 BUILDING WITH THE 2007 SUPPLEMENTS.
  
 3) 61G1-16.004 TITLE BLOCK.
 A TITLE BLOCK MUST APPEAR ON ALL ARCHITECTURAL OR
 INTERIOR DESIGN DRAWINGS AND SPECIFICATION
 IDENTIFICATION SHEETS. THE TITLE BLOCK MUST, AT A
 MINIMUM, CONTAIN THE FOLLOWING INFORMATION:
 (1) FIRM NAME, ADDRESS, AND TELEPHONE NUMBER.
 (2) FIRM LICENSE NUMBER.
 (3) NAME OR IDENTIFICATION OF PROJECT.
 (4) DATE PREPARED.
 (5) A SPACE FOR THE SIGNATURE AND DATED SEAL.
 (6) A SPACE FOR THE PRINTED NAME OF THE PERSON SEALING
 THE DOCUMENT.
  
 4) THE ADDRESS OF THE PROJECT SHALL BE INDICATED ON
 EACH SHEET INCLUDING THE SUITE NUMBER. THIS INFORMATION
 IS TO PROVIDE CONFORMITY TO THE DEPARTMENT RECORD
 RETENTION PROGRAM. FBC 106.1.3*
  
 5) THE APPLICATION INDICATES THE PROJECT AS AN INTERIOR
 REMODEL. ARE THERE ANY WALLS, DOORS OR ANY OTHER
 SYSTEMS BEING ADDED OR DEMOED? CONSTRUCTION DOCUMENTS
 SHALL BE OF SUFFICIENT CLARITY TO INDICATE THE
 LOCATION, NATURE AND EXTENT OF THE WORK PROPOSED. FBC
 106.1.1.
  
 6) IF THIS IS TRULY AN INTERIOR REMODEL, STATE THE
 LEVEL OF ALTERATION ON THE PLANS PER FBC (EXISTING)
 301.5 A DESIGN PROFESSIONAL OR AN OWNER MUST ELECT ONE
 OR A COMBINATION OF LEVELS OF ALTERATION PURSUANT TO
 SECTIONS 303, 304 AND 305 OF THIS CODE.
