B) THERE IS NO NEED FOR A NEW SANITARY SEWER LATERAL,
 THERE IS AN EXISTING 6" SANITARY SEWER LATERAL THAT
 COMES FROM THE MANHOLE IN THE STREET THRU THE CLEAN-OUT
 IN THE STREET (SHOWN ON THE PLAN) TO THE LIBRARY. THERE
 IS AN EXISTING CLEAN-OUT JUST OUTSIDE THE DOOR TO THE
 LIBRARY. SHOW CUTTING A "WYE" INTO THE LINE TO SERVE
 THE NEW BUILDING AND SHOW ABANDONING THE REST OF THE
 PIPE INTO THE LIBRARY WHEN IT IS DEMOLISHED.
